If an employee has ever been retested for an STS, he/she will receive an annual audiogram. NOTE: If an employee tests out with high threshold shift of 10dB or greater, he/she is to be retested. If the threshold shift remains 10dB or greater, the employee will be referred to an outside audiologist provider for evaluation.
3.Visual Screening: Tests with and without correction. Includes tests for right eye individually, left eye individually, both eyes together for near and far acuity, peripheral vision, depth perception, muscle balance, and color discrimination.
4.Urinalysis (UA): This test includes specific gravity tests for albumin, glucose, ketones, and bilirubin. (49 CFR 391.41 – Do this test for all DOT Drivers Certification Exams)
5.Pulmonary Function Test (PFT): This test Includes a forced expiratory capacity at one (1) second (FEV1) and forced vital capacity (FVC). If employee meets requirements for asbestos exposure, an annual PFT is required. Pre-Placement PFT required for respirator users, periodic (up-to 2 years) requires PFT at physician discretion, and is highly recommended for negative pressure respirators. Pre-Placement PFT is required for welders. ARC/TIG/MIG welders require periodic PFT per physician discretion and exposure based. Oxy/Acetylene Welder require annual PFT.

Chest X-ray: Posterior/anterior chest x-ray is to be accomplished when initially placed in MSP and interpreted by a radiologist as determined by position, past exposure and current exposure.
Additional chest x-ray may be required as determined by physician and specific exposures that warrant a look at the lungs. The health risk (due to radiation) is greater for an unnecessary chest x-ray than not getting a chest x-ray for some unknown reason. (See separate guidance for asbestos exposure) Posterior/anterior chest x-ray to be accomplished when initially placed in MSP.
Posterior/anterior chest x-ray to be interpreted by radiologist. Not to be accomplished routinely.
When abnormalities are noted on a PA x-ray a lateral x- ray should be taken. If problem seems to be occupationally related the cost will be borne by USACE. If not the patient should be referred to his private physician. Clinic must consult Corps point of contact before any tests other than those requested are performed. (CFR 29 1910.1001 - asbestos exposure guidelines) Asbestos questionnaire (initial or periodic); asbestos counseling. OSHA requires any employee having any current occupational asbestos exposure to be counseled regarding the dangers of smoking and asbestos.
Document asbestos counseling on form “USACE 5.
Electrocardiogram (ECG or EKG): A record of the electric potentials associated with the electric currents that traverse the heart that can be used to diagnose heart disease. The Standard 12-lead electrocardiogram contains three limb leads (I, II, and III), three augmented unipolar leads (aVr, aV1, and aVf) and six transverse precordial limb leads (V1-V6). Precordial electrode placement (V leads) must be consistent and patient position must be flat on the back. Technicians administering the ECG must be adequately trained in the procedure and all ECGs must be read by a physician, questionable ECGs are to be read by a specialist in internal medicine or a cardiologist. The physician reading the ECG must be familiar with the patient’s past history and physical examination results.
Frequency of Electrocardiogram (EKG): (Routine w/Interpretation & Report): Electrocardiogram (EKG). Baseline, then repeat at age 35, then every 5 years until age 50. At age 50+, an EKG to be done every 2 years. EXCEPTION-Age 40+ EKG done every 2 years if two or more risk factors for coronary artery disease (CAD) such as diabetes, smoking, family history of premature coronary artery disease, high blood pressure, high cholesterol, or when symptomatic. This authority is given to the clinic.
Pulmonary Function Test (PFT) Spirometric measurement of both force vital capacity (FVC) and time force expiratory volume (FEV 1.0) performed a minimum of 3 times (all three times must be shown in tests results) over a period of several minutes, looking for reproducibility and consistency as well as maximum effort. The corrected reading is to be compared (using Knudson Tables for comparison) with expected values for the person's age, sex, height, and race. Technicians or nurses performing PFT must be properly trained and/or have attended a NIOSH approved course in pulmonary function testing. Daily calibration checks must be performed. The fact that the calibration check was performed must be recorded on spirometric report form. All employees enrolled in the Asbestos Program will receive an annual Pulmonary Test. Asbestos (REF: 29 CFR 1910.1001) Asbestos: the PEL for asbestos (all asbestiform minerals) is measured for fibers greater than 5 micrometers in length. The PEL is 0.1 fibers per cc. Mandatory medical questionnaires applicable to surveillance of asbestos exposed persons are published in Appendix D of the OSHA asbestos standard 29 CFR 1910.1001. There is no legal requirement for medical monitoring after an employee is no longer exposed. USACE employees who have had previous asbestos exposure shall be offered an equivalent to the annual exam every five years.
a. Complete Asbestos Questionnaire (Initial or Periodic). Use only if above the permissible level of exposure. –
b. Asbestos counseling: OSHA requires any employee having any past exposure and is currently a smoker to be counseled regarding the dangers of smoking and asbestos.
c. Chest X-Ray (CXR) PA: If currently working with asbestos, refer to the following guidelines:
i. If 0-10 years since first exposure, any age is to receive a CXR every 5 years.
ii. If 10+ years since first exposure, and
iii. If age 15-35, receive a CXR every 5 years.
iv. If age 35+ - 45, receive a CXR every 2 years.
v. If age 45+, receive a CXR annually.
d. B-Reader is required on any CXR for asbestos medical surveillance. If field clinic does not have the capability to conduct the b-reader, then the CXR film is to be sent to the U. S.
