The successful offeror will be required to submit this document upon award. The APP must be accepted by the Government Designated Authority prior to the preconstruction conference. The contractor that receives this award will have 10 calendar days to provide an Accident Prevention Plan (APP). If this deadline is not met, the contractor will be given a cure notice stating that they will have 10 calendar days to submit an APP, otherwise the contract may be terminated for default.

Section C - Descriptions and Specifications

SCOPE OF WORK

RDB Guyandotte Campground Water Tank

GENERAL

Dismantle an Aquastore model 2023 water tank down to the starter ring that’s embedded into the concrete floor. Tank on sight is a 52,000 gallon tank that has a diameter of 19.58’ and height of 23.14’.

Contractor shall then furnish and erect a glass-coated, bolted-steel water storage tank, tank structure and tank appurtenances as and described herein from the first starter ring imbedded in the concrete floor.

Contractor will be responsible for removing all materials from the dismantling of the old tank that is presently on site.

All required labor, materials and equipment shall be included.

Qualifications of Tank Supplier

The Engineer's selection of factory applied glass- fused-to-steel bolt together tank construction for this facility has been predicated upon the design criteria, construction methods specified, and optimum coating for resistance to internal and external tank surface corrosion. Deviations from the specified design, construction or coating details, will not be permitted.

The offeror shall offer a new tank structure as supplied from a U.S.A. manufacturer specializing in the design, fabrication and erection of factory applied glass-fused-to-steel, bolt together tank systems. Steel used for the tanks shall be smelted and produced in the U.S.A.

The specified tank shall be a Model 2023 Aquastore Tank System as manufactured by CST Storage of

DeKalb, Illinois or equal.

Alternate glass-fused-to-steel tank products, as provided by other manufacturers, will be considered for prior approval by the Engineer. Manufacturers lacking the experience requirement will not be considered. The Owner's decision or judgment on these matters will be final, conclusive and binding.

Strict adherence to the standards of design; fabrication; erection; product quality; and long term performance, established in this Specification will be required by the Owner and Engineer.

Tank or Dome substitutions which cause engineering and contract changes - the tank installation as shown on the plans and specified herein, is based on the equipment furnished by one manufacturer. A tank which is offered as a substitute to the specific requirements of these Specifications and which differs in detail and arrangement from that shown may require changes in design and construction. All costs which result from such changes in design and construction are to be borne entirely and unconditionally by the Contractor; said costs to included but not be limited to structural, piping, mechanical and electrical changes and all engineering costs incurred as a result of the substitution, in the revision of Plans and Specifications, review of design changes by others, preparation of change orders, and any other costs directly resulting from said substitution

DESIGN CRITERIA

Tank Size

The factory coated glass-fused-to-steel, bolt together tank shall have a nominal diameter of 19.58 feet, with a nominal sidewall height (to roof eave) of 15 feet.

Tank Capacity

Tank capacity shall be 35,000 gallons (nominal, U.S. gallons).

Tank Design Standards

The materials, design, fabrication and erection of the bolt together tank shall conform to the AWWA

Standard for "Factory Coated Bolted Steel Tanks For Water Storage" - ANSI/AWWA D103, latest revision.

The tank coating system shall conform solely to Section 12.4 of ANSI/AWWA D103. NOTE: Baked-on epoxy painted, galvanized, or stainless steel bolt-together tanks are not considered equal.

The vitreous coating on the tank, bolt head encapsulation material, and joint sealant shall have been approved for listing under ANSI/NSF Standard 61 for Indirect Additives.

The tank manufacturer shall be ISO-9001 certified to assure product quality. The tank manufacturer shall undergo an annual FM (Factory Mutual) inspection of their glass-coated, bolted-steel tank factory & provide written proof thereof to assure quality.

MATERIALS SPECIFICATIONS

Plates and Sheets Note: All steel shall be smelted and produced in the U.S.A.

Plates and sheets used in the construction of the tank shell, tank floor (optional) or tank roof (optional), shall comply with the minimum standards of AWWA D103, Section 4.4.

Design requirements for mild strength steel shall be ASTM A1011 Grade 30 with a maximum allowable tensile stress of 14,566 psi per AWWA D103.

Design requirements for high strength steel shall be ASTM A1011 Grade 50 with a maximum allowable tensile stress of 26,000 psi per AWWA D103.

The annealing effect created from the glass coated firing process shall be considered in determining ultimate steel strength. In no event shall a yield strength greater than 50,000 psi be utilized for calculations detailed in AWWA D103, Sections 5.4 and 5.5.

