About this file

This document is a SAM Form 36 (April 2021 version) Questionnaire for Non-Sensitive Positions issued by the U.S. Army Corps of Engineers, Mobile District. The form is specifically designed for background investigations of potential campground attendants and volunteers, with the primary purpose of determining an individual's suitability for government service. The form requires detailed personal information and includes important legal notifications about providing accurate information, including potential penalties for false statements under U.S. Criminal Code (fines up to $10,000 and/or 5 years imprisonment).

The form covers critical administrative elements such as volunteer time period (less than or more than 6 months), U.S. citizenship verification, contact information, and an authorization allowing investigators to obtain background information. The document emphasizes that information collection is governed by the Privacy Act and is protected from misuse. Volunteers must provide personal details like name, date and place of birth, phone numbers, and email, and must sign an authorization allowing a one-year background investigation by U.S. Army Corps of Engineers representatives.
