Equipment for Dump Station Pretreatment Facility at Battle Run Day Use Area

Summersville Lake, Summersville, WV

1.0 General

The contractor shall provide all necessary equipment and spares for a complete Wastewater Pretreatment system for flow received from the Campground Dump Station at Battle Run Campground and Day Use Area at Summersville Lake, Summersville, WV and an additional pump package compatible with the septic tank at the new Battle Run Treatment Plant. This requirement is for supplies and materials only. This factory built advanced wastewater treatment system shall consist of fiberglass tanks, aeration system, pump systems, and the controls necessary, and as described herein, to make a fully operational system. Available power at the site is 120/240 VAC, 3-phase, 4-wire, 60 Hertz, open delta. Control Panel must be able to accept emergency power from a generator with safety interlock.

2.0 Manufacturer Experience

The advanced wastewater treatment system equipment furnished shall be manufactured and supplied by a company experienced in the design and manufacture of advanced wastewater treatment systems. The manufacturer shall have a minimum of ten years’ experience in the design and manufacturing of advanced wastewater treatment systems of similar size and equipment specified and shall have a minimum of twenty-five successful installations of advanced treatment systems. The manufacturer shall provide evidence of successfully obtaining approval for a system with similar permit requirements with the governing regulating authority. The Manufacturer shall supply detailed installation, operations and maintenance instructions, and evidence of an adequate service provider network shall be submitted to the government Contracting Officer. The Manufacturer shall also submit evidence that the supplier has spare parts, equipment repair ability, and experienced service personnel.

3.0 Pretreatment Facility Equipment

The advanced wastewater treatment system shall consist of two fiberglass tanks (Primary and Secondary) suitable for direct bury that contain treatment equipment as described herein. The Primary Tank shall have two chambers created by an internal partition wall. The inlet of the first chamber shall be fitted with a sanitary tee and down-tube such that flow enters the tank below the water surface elevation. The first chamber will be utilized for settling solids. Flow will transfer to the secondary chamber using two sanitary tees and submerged down-tubes. The secondary chamber will be utilized for aeration. Aeration equipment shall be provided as specified herein.

The Primary tank effluent will leave the secondary chamber through a sanitary tee and flow by gravity to the Secondary Tank. The Secondary Tank will be utilized for final clarification and flow equalization. The Secondary Tank will be fitted with a Biotube pump vault (or equal) that will pump the pretreated effluent to the new wastewater treatment plant. An additional Biotube pump vault (or equal) shall be provided for the final septic tank at the new treatment plant.

3.1 Fiberglass Tanks

A. References

1. American Concrete Institute (ACI) standard ACI 318, Building Code Requirements for Structural Concrete.

2. ANSI/AWWA D120 - Thermosetting Fiberglass-Reinforced Plastic Tanks.

3. IAPMO/ANSI Z1000 - Prefabricated Septic Tanks.

4. Tank manufacturer shall be recognized by Underwriters Laboratories as a manufacturer of tanks listed to the UL-1316 standard.

B. Tank Manufacturer

1. Acceptable Manufacturer: Xerxes Corporation, which is located at: 7901 Xerxes Ave.

S.; Minneapolis, MN 55431 OR APPROVED EQUAL.

C. Tank Construction

1. The tank size, fittings and accessories shall be as described herein.

2. Tank shall be manufactured with structural ribs which are fabricated as an integral part of the tank wall.

3. Tank shall be manufactured with a laminate consisting of resin and glass fiber reinforcement only. No sand/silica fillers or resin extenders shall be used.

4. Tank shall be vented to atmospheric pressure.

5. Tank shall be capable of handling liquids with specific gravity up to 1.1.

6. Tank shall be compatible with liquids found in domestic wastewater.

D. Design Loading Conditions

1. Internal Load - Tank shall be designed to withstand a 5-psig (35 kPa) air-pressure test with a 5:1 safety factor.

2. Surface Loads - Tank shall be designed to withstand surface H-20 and HS-20 axle loads when properly installed according to manufacturer's current Installation Manual and Operating Guidelines.

3. External Hydrostatic Pressure - Tank shall be designed for 7 feet (2.1 m) of overburden over the top of the tank, the hole fully flooded, and a safety factor of 5:1 against general buckling.