Multiple vertical bolt line sheets and plates of ASTM A1011 Grade 50 only shall be manufactured such that holes are staggered in the vertical bolt lines and that no two adjoining holes are in-line horizontally, except at the center of the sheet or plate.

1. Bolt seam design shall generally be in accordance with the requirements of AWWA D103 section

5.5.2; bolt spacing may be adjusted in the vertical bolt lines to increase the net section and improve joint efficiency to a maximum of 85%.

2. Double sheeting of tank panels shall not be permitted to achieve structural sidewall thickness requirements.

Rolled Structural Shapes

Material shall conform to minimum standards of ASTM A36 or ASTM A992 Horizontal Wind Stiffeners

Design requirements for intermediate horizontal wind stiffeners shall be of the "web truss" design with extended tail to create multiple layers of stiffener, permitting wind load to transfer around tank.

Web truss stiffeners shall be of steel with hot dipped galvanized coating.

Rolled steel angle stiffeners are not permitted for intermediate stiffeners.

Bolt Fasteners

Bolts used in tank lap joints shall be 1/2" - 13 UNC- 2A rolled thread, and shall meet the minimum requirements of AWWA D103, Section 4.2.

Bolt Material

SAE J429 (1” and 1-1/4” bolt length) heat treated to:

Tensile Strength - 120,000 psi Min.

Proof Load - 85,000 psi Min.

Allowable shear stress – 29,454 psi.

SAE J429 (>1-1/4” bolt length) heat treated to:

Tensile Strength - 150,000 psi Min.

Proof Load - 120,000 psi Min.

Allowable shear stress - 36,818 psi.

Bolt Finish - Zinc, mechanically deposited.

2.0 mils minimum - under bolt head, on shank and threads

Bolt Head Encapsulation

High impact polypropylene co-polymer encapsulation of entire bolt head up to the splines on the shank.

Natural resin with UV (ultraviolet) light inhibitor. Color to be black.

All tank shell bolts shall be installed such that the head portion is located inside the tank, and the washer and nut are on the exterior.

All lap joint bolts shall be properly selected such that threaded portions will not be exposed in the "shear plane" between tank sheets. Also, bolt lengths shall be sized as to achieve a neat and uniform appearance. Excessive threads extending beyond the nut after torqueing will not be permitted.

All lap joint bolts shall include a minimum of four (4) splines on the underside of the bolt head at the shank in order to resist rotation during torqueing.

All exterior nuts, washers, and bolt threads will be covered with a sealer-filled protective plastic cover.

Color to match tank shell.

Sealants

The lap joint sealant shall be a one component, moisture cured, polyurethane compound. The sealant shall be suitable for contact with potable water and shall be certified to meet ANSI/NSF Additives Standard 61 for indirect additives.

The sealant shall be used to seal lap joints, bolt connections and edge fillets for sheet notches and starter sheets. The sealant shall cure to a rubber like consistency, have excellent adhesion to the glass coating, low shrinkage, and be suitable for interior and exterior use.

Sealant curing rate at 73 F and 50% RH

Tack-free time: 6 to 8 hours.

Final cure time: 5 to 12 days.

Neoprene gaskets and tape type sealer shall not be used.

GLASS COATING SPECIFICATION

Surface Preparation

Following the de-coiling and shearing process, sheets shall be steel grit-blasted on both sides to the equivalent of SSPC-10. Sand blasting and chemical pickling of steel sheets is not acceptable.

The surface anchor pattern shall be not less than 1.0 mil.

These sheets shall be evenly oiled on both sides to protect them from corrosion during fabrication.

Cleaning

Sheet edges of sidewall and floor plates shall be mechanically rounded prior to glass coating. Glass coating of the sheet edges shall be similar to the flat panel surfaces. The process shall be applied to all four sheet edges, and shall be equal to EDGECOAT II by CST Storage.

After edge coating and prior to application of the coating system, all sheets shall be thoroughly cleaned by a caustic wash and hot rinse process followed immediately by hot air drying.

Inspection of the sheets shall be made for traces of foreign matter or rust. Any such sheets shall be re-cleaned or grit-blasted to an acceptable level of quality.

Coating

All sheets shall receive one coat of a catalytic nickel-oxide glass pre-coat to both sides and then air dried.

Another coat of milled cobalt blue glass shall be applied to both sides of the sheets and then dried.

A third coat of milled titanium dioxide white glass shall be applied to all wetted surfaces which must be an 18 to 22 percent titanium dioxide reinforced mixture. The specified coating shall be Aquastore Vitrium or equal. An acceptable alternate three coat two fire system must be submitted for approval prior to the offer.

