SECTION J - LIST OF DOCUMENTS, EXHIBITS AND OTHER ATTACHMENTS

The following have been modified:

SAMPLE PROJECT SOW

Sample Project Statement of Objective

1. Background. Percifield AFB (fictional) is located in Umatilla County, Oregon.

2. Requirements. As a result of historic activities at Percifield AFB, Munitions and Explosives of Concern (MEC) and Munitions Debris (MD) were discovered in a trench. The Offeror shall implement an Interim Response Action to remove and properly dispose of the contents of the trench. The Offeror will be required to remediate the sites to allow for unlimited use and unrestricted exposure (UU/UE). The Offeror shall execute the Decision Documents process with final approvals by the Oregon Department of Environmental Quality (ODEQ).

3. Statement of Work. The Offeror will be required to prepare draft, draft final and final work plans for US Army Corps of Engineers (USACE), DoD and ODEQ approval as well as all other documents needed for the Interim Response Action (IRA). As part of the IRA, the Offeror shall conduct confirmation sampling and site restoration. Upon completion of the IRA, the Offeror shall prepare the documents needed to complete final closure of the site.

4. Details of Munitions Trenches. The Offeror shall remove all MEC/MD from the munitions trench and check the area around the munitions trench for possible additional trenches or buried MEC/MD. The munitions trench is assumed to be 15 feet wide x 1000 feet long and 15 feet deep. The area around the munitions trench is 40 acres and will need to have an instrument assisted surface clearance over the entire area prior to a Digital Geophysical Mapping (DGM) that will produce a dig list of the entire area for intrusive investigation.

The Offeror shall propose the method for confirmation sampling in the trench area and the area around the munitions trenches that will satisfy the site to UU/UE and explain the reasoning in the proposal. The Offeror shall restore the area to the appropriate grade that ensures flow of surface water away from any structures and by planting a seed mix similar to what is in the area.

5. Assumptions. For the purpose of preparing a price proposal, the following assumptions shall be used. NOTE: Assumptions may not include all work efforts necessary to complete a task. Offerors should include all costs necessary to complete each item.

a. Historical records indicate the following munitions may be buried within the munitions trenches: 105 mm HEAT projectiles (practice), 120 mm HEAT projectiles (practice), 30 mm cartridges, 5” Zuni rockets, 90 mm AP projectiles, AIM-9 [AM0001] (Sidewinder) warheads (practice), BDU-33 practice bomb, BLU-106 and BLU-107 (Durandal warhead).

b. The Offeror shall anticipate 1 acre of vegetation removal to support removals and up to 30 acres to support surface clearance and DGM.

c. No road construction will be required to access the project burial trench site.

d. The Offeror shall anticipate up to 5 BIPs and 15 consolidated demolition shots.

e. The Offeror shall assume up to 250 tons of MD requiring recycling from the burial trench sites.

f. The Offeror shall assume 2 intrusive investigation excavations per acre from the area around the munitions trenches and 10 pounds of MD requiring recycling per acre from the area around the munitions trenches.

g. A commercial offsite borrow source is available (assume a 5 mile one way haul distance).

h. Groundwater is located at 50ft bgs and previous sampling has not identified any contamination.

i. A Quality Assurance Surveillance Plan (QASP) requires confirmation samples to be below residential levels for the State of Oregon.

j. There are no underground utilities within the project area.

k. The Offeror shall state any additional assumptions in their proposed plan.

l. Offerors shall assume travel from Omaha, NE to Umatilla County, OR (assume

$650.00 per person for roundtrip airfare) and per diem expenses are required at this location for all on-site personnel.

m. Offerors should assume the source of the material in the trench is unknown.

n. Offerors should assume terrain is open grassland.

o. Offerors should assume a slope factor as per soil common to Umatilla County, OR.

p. Offerors should assume that vegetation exists that is common in Umatilla County, OR.

q. Offerors should assume that the two intrusive investigations per acre are for discrete anomalies.

r. Offerors should assume the borrow source is purchased off-site at market rate for borrow in Umatilla County, OR and borrow requires compaction after placement.

s. Offerors should assume 50% of the soil in the trench is contaminated and 50% is reusable.

t. Assume no special status species are present or potentially present at the site.

u. Assume no surface water feature at the site and assume no wetlands.

v. Assume the Decision Document is NFA DD after the site is cleaned up.

w. Assume that drums will be encountered at the unknown location.

x. Assume the offeror will need to establish an area for processing MPPEH.

y. Assume a seed mix native to Umatilla County, OR.

z. Assume no impact to groundwater from the leaking drums.

aa. Assume that borrow soils will require sampling and analysis.

bb. Offerors should assume the area in uninhabited and there is a gravel, hard packed access road to the site. The nearest inhabited building is 10 miles away.

cc. Offerors should assume no controlled access.

dd. Offerors should assume there are no cultural/natural resources requiring additional efforts/support

ee. Assume this is a removal action under CERCLA

ff. Assume this is NOT a DMM trench.

