Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs.

Offerors may provide details on past performance problems and corrective actions taken as a part of the proposal. When an Offeror’s firm is divided into severable segments (e.g., division, group, unit) the Government will evaluate only the past performance of those segments of the firm that will actually perform the work. Recency Assessment. To be recent, the effort must be ongoing or must have been performed during the past three (3) years from the date of issuance of this RFP. Information that fails the recency assessment will not be evaluated.

Relevancy Assessment. To be relevant, the effort must be similar to the nature of work, size, magnitude, complexity and scope to the work required by this RFP. The Government will conduct an in-depth evaluation of all recent performance information provided to determine if it is the same or similar in nature, size, and complexity to the services/products being procured under this RFP. In determining relevancy for individual contracts, consideration will be given to the effort, or portion of the effort, being proposed by the offeror, teaming partner, or subcontractor whose contract is being reviewed and evaluated. Relevant experience information that fails this condition will not be evaluated.

The Government reserves the right to obtain past performance data from other sources than those identified by the offeror in evaluating past performance. This includes, but not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting

System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, and Fee Determining Officials; and the Defense Contract Management Agency. Offerors are reminded that both independent data and data provided by Offerors in their proposals may be used by the Government to evaluate offeror past performance. However, the burden of providing thorough, complete, and current past performance information as requested in this paragraph remains with the Offerors.

Proposals that do not contain the information requested by this paragraph risk rejection or a less than acceptable performance rating by the Government. All past performance comments received will be taken into account and could affect the overall rating. The overall past performance evaluation is a subjective decision based on the whole of all data received. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.

While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.

I. INTRODUCTION

Navy Medicine West (NMW) Detachment Hawaii is responsible for the programming, planning, construction and initial outfitting (IO) of U.S. Navy Medical Treatment Facilities. NMW is chartered with outfitting the new Medical/Dental clinic to meet mission objectives, while transitioning staff and resources from the existing point of care delivery locations on Marine Corps Base (MCB) Hawaii. The terms NMW, Navy Medicine West Detachment Hawaii, and NMW Project Officers may be used interchangeably within this document.

This SOW describes the IO services, procedures and personnel necessary to outfit the new Medical/Dental Replacement Clinic on Marine Corps Base (MCB) Hawaii.

The new Medical/Dental Replacement Clinic is currently under construction approximately one (1.5) miles from the existing MCB Hawaii Branch Health Clinic (BHC) Kaneohe Bay, located on Harris Avenue. Items from the BHC Kaneohe Bay, 21st Dental Battalion, Regimental Aid Station, CLB-3 Aid Station, and Wing Flight Line clinic selected for reuse shall be relocated to the new Medical/Dental Replacement Clinic by the IO Contractor.

Development of the IO requirements and IO integration into the project shall be accomplished concurrently with construction of the Medical/Dental Replacement Clinic

II. INITIAL OUTFITTING OVERVIEW

Navy Medicine West defines Initial Outfitting (IO) as any commodity-related planning, procurement, installation, certifying, transition, training and warranting required to equipment a new or newly renovated facility.

Initial Outfitting is a two-step acquisition process. The IO Contractor is ultimately responsible for providing Step 1 IO-T, and Step 2 IO-C, throughout the duration of the project.

The proposed Medical/Dental Replacement Clinic IO-T and IO-C planning dates as well as the Beneficial Occupancy Date (BOD) and Open for Business (OFB) dates are outlined below.

These dates are for information purposes only and are subject to change.