The sheets shall then be fired at a minimum temperature of 1500˚ F in strict accordance with the manufacturer’s ISO 9001 quality process control procedures, including firing time, furnace humidity, temperature control, etc.

The dry film interior coating thickness shall be 10.0 to 18.0 mils minimum. The finished inside color shall be white.

The dry film exterior coating thickness shall be 7.0 to 15.0 mils minimum. The finished exterior color shall be cobalt blue.

The same glass coating as applied to the sheet surfaces shall be applied to the exposed edges.

Factory Inspection

The manufacturer’s quality system shall be ISO 9001 certified.

Chemical Resistance of Glass Coating

Every batch of component frits shall be individually tested in accordance with PEI Test T- 21 (Citric Acid at Room Temperature).

Factory Holiday Test

A dry volt test using a minimum of 1100 volts is required.

Frequency of the test shall be every sheet. Any sheet registering a discontinuity shall be rejected

All inside sheet surfaces shall be holiday free.

Measurement of Glass Thickness

Glass thickness shall be measured using an electronic dry film thickness gage (magnetic induction type) approved by CST Storage. The thickness gage shall have a valid calibration record.

Frequency of the test shall be every tenth sheet. The thickness of the glass shall be between 10.0 and 18.0 mils.

Measurement of Color

The exterior color of the sheets shall be measured using a colorimeter approved by CST Storage.

The colorimeter shall have a valid calibration record.

Frequency of the test shall be every tenth sheet. The color must fall within the tolerance specified by CST Storage, else the panel shall be rejected.

Impact Adherence Test

The adherence of the glass coating to the steel shall be tested in accordance with ASTM B916-01.

Any sheet that has poor adherence shall be rejected.

Frequency of this test shall be one sheet per gage lot run minimum.

Fishscale Test

The glass coating shall be tested for fishscale by placing the full size production sheets in an oven at 400º F for one hour. The sheets will then be examined for signs of fishscale. Any sheet exhibiting fishscale shall be rejected and all sheets from that gage lot will be similarly tested.

Frequency of this test shall be one sheet per gage lot run minimum.

Packaging All approved sheets shall be protected from damage prior to packing for shipment.

Heavy paper or plastic foam sheets shall be placed between each panel to eliminate sheet-to-sheet abrasion during shipment.

Individual stacks of panels will be wrapped in heavy mil black plastic and steel banded to special wood pallets built to the roll-radius of the tank panels. This procedure eliminates contact or movement of finished panels during shipment.

Shipment from the factory to the job site will be by truck, hauling the tank components exclusively. No common carrier, drop, or transfer shipments.

Foundation The tank foundation shall be cleaned and inspected for any damage or cracking. If any damage is noted it shall be reported to the Designated Government Authority immediately.

Sidewall Structure Field erection of the glass-coated, bolted-steel tank shall be in strict accordance with the procedures outlined in the manufacturer's erection manual, and performed by an authorized dealer of the tank manufacturer, regularly engaged in erection of these tanks.

Specialized erection jacks and building equipment developed and manufactured by the tank manufacturer shall be used to erect the tanks.

Particular care shall be taken in handling and bolting of the tank panels and members to avoid abrasion of the coating system. Prior to liquid test, all surface areas shall be visually inspected by the Engineer.

An electrical holiday test shall be performed during erection using a nine (9) volt leak detection device.

All electrical leak points found on the inside surface shall be repaired in accordance with manufacturer's published touch up procedure using urethane sealer.

The placement of sealant on each panel may be inspected prior to placement of adjacent panels.

However, the Engineer's inspection shall not relieve the bidder from his responsibility for liquid tightness.

No backfill shall be placed against the tank sidewall without prior written approval and design review of the tank manufacturer. Any backfill shall be placed according to the strict instructions of the tank manufacturer.

Roof Construction

Tanks with diameters of 14 to 31 ft. shall include a radially sectioned roof fabricated from glass-coated, bolted steel panels, as produced by the tank manufacturer, and shall be assembled in a similar manner as the sidewall panels utilizing the same sealant and bolting techniques, so as to assure a water/air tight assembly. The roof shall be clear span and self-supporting. Both live and dead loads shall be carried by the tank walls. The exterior coating finish shall be cobalt blue glass. The manufacturer shall furnish a roof opening which shall be placed near the outside tank ladder and which shall be provided with a hinged cover and a hasp for locking. The opening shall have a clear dimension of at least twenty-four (24") inches in one direction and eighteen (18") inches in the other direction. The opening shall have a gasketed weather-tight cover.

