PERFORMANCE WORK STATEMENT (PWS)

For

HEADQUARTERS UNITED STATES MILITARY ENTRANCE PROCESSING COMMAND

(HQ USMEPCOM)

DATA CENTER CLEANING SERVICES

PART 1

GENERAL INFORMATION

1. GENERAL: This is a non-personnel services contract to provide data center cleaning and maintenance services for the establishment of a safe and clean environment to HQ USMEPCOM Data Center, with onsite cleaning and maintenance. The government shall not exercise any supervision or control over the contract service providers performing the services herein as outlined in table 1. Such contract service providers shall be accountable solely to the contractor who, in turn is responsible to the government.

1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary at HQ USMEPCOM (DATA CENTER) as identified in this PWS. The contractor shall perform to the standards in this contract.

1.2 Background: The United States Military Entrance Processing Command is the vital link between recruiting and training today's armed forces. The command's motto, "Freedom's Front Door" symbolizes the command's mission of determining the physical, mental and moral qualifications of every member of the armed services.

USMEPCOM, with headquarters in North Chicago, Ill., is a joint service command staffed with people from all five branches of service. The command, through its 65 Military Entrance Processing Stations (MEPS), determines whether applicants are qualified for enlistment based on standards set by each of the services.

Currently USMEPCOM data center does not have a current service agreement to maintain a clean and safe environment for reliable computing operations to meet the needs to successfully provide real-time entrance processing and qualification services to the acession community, and other government agencies thereby putting future entrance processing time, and qualification background checks at risk by severely impeding the number of applicants that can be processed into the Armed Services.

Therefore, the main goal of USMEPCOM is to establish an ongoing cleaning and maintenance program to ensure that the data center is at all times a clean, healthy, and a safe environment to work in by eliminating dust and d i r t buildup, thus ensuring a robust and reliable connectivity in support of USMEPCOM and our Accession Partners while protecting USMEPCOM systems and networks to maximize mission assurance.

1.3 Objectives: Cleaning and Maintenance Service for HQ USMEPCOM Data Center.

The work to be performed under this Task includes:

• Systematic vacuuming to remove dirt, dust and contaminants

• Antistatic cleaning and machine scrubbing of high pressure laminate raised floor surfaces

• Damp wiping of environmental equipment and counter surfaces

• Performance Work Statement (PWS). The contractor shall coordinate all efforts of this PWS with the designated government contracting officer representative (COR).

• All activities under this PWS shall be performed in compliance with best commercial business practices and/or current Government standards, DoD and Army regulations and Federal Acquisition Regulation (FAR) requirements as appropriate, and the International Standard Class 8 ISO 1464 https://www.iso.org/obp/ui/#iso:std:iso:14644:-1:ed-1:v1:en for clean spaces.

1.4 Scope:

1.4.1 Prior to the first cleaning, management and government personnel shall perform a walk through to gather specific cleaning requirements. Requirements range from which floor tiles will be removed for cleaning to which electrical outlets are designated as available for use by cleaning personnel. Detailed notes and procedures are then developed for use by cleaning staff and presented to the government program manager (GPM), and government contracting officer representative (COR) for review and approval.

1.4.2 This effort will require that all technicians have completed a comprehensive training program in data center cleaning, IT clean room, and IT cleaning services.as required by the International Standard Class 8 ISO 14644-1 https://www.iso.org/obp/ui/#iso:std:iso:14644:-1:ed-1:v1:en for clean spaces. For example, technicians are trained to never spray cleaning chemicals directly onto equipment and to use disposable wiping cloths treated with anti-static property chemical.

1.4.3 Data center cleaning supplies shall be chosen based upon specific criteria as identified during the initial walkthrough. All tools, equipment, supplies, and chemicals used are specifically designed and recommended for data center cleaning and data center environments. All equipment used will ensure the data center will be cleaned to the International Standard ISO 14644 for class 100,000 clean spaces. (Refer to part 1. 1.3 bullet number 7)

1.4. Safety cones, tape, and wet floor signs are used where appropriate to protect client data center personnel and cleaning staff. All lose items shall be secured and never left on floors.

