52.212-2 ADDENDUM; EVALUATION--COMMERCIAL ITEMS

(a) The Government will award a contract resulting from this solicitation to the responsible Offeror whose quote conforming to the solicitation will the best value to the Government. Quotations will be evaluated using the following factors:

1. Previous Experience – A project is defined as: Work performed pursuant to one specific task order of an indefinite delivery/indefinite quantity (ID/IQ) type contract at one site or multiple sites at a single installation or facility or work performed pursuant to a site-specific contract for one site or multiple sites within a single installation or facility. An ID/IQ type contract or the performance of work pursuant to multiple task orders of an ID/IQ type contract does not represent a "project" within this definition. If the Offeror provides a specific task order as its "project," it should provide the base contract number and the task order number for reference purposes. If the Offeror provides a site-specific contract as its "project," it should provide the contract number for reference purposes. The projects must be paid to date at no less than 75% (as of the proposal due date) of the current project value within the last five years to be considered as a project.

Presentation of Previous Experience: The proposal shall include four different example crane projects that show experience on the following:

• Project 1: Should demonstrate Non-Destructive Testing (NDT).

• Project 2: Should demonstrate Load Testing.

• Project 3: Should demonstrate Crane control, troubleshooting and repairs.

• Project 4: Should demonstrate an additional task listed in PWS, Section 5.0.

If more than four projects are submitted, only the first four projects listed in the proposal will be evaluated.

Each project cited should be at least $15,000 in cost and have been completed within five (5) years from the date that the proposal for this project is due. Each project summary should consist of a narrative of the example project and should not exceed two (2) pages. If more than two pages are submitted, only the first two pages in the proposal will be evaluated. The government preference is that project examples submitted be of completed projects (i.e. construction complete).

Projects presented must have been executed in the contiguous United States to include Alaska, Hawaii, and U.S. Territories. The projects selected should show diversity in the types of work accomplished by the firm, and experience with a variety of contracting award options (such as, firm fixed price, performance based, etc.). For the specific purpose of presentation of previous experience, an 8.5”x11” sheet of paper shall be used per project. For this factor of the proposal, one side of an 8.5”x11” sheet of paper will be considered as one page. Both sides of the 8.5”x 11” may be used, however, each side will be counted as one page.

Projects presented should include the outline information. This is the Government’s recommended format, however, the Government will accept alternate formats provided all outlined information is addressed:

Project Name: Project Location: City and State, or military installation name and State, if appropriate)

Contract/Task Order number: Project Description: Narrative of each project should include a brief overview of the project and its relevance to this SATOC.

Offerors Project Manager:

Customer Representative: Include name, address, telephone, email, and fax number

Contracting Officer: Include name, business, address, telephone, email, and fax number

Contract Award Amount:

Final Contract Amount:

Original Contract Finish Date:

Actual Contract Finish Date:

Project Performance Issues: This could include cost/schedule, health and safety issues, and other performance issues.

The Government intends to evaluate the four (4) projects submitted by each Offeror to determine each Offeror's previous experience. Under this evaluation factor, the Government intends to evaluate the projects submitted with each Offeror’s proposal to assess the breadth, depth and relevance of each Offeror’s proposed experience with performing the crane, hoists and lifting devices requirements called for in this RFQ and as described in the PWS, Section 5.0.

To ensure that each Offeror has a better understanding of the areas that the Government intends to emphasize when assessing an Offeror’s relevant experience with performing work called for under this RFQ, the Government provides the following guidance concerning the assignment of proposal “strengths,” “weaknesses,” and “deficiencies.”

Significant Strengths/Strengths may be given for each the following criteria:

• Demonstrated experience working at hydropower, navigational or flood risk management facilities.

• Demonstrated experience with complex issues to include working over water.

• Demonstrated ability to keep project on schedule.

• Demonstrated experience working with the Federal Government.