Primary horizontal forces into the tank shell shall be contained by an integral aluminum tension ring (unless otherwise specified). The frame shall consist of aluminum structural members with the joints arrayed on the surface of a sphere. The arrangement of members shall result in a pattern of triangular spaces. These spaces shall be closed with light gauge aluminum panels. The members shall be joined by means of bolting there flanges to aluminum gusset plates.

All metal components of the aluminum dome structure shall be aluminum or 300 series stainless steel.

No galvanized, aluminized, painted, or plated steel shall be used anywhere in the dome above the mounting bracket base plates. Dissimilar materials in the supporting structure shall be isolated from the aluminum dome by means of a compatible elastomeric gasket.

The entire structure shall be designed as a watertight system under all design load and temperature conditions. The design shall include sealant to be completely encapsulated by applying it to the gusset covers’ inner circumferences, beneath the gusset covers’ top closure plates.

The aluminum closure panels shall be attached continuously along their edges to the structural members by means of batten bars, which engage the panels in an interlocking joint. Designs that incorporate raised battens, overlapping panels and/or designs that incorporate fasteners which penetrate panels and attach to structural members are expressly prohibited. The roof panels shall be fabricated from continuous 3003-H16 aluminum sheeting.

Connection forces shall be transferred through gusset plates connected to the top and bottom flanges of the beam struts. The connections shall be designed as moment connections; a minimum of four bolts shall be used to connect the gusset plate to each strut flange. The structural analysis shall be performed using non-linear, second order, stiffness analysis models in accordance with ADM 2010 Chapter C.

Stability shall be provided for the structure as a whole and for each of its components. The available strengths of members and connections determined in accordance with Section C.3 shall equal or exceed the required strengths determined in accordance with Section C.2

Fasteners shall be designed with a factor of safety of 2.34 on ultimate strength and 1.65 on yield strength.

EXPERIENCE/QUALIFICATIONS:

No equipment shall be supplied by any manufacturer not regularly engaged in the manufacturing and production of domes in the size and character herein specified. The manufacturer must have designed, manufactured and installed at least one (1) dome of the same size as the unit(s) specified herein. This dome must be in satisfactory use for a period not less than ten (10) years.

The cover manufacturer must own and operate its own US-based manufacturing facility, and the use of a fabrication facility that is not US-based and/or owned and operated by the cover manufacturer is expressly prohibited. Manufactures that do not meet these qualifications will not be considered.

The cover manufacturer must be ISO 9001 certified.

Materials

Bolts and Fasteners – Threaded fasteners shall be 300 series stainless steel per ASTM F593, Alloy

Group 1. Lockbolts shall be 7075-T73 aluminum, 304 or 305 stainless steel. Screws shall be aluminum or 300 series stainless steel. Triangulated space truss: 6061-T6 aluminum struts and gussets.

Plates and Sheets - Plate and sheet material shall be aluminum alloy 3003-H16, 3105- H154, 6061-T6, 5052-H32 or 5052-H36; mill finish AA - M10 as fabricated. Minimum thickness for gussets shall be 5/16”. Sheet materials shall be 0.05” minimum thickness. Triangular closure panels: .050"t 3003-H16 aluminum sheet.

Structural Shapes - Aluminum structural shapes shall be alloy 6061-T6. The aluminum structural members shall be a minimum of 6 inches deep. To improve torsional stability, the dome’s structural members must incorporate a double web. The use of I-beams with only a single web is expressly prohibited.

Tension Ring - Tension ring structural shapes shall be 6061-T6 aluminum. Design of the tension ring shall be based on the net cross section of the members and shall not include top flange protrusions used for panel attachment, bolt holes, or outstanding legs that are not connected through the joints.

Miscellaneous Shapes - Miscellaneous aluminum shapes shall be alloy 6061-T6 or 6063-T5.

Gaskets - All gaskets shall be ozone resistant Silicone only. The gaskets must have a 1/8” minimum thickness.

Sealant - All sealants shall be silicone and resistant to ozone and ultraviolet light and conform to Federal Specification TT-S-00230C.

Miscellaneous Penetration Seals- All other penetration seals shall be weatherproof rubber seals.

Support Bearings - Acceptable bearing surfaces for sliding bearing are Teflon to stainless steel only. In order to avoid damage to the Teflon and to reduce the coefficient of bearing friction, Teflon shall not bear on aluminum surfaces. Dome supports shall utilize only bolted connections. The use of aluminum structural welding at the dome supports is expressly prohibited.