1.5 Period of Performance: The period of performance shall be for one (1) Base Year of 12 months and three (3)12- Month option years.

1.6 General Information

1.6.1 Quality Control: The contractor shall develop a proactive QCP for measuring and attaining quality of performance under this contract. The contractor's QCP shall explain the manner in which the contractor shall ensure all contract requirements are being accomplished IAW specifications of this contract and industry standards. A sustaining focus throughout the QCP shall be the attainment of continuous quality improvement. The program shall emphasize deficiency prevention over deficiency detection. The contractor shall demonstrate a concerted effort in improving its program to prevent unsatisfactory performance from consistently repeating in any single contract functional area and to ensure unsatisfactory performance is addressed and rectified in a timely manner The contractor's quality control program shall be submitted within 30 days after contract award or with the contractor's proposal, three copies of a comprehensive written QCP shall be submitted to the KO and COR within 5 working days when changes are made thereafter. After acceptance of the quality control plan the contractor shall receive the contracting officer's acceptance in writing of any proposed change to his QC system.

1.6.2 Quality Control Plan: The QCP shall include the use of the Interactive Customer Evaluation (ICE) system to receive and track customer comments/complaints. The contractor shall enter any customer comments/complaints into the ICE system that is collected in a manual or other process external to ICE. The program shall allow identification and correction of validated customer complaints and provide feedback to the government and customers on corrective action(s) taken. The contractor shall not directly enter replies to customers. This function will be performed by government personnel serving as COR, QAE, or others as designated by local command policy. The contractor shall have access to comments and replies posted in ICE.

The term customer refers to customers internal and external to organizations identified by this contract.

1.6.3 The QCP shall include a customer comments and complaint program and quantitative processing system.

The program shall allow identification and correction of validated customer complaints, and provide feedback to the government and customers on corrective action(s) taken. The term customer refers to customers internal and external to the organizations identified by this contract.

1.6.4 Quality Control Files: The contractor shall maintain a file of all inspections or tests conducted by the contractor, to include any corrective actions taken. This file shall be subject to KO review. The file shall be the property of the government and shall be turned over to the government upon completion or termination of the contract.

1.6.5 Recognized Holidays: Contract shall be available after hours and holidays to perform emergency service/repairs when notified by MIT-CSD Government Program Manager (GPM) at no additional fees to the Government. However, for special projects it may be necessary to conduct activities during this period to avoid unnecessary system outages during normal business hours. Proper coordination shall be made with the Contracting Officer's Representative (COR) to ensure that USMEPCOM personnel are available to support and allow access.

Legal Federal holidays:

New Year's Day Labor Day Martin Luther King Jr.'s Birthday Columbus Day President's Day Veteran's Day Memorial Day Thanksgiving Day Independence Day Christmas Day

When such holidays fall on a Saturday, the preceding Friday shall be considered a holiday. When such holidays fall on a Sunday, the following Monday shall be considered a holiday.

1.6.6 Hours of Operation: The contractor is responsible for conducting business, during USMEPCOM normal duty hours, Monday through Friday, including week-ends, except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. For other than firm fixed price contracts, the contractor will not be reimbursed when the government facility is closed for the above reasons. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential.

1.6.7 Place of Performance: The Contractor shall support data center cleaning and maintenance services at the HQ USMEPCOM, which is located in the Continental United States (CONUS) as required by USMEPCOM and within the limitations of labor hours within the contract. The Contractor shall provide data cleaning services as listed in this PWS for the initial contract period and all exercised contract options. Service includes all labor, material, parts, tools, equipment, supplies, and chemicals for the performance of this contract. The contractor shall bear all travel expense in supporting the data center cleaning and maintenance services to HQ

USMEPCOM.

1.6.8 Type of Contract: The government will award a Firm Fix Contract.

1.6.9 Security Requirements: Contractor personnel performing work under this contract must be in accordance with Army Regulation 25-2, Army Regulation 380-67, and DoD Directive 5200.8, there are certain categories of positions or duties, which, although not requiring access to classified information, if performed by unworthy persons, could enable them to jeopardize the security of USMEPCOM or otherwise endanger the national security.

