MISSION DIVISION

COMBINED SYNOPSIS/SOLICITATION

1. This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Items,” as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued.

2. Solicitation number W9124D-18-Q-6292 is issued as a Request for Quotation (RFQ). If appearing in this solicitation, the terms “RFP” shall mean “RFQ”, the words “proposal” or “offer” shall mean “quote”, the word “offeror” shall mean “quoter” and the word “contract” shall mean “purchase order”.

3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular {2005-95, effective 24 January 2018}. See http://farsite.hill.af.mil/vffara.htm for this information.

4. The associated NAICS code is 532210 and the size standard of $38.5M.

INSTRUCTIONS TO QUOTERS

Submitting your quote. Quotes shall be submitted via email to the Contracting Specialist, john.p.haarala.civ@mail.mil by 1100 am Fort Knox, KY Local Time on Tuesday, 20 March 2018. No other method of submission is acceptable. Attachments are limited to 5 MB per email.

Quotes should be in either Adobe or Microsoft Office format. Quoters are advised that the Government may be unable to receive other types of electronic files (e.g., .zip files).

Submission deadline. We will not consider any quote that we receive after the deadline unless we receive it before we issue a purchase order and considering it will not delay our purchase. In case of an emergency that delays our operations and makes submission or receipt of your quote impossible, we will extend the deadline accordingly.

Terms and content of your quote. Your quote must be based on the terms of this RFQ. Government may reject any quote that is not based on these terms in every respect. Your quote must contain all of the information described below and be valid for 30 days.

Small Business Status. In order to submit a quote, you must know whether your company is a small business, this requirement is 100% small business set-aside. The small business size standard that applies to this purchase is stated in paragraph 4 above.

Issuance of purchase order. Your quote should contain your best terms. The Contracting Officer may reject any or all quotes. After the evaluation of quotes, the Contracting Officer may negotiate final terms with one or more quoters of the Government’s choice before issuing a purchase order. The Contracting Officer will not negotiate with any quoters other than those of the Government’s choice and will not use the formal source selection procedures described in FAR Part 15.

The Contracting Officer may issue a purchase order to other than the quoter with the lowest price.

Award will be made in the aggregate to the responsive, responsible offeror submitting a quote that meets the specifications in the Performance Work Statement contained herein. Past performance from FAPISS will be reviewed.

Quotes shall contain the following:

1. Quote Cover Letter that provides your company’s Name, Address, Point of Contact, phone number, CAGE Code, DUNS Number and tax ID number. In order to be considered for award, all contractors must have active registration in System for Award Management (SAM) website and identified as small business under NAICS code listed in paragraph 4 above.

2. Price Quote that provides a firm-fixed-price quote for the refrigerators requested in CLIN and subsequently the Scope of Work. In addition to an extended price, all quotes must contain a daily rate.

3. Technical Quote that confirm that your refrigerators meet all of the minimum technical requirements in the Scope of Work. Maximum page length is two pages, with minimum font size of 10 points. At the bottom of the technical quote, explain any exceptions taken to the requirements in this solicitation. Note that exceptions to the RFQ shall render the quote unacceptable to the Government.

4. References to current or former customers who will attest to the quality of your product or service in the within the past three years, including (i) customer name, (ii) customer address, (iii) contract or purchase order numbers, (iv) persons to contact, and (v) telephone numbers.

Questions. Questions regarding this RFQ shall be submitted in writing by 1100am, 20 March 2018 via e-mail to John Haarala at john.p.haarala.civ@mail.mil. All questions must be submitted in writing.

252.232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (MAY 2013)

(a) Definitions. As used in this clause--

Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.

Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system.

(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall--

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov ; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/ .

(e) WAWF methods of document submission. Document submissions may be via Web entry, Electronic Data Interchange, or File Transfer Protocol.

(f) WAWF payment instructions. The Contractor must use the following information when submitting payment requests and receiving reports in WAWF for this contract/order:

(1) Document type. The Contractor shall use the following document type(s).

(2) Inspection/acceptance location. The Contractor shall select the following inspection/acceptance location(s) in WAWF, as specified by the contracting officer.

(4) Payment request and supporting documentation. The Contractor shall ensure a payment request includes appropriate contract line item and subline item descriptions of the work performed or supplies delivered, unit price/cost per unit, fee (if applicable), and all relevant back-up documentation, as defined in DFARS Appendix F, (e.g. timesheets) in support of each payment request.

(5) WAWF email notifications. The Contractor shall enter the email address identified below in the “Send Additional Email Notifications” field of WAWF once a document is submitted in the system.