Dormers, doors, vents and hatches: 6061-T6, 5086-H34 or 3003-H16 aluminum.

Roof Vent

A properly sized vent assembly in accordance with AWWA D103 shall be furnished and installed above the maximum water level of sufficient capacity so that at maximum possible rate of water fill or withdrawal, the resulting interior pressure or vacuum will not exceed 0.5" water column.

The overflow pipe shall not be considered to be a tank vent.

The vent shall be constructed of aluminum.

The vent shall be so designed in construction as to prevent the entrance of birds and/or animals by including an expanded aluminum screen (1/2 inch) opening. An insect screen of 23 to 25 mesh polyester monofilament shall be provided and designed to open should the screen become plugged by ice formation.

Appurtenances (per AWWA D103, Section 7)

Pipe Connections

Where pipe connections are shown to pass through tank panels, they shall be field located, saw cut, (acetylene torch cutting or welding is not permitted), and utilize an interior and exterior flange assembly.

ESPC Sealer No. 98 shall be applied on any cut panel edges or bolt connections.

Overflow piping shall be 6 inch diameter schedule 80 PVC and shall have a stainless steel mesh screened end.

Outside Tank Ladder

An outside tank ladder shall be furnished and installed to meet EM385-1-1 (SECTION 21.J) and all

OSHA requirements governing fixed ladders.

Ladders shall be fabricated of aluminum and utilize grooved, skid-resistant rungs, with spacing not to exceed rung to rung of 12”, all rungs must be a diameter of ¾”

The exterior ladder will be equipped w/ an OSHA approved safety rail/cable, and ladder climbing device (LCD) that is equipped to attach a fall protection harness.

Anchorage strength for LCD shall be a minimum of 3,000 lbs.

A work platform shall be installed at the top of the fixed ladder with a dimension of 30”X32” and a working weight of 1000 pounds

The platform must be equipment with a guardrail with a maximum height of 38” and a minimum height of 34” and a middle rail height of 24”.

The platform must be equipped with a latching entry hinged guardrail gate to allow closing and avoiding individual from falling backward while working off of platform Bottom of fixed ladder shall be equipped with a locking device to avoid unauthorized access to fixed ladder.

One sidewall access manway shall be provided as shown on the contract drawings in accordance with

AWWA D-103.

Such manway shall be a minimum of 24 inches in diameter and shall include a properly designed reinforcing frame and cover plate. A davit to hold the cover plate, when opened, is required.

Identification Plate A manufacturer's nameplate shall list the tank serial number, tank diameter and height, and maximum design capacity. The nameplate shall be affixed to the tank exterior sidewall at a location approximately five (5') feet from grade elevation in a position of unobstructed view.

Cathodic Protection

The Manufacturer will provide a cathodic protection system consisting of sacrificial magnesium anodes which provide corrosion protection for the portions of the structure immersed in liquid. The anodes are equally spaced (to the nearest vertical bolt line) around the structure, attached to the floor, and bolted through existing shell sheet bolt holes. In special cases where anodes may be spaced differently, a layout plan will be provided as part of the submittal package. Lead wires and buss bars are used to ensure continuity between anodes and structure shell sheets.

Electrical continuity between all tank sidewall panels shall be the responsibility of the tank manufacturer.

The design life shall be calculated at 10 years. The cathodic protection system shall be designed for protection of uncoated steel surfaces in the product zone, including rebar within an uncoated concrete tank floor.

FIELD TESTING

Hydrostatic

Following completion of erection and cleaning of the tank, the structure shall be tested for liquid tightness by filling tank to its overflow elevation.

Any leaks disclosed by this test shall be corrected by the erector in accordance with the manufacturer's recommendations.

Water required for testing shall be furnished by the owner at the time of tank erection completion, and at no charge to the tank erector. Disposal of test water shall be the responsibility of the owner.

Labor and equipment necessary for tank testing is to be included in the price of the tank.

DISINFECTION

Standards

The tank structure shall be disinfected at the time of testing by chlorination in accordance with AWWA

Specification C652 "Disinfection of Water Storage Facilities" as modified by the tank manufacturer.

Disinfection shall not take place until tank sealant is fully cured (10 to 12 days at 73 F/50% relative humidity).

Acceptable forms of chlorine for disinfection shall be:

Liquid chlorine as specified in AWWA C652.

Sodium hypochlorite as specified in AWWA C652.

Calcium hypochlorite (HTH) is not acceptable.