Army Corps of Engineers (ATTN: CENWO-SO) for acquiring that service in Omaha. CXR, PA & Lateral may be requested by the agency, the examining physician or the Corps of Engineers. NOTE: Request chest x-ray film & all tests results be sent to the Corps at the same time. This is less confusing for our Health Unit when compiling data to accompany the film for obtaining B-Reader.
Urinalysis: Includes a quantitative evaluation for the presence of protein, glucose, ketones and blood, determination of pH, specific gravity, and a microscopic examination of the sediment. UA is part of the standard examination and a requirement for all CDL Exams.
Visual Screening: Perform test with and without glasses. Includes tests for right eye individually, left eye individually, both eyes together for both near and far acuity, peripheral vision, depth perception, muscle balance, and color discrimination. Visual Screening Requirements for Commercial Driver License (CDL).
Tuberculosis Skin Test (Mantoux-PPD): Administered and interrupted if occupationally exposed;
e.g., negative, not positive, but actual milimeter reading. (High risk for TB are those employees whose job involves direct contact with occupants of homeless shelters, migrant farm camps, prisons and jails, and some nursing homes, a TB Skin Test is warranted on an annual basis; If a person’s job duties require him/her to ride in a vehicle with prisoners on a frequent basis, then a TB Skin Test on an every 6-month basis is warranted). For exceptions to this, contact CENWO-SO. Please check first with CENWO-SO. At present, we have only a few employees that require the TB Test.
Diphtheria Tetanus Booster (Td): Required every 10 years for occupational risk exposures.
Benzene: a semi-annual physical examination is required with a history that includes emphasis on disorders of the blood and blood-forming organs. Laboratory tests, including CBC with white cell differential count, RBC with indices (MCV, MCH, MCHC), platelet count, reticulocyte count, and serum bilirubin shall be performed. Following an emergency exposure of an employee to benzene, he or she should have a urinary phenol test collected and performed at the end of their shift. If the phenol level is greater than 75-mg phenol per liter of urine, a complete blood count shall be obtained.
Lead: semi-annual biological exposure monitoring (BEM) shall be provided to personnel exposed above the lead action level (½ the PEL) for more than 30 days per year. This will include blood sampling and analysis for lead and zinc protoporphyrin levels. Annual physical examinations shall include blood lead level, a complete blood count with WBC differential and examination of blood cell morphology on the peripheral smear, RBC indices, reticulocyte count, lead and zinc protoporphyrins in the urine, urinalysis with microscopic examination of the sediment, serum creatinine and blood urea nitrogen (BUN) levels
DOT Certification Examinations are required every 2 years for all employees where a DOT certificate is required to perform their work with the Corps. The commercial driver’s license (CDL) must be a requirement outlined in the employee’s position description. Unless the CDL is listed in the position description, the CDL exam WILL NOT be provided. Urinalysis Test is performed for all employees in the MSP getting a comprehensive exam (a person receiving only an audiogram would not get this test). Vision Exam. Vision screening with & without glasses must be performed. DOT examinations are required every other year. Employees should wear their newest prescription glasses or contact lenses for the vision screening. If the exam indicates that a new prescription is needed, the employee is responsible for obtaining a new prescription from an optometrist or ophthalmologist.
Chest X-ray: 14”x17” posterior/anterior chest film to be accomplished when initially placed in MSP and at exit exam. 14” x 17” Posterior/anterior chest film to be interrupted by radiologist. When abnormalities are noted on a PA film, a lateral x-ray should be taken. If problem seems to be occupationally related, the cost will be borne by USACE. If not occupationally related, the patient should be referred to his private physician. Initial and exit exam only. At discretion of physician if needed to monitor exposure to job-related conditions.
Red Blood Cell Cholinesterase: Every year if Corps employee is mixing and spraying organicphosphate & carbinate pesticides and herbicides. This is not required if Corps employee is only performing functions as observer, and contractor is performing function. This test should be performed before spraying begins, again at middle of season, and again 2 weeks after spraying is completed to determine if a decrease in cholinesterase has occurred.
Hepatitis A: For employees whose job duties bring them into contact with raw sewage or other potentially infectious materials at or above the PEL. Preventive treatment of Hepatitis A involves a series of two injections over a period of six months. For adults, a small amount of vaccine fluid is injected into the army muscle initially, and is followed by a booster does six months after the first injection. Coordinate first with CENWO-SO before scheduling with clinic.
Hepatitis B: For employees whose job duties involve an inherent potential for mucous membrane or skin contact with blood, body fluids, or tissues or a potential for spills or splashes. Vaccinations consist of 3 doses & booster. Initial Dose, Dose 2 (1 month after Dose 1), Dose 3 (6 months after Dose 1). Coordinate first with CENWO-SO before scheduling with clinic.
