155th Security Forces Squadron Visitor Access Request

Nebraska Air National Guard Base Lincoln, Nebraska

DEFENDER of the FORCE

Procedures:

1. Do NOT request access for anyone:

a. Who has a valid DoD Card (e.g. Common Access Card, retired military ID, dependent military ID, etc.).

This card grants them access to the Base.

b. Under the age of 18, as they may enter the Base when escorted by an adult who is granted access. If the child does not have a military ID, or is not escorted by an adult, they will need to be listed on an Access Request.

2. For all others, provide the information required below.

3. I understand that it is my responsibility to:

a. Notify my visitors of any changes to the status of the Base that may affect their ability to gain access.

b. Advise my visitors that all visitors over the age of 18 must have a government issued photo ID.

4. Visitors completing this form must return it to their military or civilian sponsor.

5. Military or DoD civilian sponsor mail this request form to usaf.ne.155-arw.mbx.sfs-bdoc@mail.mil with a “read receipt” when complete. The Access Request Form should be sent to BDOC as early as possible before the appointment. Recommend a follow up phone call to BDOC at 309-1552. (If a read receipt is not returned to you stating that the form has been opened, call BDOC prior to the visitor’s arrival.)

6. Identity vetting will be conducted by the 155 SFS for all visitors to the Nebraska Air National Guard Base.

During high traffic / visitation times, this can cause delays entering the installation.

7. After vetting, SFS will issue a Visitor Pass for all personnel entering the installation. These may be issued for a maximum of 30 days. Passes will be issued in 3 day increments.

8. Unannounced visitors may not be granted access to the Base.