Acceptable methods of chlorination per AWWA C652:

Section 4.1.1.

Section 4.1.2 - chemical feed pump only (4.l.2.l).

Section 4.3.

Section 4.2 is not acceptable.

TANK GAUGE

Tank shall be equipped with a tank gauge which shall display the accurate water level inside of tank and be visible from the from looking at the front of the tank. Gauge shall consist of a pulley system with stainless steel cable that is attached to a float inside of the tank and be calibrated correctly to read the water level from a staff gauge that is attached to the front of the tank.

SECURITY REQUIREMENTS

All Contractors and subcontractor employee(s) working on this contract who require access in and/or around any Huntington District facilities shall receive authorization to be on site at these facilities based upon a favorable background investigation in accordance with “Homeland Security Presidential Directive 12: Policy for a Common Identification Standard for Federal Employees and Contractors” (HSPD 12). The Huntington District Security Management Office shall submit contractor(s) personal information to complete the basic background investigation in order to make a recommendation to the Contracting Office of an employee’s suitability for work on Government property. The Procuring Contracting Officer (PCO) has the right to deny any contractor or subcontractor employee from working on the contract based upon the results of the background investigation provided by the Huntington District Security Management Office. The Government, Contractor, and subcontractors shall not permit any employee(s) access to drawings, blueprints, records, photos, sites, or projects until the Huntington District Security Management Office has completed an Operations Security (OPSEC) review of such items and determined they can be released.

FOR ALL CONTRACTS 1-YEAR AND SUBSEQUENT OPTION YEARS:

Contractors shall submit a complete investigation packet for each contractor and subcontractor employee to the Huntington District Security Management office within 10 working days after award of any contract, or prior to the individual being permitted unescorted access in or on District facilities.

The Contractor must also provide the following: (1) Name of the Contract Specialist with whom he/she is working,

(2) Project Name, (3) Contract Number (4) Task Order Number when applicable, and (5) Security Information sheet on each employee working on site must be submitted directly to the Huntington District Security Office.

The Contractor must provide a full legal name, complete SSN#, Date of Birth, Place of Birth (City and State) and Driver’s license number if available. It is the Contractor’s responsibility to ensure the above information is accurate and legible, otherwise delays may occur.

Contractors shall have background checks completed for their employees annually in the case of Contracts with option periods. For instance, if the employee was cleared in May of 2018, they shall be cleared again in May of 2019. It is the Contractor’s responsibility to ensure that each employee’s security is up to date. Failure to comply may lead to possible removal from the project/jobsite.

The Contractor must ensure if new employees are brought in to work on an existing Contract with the U.S. Army Corps of Engineers (USACE), they must process the employee through the Huntington District USACE Security Management Office for background purposes.

FOR CONTRACTORS REQUIRING DOD COMMON ACCESS CARDS (CAC)

The documents listed below are required:

SF 85, Questionnaire for Non-Sensitive Positions (see http://www.opm.gov/forms/ to print form). FD 258, Contractor Fingerprint Card (one card for digital prints, two for ink prints) (NOTE: a trained/qualified individual must take prints, ORI block on the FD 258 must be USOPM000Z) (Contractor Fingerprint Cards (FD-258) may be obtained by Contracting Division by contacting the Huntington District Security Management Office).

Contractor shall report any changes in personnel, or at any time conditions change at least 5 working days prior to individual(s) need to have access to the site. The Contractor shall provide the same information as listed above for all new employees.

Following submission of paperwork, the Huntington District Security Management Office will conduct an initial criminal check based upon the information provided. If there are no issues, the Huntington District Security Management Office will make recommendations to the PCO or their Designated Representative. The Huntington District Security Management Office cannot give approval due to unfavorable report(s). The Huntington District Security Management Office shall coordinate with Office of Counsel and will jointly make recommendations to the PCO and/or the Designated Representative should a background investigation come back with derogatory information. The PCO will make the determination whether to allow the Contractor personnel to work on the site based on Security and Office of Counsel recommendations, and the PCO will advise the Contractor of the decision.