The investigative requirements for personnel occupying Information Systems Positions designated IT-I, IT-II and IT-III are as follows:

IT-I: SSBI (Single Scope Background Investigation), IT-II: NACLC (National Agency Check with Local Credit Check) and IT-III: NAC (National Agency Check). This investigation is required for all contractor personnel working on and off-site under this task. The request for investigation shall be submitted by the Contractor's Facility Security Officer to OPM before the start of the task, in accordance with DoD5220.22-M, Chapter 2.at time of the proposal submission, and must maintain the level of security required for the life of the contract. The security requirements are in accordance with the attached DD254.

The work is SENSITIVE but UNCLASSIFIED. Each task in this PWS shall require at a minimum an IT-II NACLC investigation for all positions. NACLC (National Agency Check with Local Credit Check) is required.

The procedure for contractors to submit security investigation requests through the Contractor's Facility Security Officer before the start of the task is as follows:

a. Contractor company shall notify the COR/Contracting Officer's Representative of new personnel or at the start of a new task. If personnel are in JPAS (Joint Personnel Adjudication System) indicating the individual has a previously completed investigation that allows him/her to work at the level of the contract requirements, then all that is required from the contractor/vendor company is a completed DD Form 2875/SAAR for that individual to be submitted to the USMEPCOM J-6 COR along with a Visit Access Request (see below).

b. If the contractor does NOT have a completed investigation and is NOT in JPAS, it is the Contractor Company's responsibility to submit the proper information to OPM to initiate a background investigation through the Contractor's Facility Security Officer. No contractor shall be permitted to begin work on any task until a favorable investigation is completed on that individual. This process may take 30-90 days or longer. The Contractor shall bear the cost of any security clearances required for order performance

c. Verification Access Roster/Visit Access Request (VAR).

Any Contractor management or contractor who requires access to USMEPCOM on either a regular or an occasional basis shall provide an initial or updated VAR to the USMEPCOM J-6, IAO specifying those employees who require access. The initial VAR shall be provided not later than one week in advance of arrival. A VAR shall be completed once per contract option period or when any significant information changes. The information on this document shall include: name of individual who requires access, reason for access, social security number, place and date of birth, clearance level and investigation date, and country of citizenship.

1.7 Physical Security: The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.

1.7.1 Key Control. The Contractor shall establish and implement methods of making sure all keys/key cards issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons.

NOTE: All references to keys include key cards. No keys issued to the Contractor by the Government shall be duplicated. The Contractor shall develop procedures covering key control that shall be included in the Quality Control Plan. Such procedures shall include turn-in of any issued keys by personnel who no longer require access to locked areas. The Contractor shall immediately report any occurrences of lost or duplicate keys/key cards to the Contracting Officer.

1.7.2 In the event keys, other than master keys, are lost or duplicated, the Contractor shall, upon direction of the Contracting Officer, re-key or replace the affected lock or locks; however, the Government, at its option, may replace the affected lock or locks or perform re-keying. When the replacement of locks or re-keying is performed by the Government, the total cost of re-keying or the replacement of the lock or locks shall be deducted from the monthly payment due the Contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the Contractor.

1.7.3 The Contractor shall prohibit the use of Government issued keys/key cards by any persons other than the Contractor's employees. The Contractor shall prohibit the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the Contracting Officer.

1.7.4 The Government shall be NOT responsible for safeguarding Contractor's equipment, and materials including loss, damage and thief. At the close of each work period, contractor's equipment, and materials shall be secured by the contractor.