Scope of Work 78th Training Division

Refrigerated Trailers (Reefers)

LOCATION: Camp Atterbury, IN 46164

SPECIFICATIONS:

• Ten (10) Walk in Refrigerators Containers (units)

• Size 8 X 20 or 20L x 8W x 8’6 Equipped with loading ramp in the rear of the unit

• Diesel powered OR /Electric-powered Integrated Power Source or Generator powered

• Operating temperatures range from 35°F (2°C) to -10°F (-23°C) or lower and up to 40 degrees Fahrenheit (2-3 degrees Celsius) Remote Gauge to monito temperature inside the refrigeration unit

• Pre-wired, pre-assembled, and ready to operate

• Contractor shall provide maintenance

• Government will make arrangements with contractor for delivery location

• Lockable door latch

• Contractor shall provide, one (1) #5 ABC type fire extinguisher for each Walk in Refrigerators Containers (units). All extinguishers shall be inspected to ensure they are in proper operating condition.

REQUIREMENTS:

The ten (10) Reefers will be delivered to multiple training sites:

- 3 x 20 foot reefer trailers with power source at FOB Night Hawk in Camp Atterbury at 3008 Old Hospital Rd. Edinburgh, IN 46124

- 2 x 20 foot reefer trailers with power source at FOB Panther at N Base Rd. North Vernon, IN 47265

- 2 x 20 foot reefer trailers with power source at Jennings County Fairgrounds 4950 North State Highway 3 North Vernon, IN 47265

- 3 x 20 foot reefer trailers with power source (Class I Yard/TAA Warrior) in Camp Atterbury at 3008 Old Hospital Rd. Edinburgh, IN 46124

• Delivery date NLT 04 April 2018

• Pick up date NLT 28 April 2018

• Government must be permitted to re-locate containers, at will

• Vendor shall provide “as needed” repairs, as well as preventative maintenance on units or replace unit if repairs are not made within a timeframe as to prevent food spoilage.

See below specification for any issue pertaining to maintenance. Contents of the refrigerated containers is perishable, the contractor shall ensure prompt and swift action to correct any malfunction or any equipment to ensure all perishables remain properly refrigerated.

The units shall be on site, ready to go no later 04 April 2018. This is a turn-key requirement and the refrigerated units must all be delivered for the entire event before 04 April 2018.

Period of Performance and Operating Hours Period of performance for this contract shall be from 04 April 2018 to 28 April 2018. Contractor is required to perform services during the term of the contract, to include during all Federal, State, and local holidays.

Installation Hours of Operation. Unless otherwise specified, entrance and exit from the installation is no earlier than 07:30am and no later than 4:00pm.

The government representative will coordinate gate entry with installation guards.

All Walk in Refrigerators Containers (units) shall be available in operable condition no later than 04 April 2018.

Any down time shall be deducted from the award's amount.

A government representative will be appointed to the awardee at the time of award. Coordinating instructions regarding delivery/pick up, maintenance and/or repair, and pickup be coordinated with the representative.

GENERAL DESCRIPTION

The contractor and government representative shall coordinate delivery or pick up leased vehicles at the start and completion of the contract.

The leased Walk in Refrigerators Containers (units) shall be used during the for the Guardian Response 18 exercise. The Government assumes responsibility for damages sustained during each exercise.

The contractor shall provide maintenance/repair service, as required, throughout the duration of the exercise.

A. Contractor shall provide a 24/7 phone number for service related issues. Contractor shall respond to service calls within 24 hours and complete the service or replace the vehicles within 12 hrs.

B. Contractor shall provide recovery support for leased Walk in Refrigerators Containers that become inoperable on the installation.

C. Transportation to the work site(s) for all contract employees shall be at contractor's expense.

D. Contractor shall provide all labor, material, equipment, transportation and supervision to provide repair or recovery, as required.

E. The intended use of the Walk in Refrigerators Containers is to store CL I food rations in the garrison and field training environment.

F. Contractor submitted bid costs shall include transportation or delive1y charges, if applicable.

G. Government Maintenance: Government will provide a representative who, on the date of delivery and pick up, will coordinate pick up times/locations and repair work, as required.

H. All Walk in Refrigerators Containers (units) shall undergo a joint inspection at the point of pick up and drop off by both a government and contractor appointed representative. Contractor shall ensure signed copies of delivery and pick up inspection results are provided or contractor shall assumes all liability at the point of contract termination.

I. Government furnished M915 and/or M930s will be used to relocate units as needed throughout the course of the contract.