• Demonstrate access to engineering services for crane component analysis, including crane capacity up-rating and design work for incidental repair.

• Demonstrate access to engineers on staff within Offeror’s company.

• Any other aspect of a proposal that the evaluator(s) and/or the KO believe enhances the merit of the proposal or increases the probability of successful performance of the contract.

Significant Weaknesses/Weaknesses may be given for each of the following criteria:

• A lack of breadth or depth in the projects submitted by:

o Not demonstrating a diversity of work in the different types of cranes.

o Not demonstrating a diversity of types of work (Rehabilitation, Inspection, etc) o Not demonstrating a diversity of work experience with different crane tonnage capacities.

• Not demonstrating projects related to the scope of work as laid out in Section C, Paragraph 2.3.

• Omission or incompleteness of any requested information listed in Section L – Paragraph 4.1. that is not otherwise stated as a deficiency.

• Any other aspect of a proposal that the evaluators and/or the SSA believe is a flaw that increases the risk of unsuccessful contract performance.

• Failure to demonstrate Non-destructive Testing (NDT) as a project example for Project 1.

• Failure to demonstrate Load Testing as a project example for Project 2.

• Failure to demonstrate Crane controls, troubleshooting and repairs for Project 3.

Deficiencies shall be given for each of the following criteria:

• Submitting fewer than four (4) project examples.

• Any project example submitted for which the Offeror was not the Prime Contractor or Joint Venture participant at the time of project execution; any project example submitted for which the Offeror was a subcontractor.

• Any project for which the Building Occupancy Date (BOD) (or date when services/repair was complete and system was operational) occurred more than five (5) years prior to the date that proposals are due.

• Any project example submitted should not be a collection of numerous Task Orders combined to meet requirements.

• Any other material failure of a proposal to meet a Government requirement or a combination of significant weaknesses in a proposal that increases the risk of unsuccessful contract performance to an unacceptable level.

2. Past Performance – The Offeror shall provide contact information and questionnaires for the past performance of four (4) projects within the past five years of crane inspections and repair services, the past performance information preferably a copy of CPARS, however a Past Performance Questionnaire (PPQ) is also acceptable for each project listed in the Offeror's experience in response to Volume 1, Factor I (Previous Experience) of this solicitation. The template for a PPQ is attached. Previously completed PPQs are acceptable. The title of the project shall be consistent with the project experience descriptions

Additionally, past performance may be based on the contracting’s officer’s knowledge of and previous experience with similar purchases acquired and any information contained in the Governmentwide Past Performance Information Retrieval System (PPIRS), contained at www.ppirs.gov.

The Offeror will receive a rating of exceptional for past performance that demonstrates successful completion of similar purchases on all references and the additional information retrieved from PPIRS and from the Contracting Officer is favorable. The Offeror will receive a rating of satisfactory for past performance that has at least one reference that demonstrates successful completion of similar purchases and the information from PPIRS and the Contracting Officer is favorable. The Offeror will receive a rating of unsatisfactory for past performance that does not demonstrate any successfully completed similar purchases on the references and the additional information from PPIRS and the Contracting Officer is not favorable. Offerors with no past performance history will be given a neutral rating. Finally, the Government places a higher value on projects, which were completed on time, within budget, and meets the project objectives.

http://www.ppirs.gov/

Price –the Quote will be evaluated for price reasonableness. Price evaluation will be based on the total price proposed for the “burdened rate, the escalation rate , and any other miscellaneous priced items.

Per FAR 15.404-1 (b), price analysis will be used to examine and evaluate the proposed price to determine if the offered price is fair and reasonable. The Government may use various price analysis techniques and procedures.

ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT

YOUR EVALUATION OF THE CONTRACTOR’S PERFORMANCE

RATING DEFINITION NOTE

(E) Exceptional Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.

An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO significant weaknesses identified.

(VG) Very Good Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.

(S) Satisfactory Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.

(M) Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.

A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.

(U) Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.

An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