FOREIGN NATIONALS

The Contractor shall provide the package listed above, as well as additional materials noted below for all Foreign Nationals. The Contractor is to expect additional time in the approval process for all Foreign Nationals due to the level of review and elevation of the review to Headquarters staff to complete. All Foreign Nationals must comply with the following paragraph:

In accordance with Engineering Regulation 380-1-18, “Technology Transfer, Disclosure of Information and Contacts with Foreign Representatives” dated 1 August 1996, Section 4, all foreign nationals who work on Corps of Engineers’ contracts shall be approved by the Headquarters US Army Corps of Engineers (HQ USACE) Foreign Disclosure Officer before beginning work on this contract; this regulation includes subcontractor employees. The Contractor shall submit to the Security Management Office, the names of all foreign nationals proposed for performance under this contract, along with documentation to verify legal admittance into the United States. Such documentation shall include at least one of the following: a U.S. passport, Certificate of U.S. Citizenship (INS Form N-560 or N-561), Certificate of Naturalization (INS Form N-550 or N-570), Alien Registration Card with photograph (INS Form I-151 or I-551), Employment Authorization Card (INS Form I-688A), etc.

In order to assure forms are properly completed, and to minimize time delays, the Contractor is encouraged to work closely with the Huntington District Security Management Office. The Government will return for correction incomplete forms, illegible forms, etc. which will delay the contractor / subcontractor from beginning work.

Security requirements are subject to change in accordance with HSPD-12, Office of Personnel Management, regulation or policy changes, etc. The Government shall add any changes to security requirements by modification to the contract.

AT/OPSEC Requirements. The following shall be accomplished by the contractor before any work will be ordered:

Access and General Protection/Security Policy and Procedures. All Contractor and all associated sub‐ contractors employees shall comply with applicable installation, facility, and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks or background investigation and to meet installation/facility access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services, or Security Office. Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

Suspicious Activity Reporting Training (e.g. iWATCH, CorpsWatch, or See Something, Say Something). The contractor and all associated sub-contractors shall receive a brief/training (provided by the RA) on the local suspicious activity reporting program. This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the project manager, security representative or law enforcement entity. This training shall be completed within 30 calendar days of contract award and within 30 calendar days of new employees commencing performance with the results reported to the COR NLT 5 calendar days after the completion of the training.

Pre-screen candidates using E-Verify Program. The Contractor must pre-screen Candidates using the E-verify

Program (http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements.

The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the COR no later than 3 business days after the initial contract award." *When contracts are with individuals, the individuals will be required to complete a Form I-9, Employment Eligibility Verification, with the designated Government representative. This Form will be provided to the Contracting Officer and shall become part of the official contract file.

Contractor Safety Requirements

General

If the Contractor fails or refuses to promptly comply with safety and security requirements as specified herein, any project employee may stop all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to any such work stoppage shall be made subject to any claims for extension of time or for excess costs or damages sustained by the Contractor. Also, the Contractor will not be paid for work not performed as a result of the stop order.

Contractor Accident Prevention Plan (APP).

A. General. Contractor shall comply with all Federal, State and Local safety requirements including those specified by the Occupational Safety and Health Act (OSHA) in 29 CRF 1910 General Industry Standard, 1926 Construction Standard and the U.S. Army Corps of Engineers Safety & Health Requirements Manual EM 385-1-1 as currently revised. Where a conflict exists between safety standards/requirements the contractor shall comply with the more stringent requirement. If the Contractor fails or refuses to promptly comply with safety and security requirements as specified herein, any project employee may stop all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to any such work stoppage shall be made subject to any claims for extension of time or for excess costs or damages sustained by the Contractor. Also, the Contractor will not be paid for work not performed as a result of the stop order.

B. Contractor Accident Prevention Plan (APP). The Contractor shall prepare a site-specific Accident Prevention Plan following the outline provided in EM 385-1-1, Appendix A as a minimum. The contractor shall address each element and sub-element in the outline in the order they appear in Appendix A. Any item from the outline that does not apply must be listed, in its assigned place and a brief explanation of the reason it is not required/provided. The APP shall address any unusual or unique aspects of the project or activity. This plan must be accepted by the Contracting Officer prior to commencement of any work. Contractors are encouraged to contact the USACE Safety & Occupational Health Office for questions or information regarding the development of contract Accident Prevention Plans. (1) The contractor shall have on site an employee designated as a Site Safety and Health Officer (SSHO) who has completed the OSHA 30-hour Construction Safety training or its equivalent as specified in EM 385-1-1, 01.A.17 and has at least five years’ experience relative to the work to be performed. No work will be permitted unless the SSHO is present at the work site.

(2) For task orders limited to routine mowing, cleaning and janitorial work SSHO responsibilities may be a collateral duty. Contractor may also elect to use an SSHO with either OSHA 30-hour Construction or General Industry qualifications.

(3) Work crews of two or more must have two people holding current certification in First Aid and CPR from the American Red Cross, American Heart Association or a licensed physician. Training must include a hands-on component that cannot be taken online and the contractor must maintain appropriate First Aid kit on site. Single individuals working alone must have current certification in First Aid as a minimum.