1.8 SITE ACCESS:

a. Upon entering the Government facility, all personnel employed by or representing the Contractor in the performance of this contract shall abide by all USMEPCOM security procedures, which are in effect. No Contractor employee will be permitted on the installation if his/her presence is deemed by the activity Security Guard, J-1/MEFR or J-6/MEIT management to be detrimental to the security of the USMEPCOM facility.

b. Contractor personnel shall not be permitted to enter USMEPCOM's Enterprise Data Center (EDC) or USMEPS without proper identification. Upon entry into HQ MEPCOM or any MEPS facility, after verification that the contractor employee is on the EDC Access List and in the presence of a government employee escort, the individual will be issued a temporary identification badge by the government. While within the EDC or MEPS facility, all personnel are required to wear and display the USMEPCOM identification badge on the outside and above the waist of all outer clothing.

c. The contractor shall establish and implement methods of ensuring that badges issued by the USMEPCOM are not lost. The contractor shall report the loss of any government issued identification at the first opportunity to the MEIT Security Manager, J-1/MEHR, or the Government Program Officer's point of contact (POC),

d. It shall be the responsibility of the contractor to prohibit and prevent the use of card key badges issued by the government by any persons other than the individual to whom the card key badge was issued. It is also the responsibility of the contractor to prohibit and prevent the opening of locked areas by the contractor's employees to permit entrance of persons other than the contractor's employees engaged in the performance of assigned work in those areas.

e. When an employee leaves the Contractor's service, all Government issued identification cards, passes, and badges shall be turned over to the USMEPCOM COR or J-1/MHER Security Manager immediately upon termination of employment.

1.6.8 Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal

Acquisition Regulation Subpart 42.5. The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

1.9 Contracting Officer Representative (COR): The (COR) will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract:

perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.

1.10 Key Personnel: The follow are considered key personnel by the government:

The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the contracting officer. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager or alternate shall be available between Insert the hours, i.e., 8:00 a.m. to 4:30p.m., Monday thru Friday, including week-ends during network cutovers except Federal holidays and when the government facility is closed for administrative reasons. Qualifications for all key personnel are listed below:

1.11 Identification of Contractor Employees: All contract personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. They must also ensure that all documents or reports produced by contractors are suitably marked as contractor products or that contractor participation is appropriately disclosed. All contractor personnel will be required to obtain and wear badges in the performance of this service.

1.12 Organizational Conflict of Interest: Contractor and subcontractor personnel performing work under this contract may receive, have access to or participate in the development of proprietary or source selection information (e.g., cost or pricing information, budget information or analyses, specifications or work statements, etc.) or perform evaluation services which may create a current or subsequent Organizational Conflict of Interests (OCI) as defined in FAR Subpart 9.5. The Contractor shall notify the Contracting Officer immediately whenever it becomes aware that such access or participation may result in any actual or potential OCI and shall promptly submit a plan to the Contracting Officer to avoid or mitigate any such OCI. The Contractor's mitigation plan will be determined to be acceptable solely at the discretion of the Contracting Officer and in the event the Contracting Officer unilaterally determines that any such OCI cannot be satisfactorily avoided or mitigated, the Contracting Officer may affect other remedies as he or she deems necessary, including prohibiting the Contractor from participation in subsequent contracted requirements which may be affected by the OCI.

1.13 Contract Personnel: The Contractor shall provide a sufficient number of trained personnel to perform the duties and services specified herein. The Contractor shall be responsible for maintaining the standards of employee competence, conduct and integrity. The Contractor shall ensure that the same requirements and standards applicable to the Contractor are maintained by any/all Sub-Contractors. Should the continued assignment to this contract of any person in the Contractor's organization be deemed by the Contracting Officer to conflict with the interests of the Government, that person shall be immediately removed from the assignment, and the reason for removal will be fully documented in writing by the Contracting Officer.

1.14 Key Personnel: Key personnel are those personnel considered essential to successful contractor performance.

The Contractor shall designate a Senior Task Manager and one (1) Team Leader under this contract. Key technical personnel shall have an in-depth understanding and are thoroughly trained in computer room training facilities, and the ability, knowledge, experience, and skills to perform those requirements as per ISO standards https://www.iso.org/obp/ui/#iso:std:iso:14644:-1:ed-1:v1:en for clean spaces. Employee roster shall be provided initially and upon change. The Contractor shall provide written notification (Employee Roster) to the KO, COR and government Security two weeks prior to replacing or reassigning key technical personnel.