J. Walk in Refrigerators Containers (units) will be delivered Camp Atterbury, IN for use during

Guardian Response 17, an Army Training exercise. Walk in Refrigerators Containers (units) will be used on multiple training Area located within a 40 mile radius of Camp Atterbury, IN.

K. Contractor shall provide all manpower, transportation, and lift support assets for both delivery and pickup.

L. Contractor will provide maintenance services as required, will respond to maintenance requests within (2) hours of notice, and fix any problems with the unit within (24) hours of notification. If upon assessment the contractor is unable to repair the refrigeration unit(s), he will provide, set up, and power a replacement that day.

M. If generator powered Reefers are provided, contractor shall supply the generator with sufficient wattage to maintain required temperatures for the duration of the exercise. Contractor shall also provide all applicable cords and supplies to operate generator powered Reefer. Generators shall be installed topped off with diesel fuel. Refueling will be accomplished by the Government personnel.

Training Area Conditions Training locations are on both paved and remote sites. Remote sites are considered unimproved land wherein personnel and equipment can be exposed to .extremes in weather conditions (extreme temperatures, snow storms, heavy winds, and rain).

Conduct If repairs/replacement are necessary, contractor shall ensure sufficient management presence at the work site to ensure good order, discipline and safety is maintained. The contractor shall be responsible for the conduct of its employees at all times. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government reservation, shall abide by all regulations of the installation. The contractor shall be responsible for ensuring that any employee providing services under this contract conduct themselves, and perform services, in a professional, safe, and responsible manner and at all times they behave, act, and perform in a manner consistent with the Camp Atterbury, IN philosophy or IAW government directives, etc. Revocation of contractor personnel access to the Installation shall not relieve the contractor from the requirement to meet all the terms and conditions of the contract.

Quality Control Quality control is the responsibility of the Contractor. The Contractor is responsible for the delivery of quality services/supplies to the Government.

Safety Contractor personnel performing maintenance/repair/recovery shall not interfere with or impede Government activities. The contractor shall ensure their employees use the appropriate personal protective equipment required by statutory or regulatory standards. Personal protective equipment shall be maintained in a safe, sanity and operational condition. Employees shall be trained in the use, care and maintenance of personal protective equipment.

The contractor shall perform all operations in accordance with the Occupational Safety and Health Act (OSHA) and all applicable Federal and National standards. The contractor's safety program shall fully comply with the provisions of all Installation, Army and DOD safety regulations, to include, AR 12-9, TB MED 593, TB MED 530, The Army Radiation Safety Program; AR 385-10, Army Safety Program; AR 385-40, Accident Reporting and Records; AR 40-5, Preventive Medicine; Title 29 CFR 1910, 1926, and 1960 Occupational Safety and Health Administration (OSHA) regulation; EM 385-1-1, copies of Engineers Safety Manual; TM 5-682, Facility Engineer/Electrical Facilities Safety and, applicable federal state and local standards shall apply. The above mentioned regulations may be accessed at www.usapa.anny.mil. The contractor shall be responsible for all damages to persons or property that occur as a result of the contractor's fault or negligence, and shall take proper safety and health precautions to protect the work, the workers, the public and the property of others.

Emergency Medical Treatment. Medical services for contractor personnel shall be the responsibility of the contractor. However, the Government may provide, on an emergency basis, medical services for job related injuries, while an employee is performing under this contract if government facilities are available. Such emergency medical care shall be limited to those services necessary to prevent undue suffering or loss of life or limb and shall be provided only during the period of emergency. In such circumstances, action shall be taken to transfer employees of the contractor to a civilian health care provider as soon as the emergency period ends.

The contractor shall reimburse the Government for any emergency medical care and/or MEDEVAC. Following the initial emergency care, the Government is not responsible for any continued or follow up care.

Security Requirements: Some areas are restricted to personnel who have not been properly cleared by federal law enforcement. Consequently, personnel without clearances shall be provided with a U.S. Government escort during times when contractor personnel are performing work under this contract in a restricted area. To avoid unnecessary delays in the performance of service in restricted areas, the contractor shall arrange for a U.S.

Government escort into those areas by coordinating a time/hour with the building/site coordinator. If the coordinator's alternate is not available, the Contractor shall coordinate with the government representative. The Contractor shall notify the government representative when the Contractor's personnel are unable to perform work because escort personnel are not available.

Employee Vehicle Registration Any Privately Owned Vehicle (POV) used by an employee to enter the Installation shall be registered. Proof of employment, insurance and current vehicle registration shall be required. The contractor shall ensure each employee properly registers their vehicle according to the Installation requirement, regulations, and policies.
