If unforeseen events cause a variation in the scheduled service, the contractor shall notify the COR in advance of the delay or as soon as possible if unable to notify in advance. Failure to provide the service or variation from the agreed schedule will require written explanation by the contractor. If and when additional service(s) are added, the schedule will be modified.
4.2 Scope of work.
The roof of the Admin Building is constructed of wood trusses and wood deck, and between and below the ceiling joists is hard lid lath and plaster ceiling system throughout the entire interior of the facility. The attic space roof and walls currently have no insulation which impacts the building envelope thermal performance and overall building energy efficiency. There are currently 2 ceiling access panels (approximately 24”x 24”) for personnel access to the attic, located proximate to the breakroom and to the library. There are two non-operable windows located at the attic end walls, caution shall be exercised with these windows due to their historic value.
The Contactor shall provide and install faced fiberglass batt insulation and baffles to the underside of the roof deck and insulation on the end walls in the attic above the ceilings. Blown-in insulation on top of the lath and plaster ceiling or ceiling-mounted water and electrical utilities is not permitted. The insulation must be installed between structural elements throughout the entire underside of the roof deck enclosing the attic space and wall studs. The insulation must be secured to avoid sliding and sagging with 14 ga wire batt supports; the wires support must extend to the width of the batt provided. Baffle installation must go all the way up to the ridge of the underside of the roof deck. The work to be performed must ensure that the insulation and baffles do not block the existing attic vents and wall windows. Care must be taken to verify footing when working in the attic, and some spots could be missing lathe and plaster. The Contractor shall submit the insulation and accessory materials for Government review and approval; the Government estimates that 6,600 SF of R-38 kraft faced fiberglass insulation batts would be adequate for the underside of the deck area and 405 SF of R-13 kraft faced fiberglass insulation batts would be adequate for the wall areas. All applicable manufacturer accessories and recommendations shall be followed and products used shall be free of hazardous materials. The Contractor shall exercise caution to not to damage the integrity of the existing roof, lath and plaster ceiling structure, walls, utilities, and conduits during the installation process and take proactive steps to protect and preserve those features. Any damages incurred during the installation process shall be restored to original condition. Any damages to the historic windows shall be restored to its original condition per historical preservation standards and guidelines. Submit workplan and material data for Government review and approval prior to start of work on site.
Quantity Take Off is estimated as follow (Contractor shall field verify quantities):
Roof Area Approx.6,600 SF
Roof Slope Approx.6”:12”
Wall Area (at ends) Approx. 405 SF
4.3 Contractor Reporting.
4.3.1 Status Updates. The contractor shall submit status report via email to the Government COR at the request of the Government but at no more frequently than a bi-weekly basis. These status updates shall include, at a minimum:
a) A summary of work completed in the field, permit status, and any significant events.
b) Progress photos (in electronic format) depicting major work activities accomplished during the reporting period.
4.3.2 Progress Reports. The Contractor shall submit a Progress Report no later than 1700 hours on Friday afternoon, as warranted but not more frequently than a bi-weekly basis, and according to the distribution and quantities in paragraph 4.5.3 below. These weekly Progress Reports shall address the following:
a) A summary of work completed in the field with respect to all schedule milestones. Annotate and explain all deviations. Denote anticipated or actual delay of a scheduled field activity, to include basis and any effect on subsequent events or scheduled activities.
b) Any outside inspection reports, audits, or other administrative information developed/completed or anticipated.
c) Copies of all Quality Assured Data, sampling and test results and any other laboratory deliverables received, if any.
d) Documentation of material recycling including receipts of recycle centers. Project photos are acceptable provided a sufficient explanation is attached.
4.3.3 Final Report. The Contractor shall provide a final report no later than 10 working days after completion of project. The final report shall include a detailed description of work performed and lessons learned. A summary of the disposition of all project debris materials will include the quantity and type of debris materials recycled, salvaged, reused, and disposed of and shall be presented in chart form showing original material quantity estimated, quantity recycled, percentage recycled, and approximate cost or cost savings versus a commercial landfill alternative.
5.0 QUALITY CONTROL

5.3 Quality Assurance Surveillance Plan. The Government will evaluate the Contractor’s performance under the enclosed Quality Assurance Surveillance Plan (QASP). The QASP provides a systematic method to evaluate performance for this contract. The Contractor is fully expected to comply with all requirements in the PWS. The COR will monitor the Contractors performance to ensure it meets the standards of the contract. When an observation indicates defective performance, the Contracting Officer will require the COR or on-site representative to initial the observation. The initialing of the observation does not necessarily constitute concurrence with the observation, only acknowledgement that he or she has been made aware of the defective performance. Unacceptable performance may result in the Contracting Officer taking any of the following actions: Require the Contractor to take necessary action to ensure that future performance conforms to the contract requirements, reduce the contract price to reflect the reduced value of the services, issues a Contract Discrepancy Report (CDR), or require the Contractor to re-perform the service. The Contracting Officer may require the Project Manager to meet with the Contracting Officer, contractor administrator, COR, and other Government personnel as deemed necessary. The Contractor may request a meeting with KO when he or she believes such a meeting is necessary. Written minutes of any such meetings shall be recorded by the Contractor, approved by the Contracting Officer and signed by the Project Manager and the Contracting Officer or Contract Administrator. Copies of any such meeting minutes shall be maintained by the Contracting Officer. If the Contractor does not concur with any portion of the minutes, such non-concurrence shall be provided in writing to the Contracting Officer within one calendar day following receipt of the approved minutes.
All work performed and equipment used under the contract will be subject to inspection at all times by the Government. The Government’s inspection program is not a substitute for the Quality Control by the Contractor.