(4) All work that potentially exposes the worker to falls of six feet or more will require the contractor to submit a Site Specific Fall Protection & Prevention Plan in accordance with EM 385-1-1, Section 21.D.

(5) Cranes and hoisting equipment shall be operated only by qualified personnel (operators licensing, physical and drug testing in accordance with EM 385-1-1 Section 16.B). Note: CDL Medical Clearance is not acceptable. Please see attached medical clearance requirements.

(6) Effective November 8, 2010 all “Signalpersons” and “Riggers” shall be qualified. This is a mandatory requirement in accordance with 29 CFR 1926.1400 Regulations. Riggers must be a qualified person for the performance of specified hoisting activities such as during assembly/disassembly, in accordance with OSHA 1926.1404(r)(1) and 1926.1425(c)(3).

(7) Work orders involving potential exposure to Hazardous, Toxic or Radiological Waste shall require development of Site Specific Safety and Health Plan (SSHP) by qualified personnel as specified in EM 385-1-1 Sections 6 and 33.

C. Risk Management Process. The Contractor shall utilize risk management tools such as Activity Hazard Analysis (AHA), Job Hazard Analyses (JHAs), Job Safety Analyses (JSAs) or similar tools to identify, assess, prioritize risks and develop controls to minimize, monitor and control the probability and/or impact of unfortunate events to an acceptable level. Within each major phase, all activities, tasks or Definable Features of Work (DFOWs) shall be identified that will require an AHA/JSA/JHA. For each work activity, task or DFOW contractor shall prepare the initial AHA/JSA/JHA. A Risk Assessment Code (RAC) is assigned to each step, to the risk that remains after controls have been applied (residual risk) a final RAC (cannot be lower than the highest step RAC) shall be assigned. See EM 385-1-1 Section 01.A.14.b for further guidance or contact the SOHO for assistance. The AHA/JSA/JHA shall be reviewed and accepted by the proper authority in order to proceed with the activity. All on-site employees shall review the accepted AHA/JSA/JHA prior to performing work and during weekly and “Tool Box” safety meetings as appropriate.

1. AHA/JSA/JHA shall define the activities being performed and identify the work sequences, the specific anticipated hazards, site conditions, equipment, materials, and the control measures to be implemented to eliminate or reduce each hazard to an acceptable level of risk. AHA/JSA/JHA should indicate compliance with EM 385-1-1 personal protective equipment requirements (safety-toed work boots, safety glasses, gloves, hard hats, etc.), work crews of two or more must have two people currently qualified in first aid and CPR and the appropriate first aid kit on site. Single individuals working alone must have current certification in First Aid only.

2. The names of the Competent Person(s) and Qualified Person(s) required for a particular activity (e.g., excavation, scaffolding, fall protection, or other activities as specified by OSHA and this manual) shall be identified and included in the AHA/JSA/JHA, as well as proof of their competency/qualification.

D. Accident/Mishap Reporting: The Contractor shall report all recordable mishaps to the GDA within 24-hours in accordance with EM 385-1-1 Section 01.D. Accidents shall be investigated to determine causal factors and corrective actions shall be implemented. Accidents resulting in: days away injuries, days away illnesses, restricted/transfer injuries, property damage of $5,000.00 or more shall be investigated and reported to the GDA using the USACE Accident Investigation Report Form ENG 3394 within 24 hours of the occurrence.

(1) Immediate notification to the GDA is required for all serious accidents:

a. Fatal Injury/illness;

b. Permanent totally disabling injury/illness;

c. Permanent partial disabling injury/illness;

d. One (1) or more persons hospitalized as inpatients as a result of a single occurrence;

e. $500,000 or greater accidental property damage;

f. Three (3) or more individuals become ill or have a medical condition suspected to be related to a site condition, or a hazardous or toxic agent on the site;

g. USACE aircraft destroyed or missing;

The Contractor shall maintain an OSHA Log of Work Related Injuries and Illnesses, OSHA Form 300 and comply with all accident reporting and recordkeeping requirements specified in 29 CFR 1904.39.

(2) In addition to the above, any mishap occurring in any of the following high hazard areas shall be reported immediately to the GDA, investigated and corrective actions implemented.

a. Electrical (includes Arc Flash, electrical shock, etc.);

b. Uncontrolled Release of Hazardous Energy (includes electrical and non-electrical);

c. Load Handling Equipment (LHE) or Rigging;

d. Fall-from-Height (any level or…