1.15 The successful completion of this Task is, in part, dependent upon close cooperation between USMEPCOM personnel and Contractor personnel. Maintaining established schedules requires responsiveness in providing functional expertise, guidance, information, review of deliverables, and copies of, or access to, publications and documentation. USMEPCOM personnel will be made available as may be required by Contractor personnel. All government resources and documentation shall be returned to the USMEPCOM GPM upon completion of this contract.

1.16 All employees of this contract must be U.S. Citizens and shall speak, read, write and understand the English language fluently.

1.17 Contractor personnel shall at all times and in all conditions be cordial, respectful and business like during all telephone and personal conversations.

1.18 The Contractor is prohibited from hiring "Off Duty" Government personnel and/or active duty military personnel or other personnel whose employment conflicts with Joint Ethics Regulation 5500.7-R.

1.19 The Contractor shall comply with all applicable regulations and laws.

1.20 Attire. Contractor personnel shall wear clothing commensurate with that worn by their counterparts in the civilian business community. Contractor personnel shall not wear undergarments as outer garments, transparent or loose weave clothing without underclothing; any ornament or symbol on the clothing expressing derogatory, lewd, profane or obscene words, gestures, connotations or drawings; swimsuits; any ornament, insignia, or symbol on the clothing implying disrespect for the United States, the United States flag, or military service; any item advocating or glorifying the use of illegal drugs or other unlawful conduct; cut-offs or short shorts.

1.21 Smoking Policy. Contractor personnel shall comply with smoking policies in federal buildings. Smoking is prohibited within 50 feet of any exit or entrance to the building. Contractor personnel shall place cigarette butts in proper containers and shall not dispose of butts at or near entrances and/or exits to the facility. USMEPCOM provides a designated "smoke shack" outside the west entrance of Building 3400.

1.22 Identification. Per published DoD rule (FR Volume 76, Number 87) all contractor employees shall identify themselves as contractor personnel by introducing themselves or being introduced as contractor personnel and displaying distinguishing badges or other visible identification for meetings with Government personnel. In addition, contractor personnel shall appropriately identify themselves as contractor employees in telephone conversations and in formal and informal written correspondence.

1.23 Employment and staffing difficulties will not be justification for failure to meet established schedules, and if such difficulties impair performance the contractor may be subject to default on the contract.

1.24. Employee relationship. The services to be performed under this contract do not require the contractor or his/her personnel to exercise personal judgment and discretion on behalf of the government. Rules, regulations, and requirements that are issued by the U.S. Army under its responsibility for good order, administration, and security are applicable to all personnel who enter the government installation, government leased property, or travel on government transportation. This is not to be construed or interpreted to establish any degree of government control that is inconsistent with a non-personal services contract.

1.25 Inapplicability of employee benefits. This contract does not create an employer-employee relationship.

Accordingly, entitlements and benefits applicable to such relationships do not apply. (1) Payments by the government under this contract are not subject to Federal income tax withholdings. (2) Payments by the government under this contract are not subject to Federal Insurance Contributions Act. (3) The contractor is not entitled to unemployment compensation benefits under the Social Security Act, as amended, by virtue of performance of this contract. (4) The contractor is not entitled to workman's compensation benefits by virtue of this contract. (5) The entire consideration and benefits to the contractor for performance of this contract are contained in provisions for payment.

1.26 Notice. It is both the contractor's and government's responsibility to monitor contract activities and notify the contracting officer if the contractor believes the intent of this contract has been or may be violated. The contractor should promptly notify the contracting officer in writing, within seven business days from the date of any incident that the contractor considers to constitute a violation. The notice should include the date, nature and circumstance of the conduct; the name, function and activity of each government employee, contractor official, or employee involved or knowledgeable about such conduct; identify any documents or substance of any oral communication involved in the conduct; and the estimated time by which the government must respond to this notice to minimize cost, delay or disruption of performance. The contracting officer will promptly, within seven business days after receipt of notice, respond in writing. In responding, the contracting officer will either:

(1) Confirm that the conduct is in violation and when necessary direct the mode of further performance; (2)

Countermand any communication regarded as a violation;

(2) Deny that the conduct constitutes a violation and when necessary direct the mode of further performance; or

(4) in the event the notice is inadequate to make a decision, advise the contractor what additional information is required, and establish the date by which it should be furnished by the contractor and the date thereafter by which the government will respond.