E. Onsite Wastewater Treatment Applications

1. Governing Standards, as applicable:

a. ANSI/AWWA D120 - Thermosetting Fiberglass-Reinforced Plastic Tanks.

b. American Concrete Institute (ACI) standard ACI 318, Building Code

Requirements for Structural Concrete.

c. IAPMO/ANSI Z1000 - Prefabricated Septic Tanks. Tank shall be IAPMO/ANSI

Z1000 listed and labeled.

d. Tank manufacturer shall be recognized by Underwriters Laboratories (UL) as a manufacturer of tanks listed to the UL-1316 standard.

2. Tank Accessories - Onsite Wastewater Storage Applications:

a. Tank Anchoring:

1) Anchor straps shall be as supplied by tank manufacturer and designed for a maximum load of 25,000 lbs (11340 kg).

2) Galvanized turnbuckles shall be supplied by the tank manufacturer.

3) Prefabricated concrete anchors shall be supplied by the tank manufacturer, designed to the ACI 318 standard, manufactured with

4,000 psi concrete and shall have adjustable anchor points.

b. Access Openings:

1) All access openings shall have a diameter of 30 inches complete with riser, lid and necessary hardware.

c. Access Risers:

1) Access risers shall be 30” diameter PVC or FRP as supplied by Orenco Systems, Inc. or equal.

2) Access risers shall be attached to access openings during installation utilizing adhesive or FRP bonding kits as supplied by Orenco Systems, Inc. or equal.

d. Piping and Fittings:

1) Tank shall have factory-installed threaded fittings or pipe stubs.

2) PVC piping shall at least meet the requirements of ANSI Sch-40.

3) All flanged nozzles shall be flanged and flat-faced, and conform to Class

150 bolting patterns as specified in ANSI/ASME/ B16.5.

4) Carbon steel and stainless steel NPT fittings shall withstand a minimum of 150 foot-pounds (203 NM) of torque and 1,000 foot-pounds (1356 NM) of bending, both with a 2:1 safety factor.

e. Manway Openings:

1) The standard manway shall be flanged, 22 inches (559 mm) I.D. and complete with gaskets, bolts and cover.

2) Manway openings shall be designed to withstand 5-psig (35 kPa) test pressure with a 5:1 safety factor.

f. Ladders:

1) Ladders shall be the standard FRP ladder as supplied by tank manufacturer.

g. Baffles and Partitions:

1) Baffles and Partitions shall be capable of withstanding hydrostatic loads occurring when one compartment is empty and the remaining compartment(s) is full.

3.2 Pumps

All pumps shall be approved for use in the treatment unit as described in these specifications.

Pumps shall be 3/4 to 2.0 HP, 240 VAC, three-phase, 60 Hz, three-wire motor, with 30 foot long extra heavy duty (SOOW) electrical cord with ground. The pumps shall be submersible High-Head Effluent pumps. Pumps shall be UL and CSA listed for use with effluent. The pumps must have a minimum 24-hour run dry capability without water lubrication while motor is submerged in liquid.

The pumps shall have a 1/8-inch bypass orifice to ensure flow circulation for motor cooling and to prevent air bind. The pump shall have a floating impeller design to protect against up thrust and increase pump life. The pumps liquid ends must be repairable (by replacing impellers and/or diffusers) for better long-term cost of ownership. The motor shall be rated for continuous use and frequent cycling, at least 100 cycles per day. The motor cable must be suitable for Class 1, Division

1 and 2 applications. The pumps shall be lightweight for easy removal and maintenance. The pump intake screen must be 1/8-inch mesh polypropylene. The pump shall have internal thermal overload protection and internal lightning protection. All pumps shall undergo 3-point (Dead head, Design Flow, and Design Flow + 30%) wet testing at the factory to confirm performance.

3.3 Tank 1 – Primary Settling and Aeration Tank

Tank 1 shall be a 12,000-gallon fiberglass tank and used for primary settling and aeration. Tank 1 shall be 10-feet in diameter and shall include anti-buoyancy measures such as deadman, turnbuckles and straps. Tank 1 shall meet all structural, water-tightness, and accessory specifications as stated in Section 3.1. Tank 1 shall be a two-compartment tank with first compartment used for primary settling and the second compartment used for aeration. The inlet pipe to Tank 1 shall be fitted with a sanitary tee and down-tube such that flow is introduced at least 24” below the tank liquid level. A baffle shall be provided in the center of the tank and include a 4” cross-over tee with an access riser above it. The cross-over tee shall consist of two sanitary tees and down-tubes that remove and transfer water at least 24” below the water surface. The outlet pipe from Tank 1 shall also be equipped with a sanitary tee and down-tube such that water is removed from the tank at least 24” below the water surface. Tank 1 shall be equipped with an aeration manifold supplied by Orenco Systems, Inc. or approved equal, capable of delivering at least 17 cubic feet per minute (CFM) of air. Tank 1 shall be fitted with three (3) risers to grade. Risers shall be located above the inlet tee, the aeration manifold, and over the outlet tee.