**For all remaining questions, offerors should assume worse case MEC scenario and make assumption based on what is given in the scenario and professional judgment.

6. Price Proposal. The Government will evaluate for fair and reasonableness IAW FAR 15.404-1. The Offeror shall submit a detailed price estimate for implementing the proposed Interim Action for the sample project, including the costs for associated plans, reports and closure documents. The proposal shall include burdened labor rates, number of hours, mix of labor categories, equipment and material costs, subcontractor costs (if appropriate),and profit. Prior to implementation of the Interim Action, the Offeror shall prepare a work plan that includes a technical approach including, at a minimum, site safety and health, quality control and quality assurance. To develop the schedule, assume USACE and DoD will review all Draft documents within 30 days and the ODEQ will review all Draft Final documents within 30 days and comments will need to be addressed. Assume any utilities required are located on site and will be provided by the government to the contractor at no cost.

7. During the investigation phase of the Interim Action. During the investigation phase it was discovered that there may be up to 3-55 gal drums of motor oil buried in the munitions trench. The Offeror shall assume that there are 3-55 gal drums of motor oil in the trench. Assume the discovery was made prior to project plans being drafted. Assume the drums to be leaking. The detailed price estimate needs to include the cost to clean up the drum area of all residual contamination that was left by the leaking drums to UU/UE and perform confirmation sampling of the area to assure the ODEQ that all the contamination was removed.

Assumptions:

1. This is a onetime find within the trench.

2. All documents shall include the execution of this effort.

Requirements:

1. Explain the sampling procedure that is going to be accomplished to assure that all the contamination created by these drums have been found.

2. Explain the sampling procedure that is going to be accomplished to assure that all the contamination created by these drums has been cleaned up.

3. Explain how the contractor will handle the disposal of all the contaminants.

4. All costs for this effort, with supporting detail shall be provided for USACE evaluation.

NOTE: The CLIN Schedule is provided as a guide for evaluation purposes only. If the offeror determines there is no cost associated with a CLIN or CLINS, indicate $0 for Item Amount.

4.0 Volume I - Technical Proposal and Past Performance Information:

Proposals submitted in response to this solicitation shall consist of the contents required in the sub-paragraphs described below:

4.1. Volume I – Technical Requirements

4.1.1. Volume I, Factor I –Previous Experience – Environmental Remediation Services w/ Military Munitions Response Program

A project is defined as: Work performed pursuant to one specific task order of an indefinite delivery/indefinite quantity (IDIQ) type contract at one site or multiple sites at a single installation or facility or work performed pursuant to a site-specific contract for one site or multiple sites within a single installation or facility. An ID/IQ type contract or the performance of work pursuant to multiple task orders of an ID/IQ type contract does not represent a "project" within this definition. If the offeror provides a specific task order as its "project," it shall provide the base contract number and the task order number for reference purposes. If the offeror provides a site-specific contract as its "project," it shall provide the contract number for reference purposes. The projects must be paid to date at no less than 75% (as of the proposal due date) of the current project value within the last five years to be considered as a project.

Presentation of Previous Experience: The proposal shall include six (6) environmental remediation services projects, to include at least three (3) MMRP projects and three (3) HTRW projects that have been executed by the offeror and its team members as either a prime contractor or a subcontractor. Projects presented must have been executed in the contiguous United States to include Alaska, Hawaii, and U.S. Territories. Small arms range projects will not be accepted as demonstration of MMRP capabilities. The projects selected should show diversity in the types of work accomplished by the firm, experience with a variety of contracting award options (such as, firm fixed price, performance based, etc.), teaming arrangements, key personnel and their roles on the project, regulatory information, in addition to basic cost information. For the specific purpose of presentation of previous experience, an 11”x17” sheet of paper shall be used per project. For this factor of the proposal, one side of an 11”x17” sheet of paper will be considered as one page. Both sides of the 11”x 17” may be used, however each side will be counted as one page.

The following information should be included as a minimum in the presentation of previous experience:

Box A: Project #: Name of project and location (City and State, or military installation name and State, if appropriate)