1.27 Privacy Act

1.27.1 This contract shall be subject to the Privacy Act and all the rules and regulations of USMEPCOM.

Contractor personnel shall have access to information subject to the Privacy Act. Although information that the contractor may be privileged to, during the course of completing this task, is unclassified, such information shall not be released by any contract person to any outside sources for any reason.

1.27.2 The contractor shall not disclose, or release to other than government authorized persons or activities, the content of any government software, procedures or information provided to the contractor. The contractor personnel shall have experience working with and safeguarding Privacy Act data. The contractor agrees to comply and assumes responsibility for compliance by its employee as stated in DFAR clause 252.204-7000 Disclosure of Information, unless-

1. The Contracting Officer has given prior written approval;

2. The information is otherwise in the public domain before the date of release; or,

3. The information results from or arises during the performance of a project that has been scoped and negotiated by the contracting activity with the contractor and research performer and determined in writing by the contracting officer to be fundamental research in accordance with National Security Decision Directive 189, National Policy on the Transfer of Scientific, Technical and Engineering Information, in effect on the date of contract award and the USD (AT&L) memoranda on Fundamental Research, dated May 24, 2010, and on Contracted Fundamental Research, dated June 26, 2008, (available at DFARS PGI 204.4).

1.27.3 Requests for approval under paragraph (a) (1) shall identify the specific information to be released, the medium to be used, and the purpose for the release. The Contractor shall submit its request to the Contracting Officer at least 10 business days before the proposed date for release.

1.27.4 The Contractor agrees to include a similar requirement, including this paragraph (c), in each subcontract under this contract. Subcontractors shall submit requests for authorization to release through the prime contractor to the Contracting Officer.

1.27.5 All work shall be performed under the supervision of the contractor or the contractor's responsible management personnel.

1.27.6 Any USMEPCOM information made available shall be used only for the purposes of carrying out the provisions of this contract and shall be treated as confidential and shall not be divulged or made known in any manner to any person except as necessary in the performance of the contract. Disclosure to anyone other than an officer or management of the contractor shall require prior written approval.

PART 2

DEFINITIONS & ACRONYMS

2. DEFINITIONS AND ACRONYMS:

2.1. DEFINITIONS:

2.1.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

2.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

2.1.3. CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

2.1.4. DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.

2.1.5. DELIVERABLE. Anything that can be physically delivered, but may include non-manufactured things such as meeting minutes or reports.

2.1.6. KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.

2.1.7. PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.

2.1.8. QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.

2.1.9. QUALITY ASSURANCE Surveillance Plan (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.

2.1.10. QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.

2.1.11. SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.

2.1.12. WORK DAY. The number of hours per day the Contractor provides services in accordance with the contract.

3.1 GOVERNMENT FURNISHED ITEMS AND SERVICES:

3.1.2. Utilities: Normal usage of water and electricity for companies providing this type of service.

3.2.1 General: The Contractor shall furnish all supplies, equipment, facilities and services required to perform work under this contract that are not listed under Section 3.1 of this PWS.

Part 4

CONTRACT DELIVERABLES:

4.1 The contractor shall provide a business letter to the government POC indicating the primary maintenance POC with associated telephone number and address within 10 working days of contract award. The contractor shall also provide an alternate POC in the event the primary POC is not available. The contractor shall update the names and telephone numbers as changes occur. Answering machines are not acceptable.

4.2 Technical Reports. The Contractor shall ensure all deliverables meet the requirements set forth in this PWS. All deliverables shall be provided to USMEPCOM in hard copy and soft/electronic copy (CD or DVD) forms. The hard copy shall be provided unbound and printed on one side. The software will be compatible with USMEPCOM's Command standard, which is currently the following:

a. Text - MS Word 2013

b. Spreadsheets - MS Excel 2013

c. Graphics/drawings - PowerPoint, Visio, AutoCAD

4.3 Semi-Annual Status Report (SASR). The Contractor shall provide a Semi-Annual status report (if service has been provided), listing work/support accomplished each week to USMEPCOM. These reports shall be sent via e-mail to the Contracting Officer Technical Representative (COTR) to support USMEPCOMs Significant Actions report.

4.4 Walk Thru Reports. The Contractor shall provide walk thru reports for each trip to include technical site assessment visits, anticipated delivery is 15 working days after completion of each on-site assistance visit or trip.

4.5 Trip Reports. N/A

4.6 Monthly Progress Report (MPR). N/A

4.7 The GPM/ITS/COR has the right to perform inspection and acceptance or recommendation for rejection of Contractor deliverables and identify deficiencies in delivered items. This does not replace any other quality assurance inspection requirements that are specified elsewhere within this PWS. The Contractor will be notified in writing by the GPM/COR through the Contracting Officer (KO) of the specific reasons why the work or part is being rejected. If the work cannot be resolved, deficiencies of a minor technical nature shall be evaluated by the GPM/ITS/COR and the KO to determine its significance that could lead to rejection of deliverables.

4.8 The repair shall be performed with proper and industry standard service, with a high level of attention to detail.

4.9 Correspondence: To promote timely and effective administration, correspondence shall be subject to the following procedures:

a. Technical correspondence (where technical issues relating to compliance with the requirements herein) shall be addressed to the Contracting Officer's Representative (COR) with an information copy to the Contracting Officer (CO - KO).

b. All other correspondence, including invoices, (that which proposes or otherwise involves waivers, deviations or modifications to the requirements, terms or conditions of this SOW) shall be addressed to the Contracting Officer with an information copy to the COR.

5. Specific Task:

5.1 Basic Services: The Contractor shall provide Data Center Cleaning and Maintenance Services for the establishment of a safe and clean environment to HQ USMEPCOM Data Center, with onsite cleaning and maintenance. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.

5.2 Prior to the first cleaning, Management and Government personnel shall perform a walkthrough to gather specific cleaning requirements. Requirements range from which floor tiles will be removed for cleaning to which electrical outlets are designated as available for use by cleaning personnel. Detailed notes and procedures are then developed for use by cleaning staff and presented to the Government Program Manager (GPM), and Government Contracting Officer Representative (COR) for review and approval.

5.3 The contractor shall HEPA vacuum the raised floor surface to remove loose particulate prior to removing floor tiles (floor tile is Laminate Square with wood core encased with still on bottom of square).

5.4 The contract shall lift a limited number of floor tiles to provide underfloor plenum access.

5.5 The contract shall manually remove large debris and contaminants.

5.6 The contract shall HEPA vacuum the subfloor slab to remove dust and debris.

5.7 The contract shall HEPA vacuum and wipe clean floor stringers, pedestal heads and large mechanical components.

5.8 The contract shall HEPA vacuum all floor surfaces, including accessible areas beneath hardware devices, to remove loose particulate.

5.9 The contract shall safely damp mop all floor surfaces, including accessible areas beneath hardware devices, wipe clean the subfloor slab to remove fine dust and residue that vacuuming alone will not remove.

5.10 The contract shall HEPA vacuum, damp wipe and spot scrub exterior surfaces of equipment cabinets, racks and stand-alone equipment, e.g. mainframes, tape libraries.

5.11 The contract shall HEPA vacuum, damp wipe and spot scrub exterior surfaces of support equipment, e.g. CRACs, PDUs.

5.12 The contract shall HEPA vacuum, damp wipe and spot scrub exterior surfaces of furniture and desktop equipment, e.g. tables, storage cabinets, monitors.

5.13 HEPA Vacuum, Damp Wipe and Spot Scrub major infrastructure components, e.g. interior windows, window sills, wall mounted hardware.

5.14 The contract shall HEPA vacuum, damp wipe and spot scrub exterior surfaces of overhead cable trays.

5.15 Task Heading.

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services under this contract for the HQ MEPCOM via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/, and click on "Department of the Army CMRA" on the icon of the DoD organization that is receiving or benefitting from the contracted services.

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2016.