3.4 Tank 2 – Final Clarification and Flow Equalization Tank

Tank 2 shall be a 12,000-gallon fiberglass tank and used for final clarification and flow equalization. Tank 2 shall be 10-feet in diameter and shall include anti-buoyancy measures such as deadman, turnbuckles and straps. Tank 2 shall meet all structural, water-tightness, and accessory specifications as stated in Section 3.1. Tank 2 shall not be compartmentalized or subdivided. The inlet pipe to Tank 2 shall be fitted with a sanitary tee and down-tube such that flow is introduced at least 24” below the tank liquid level. Tank 2 shall be equipped with a Biotube Pump Vault by Orenco Systems, Inc. or approved equal, capable of pumping at least 30 GPM @ 50-feet TDH to the new wastewater treatment plant approximately 300-feet away. Tank 2 shall be fitted with two (3) risers to grade. Risers shall be located above the inlet tee, at the center of the tank, and at the pump vault.

3.5 Tank Assemblies and Appurtenances

All pumping systems shall be supplied by a reputable manufacturer with at least five years of experience in supplying equipment for effluent sewers. References must be available upon request. Pump systems shall be Orenco Systems, Inc. high-head pumping assemblies or approved equal, and comprised of:

A. Risers

Risers shall be ribbed PVC as manufactured by Orenco Systems, Inc. or approved equal. The material shall be PVC as per ASTM D-1784 and tested in accordance with AASHTO M304M-89.

The risers shall be constructed of non-corrosive material and designed to be buried in soil. Risers shall have a minimum stiffness of 10 psi, when tested according to ASTM D2412. Risers shall be capable of withstanding a truck wheel load (54 square inches) of 2500 pounds for 60 minutes with a maximum vertical deflection of 1/2 an inch. Risers shall be at least 12 inches high, and shall be factory-equipped with Electrical and Discharge Grommets: EPDM grommets shall be provided by the manufacturer for discharge piping, vent piping, and/or the electrical conduit to assure a watertight seal.

Risers shall be required for access to internal vaults and access into the tanks for sludge pumping.

All risers shall be constructed watertight. The risers shall be attached to the tanks such that a watertight seal is provided. Risers shall extend three inches (3") above finished grade to allow for settlement and to ensure positive drainage away from the access. All risers shall be 30 inches in nominal diameter. Adhesive required to adhere the PVC or fiberglass risers to either fiberglass or ABS tank adapter shall be a two-part adhesive, Model MA8120 or approved equal. To ensure product compatibility, a single manufacturer shall supply risers, lids, and attachment components.

B. Lids

One lid shall be furnished with each access riser. Lids shall be Orenco Systems, Inc. Model FLD30GVCFI2, or approved equal, fiberglass with green non-skid finish, and provided with stainless steel bolts, and wrench. Manufacturer shall provide evidence that lids have been used successfully in continuous field service for a minimum of five years to demonstrate long-term integrity and suitability for the application. Lids shall be waterproof, corrosion resistant and UV resistant. Lids shall be flat, with no noticeable upward dome. A crown or dome of no more than 1/8-inch is allowable. Lids shall not allow water to pond on them. Lids shall have a green non-skid finish. Self-lubricating plastics, such as polyethylene, shall not be considered non-skid without addition of a non-skid coating. Lids shall form a watertight seal with the top of riser. Lids shall be capable of withstanding a truck wheel load (54 square inches) of 2500 pounds for 60 minutes with a maximum vertical deflection of 3/4 of an inch. Lids shall be provided with tamper-resistant stainless steel fasteners and a tool for fastener removal. Tamper-resistant fasteners include recessed drives, such as hex, torx, and square. Fasteners that can be removed with common screwdrivers, such as slotted and Phillips, or fasteners that can be removed with standard tools, such as pliers or crescent wrenches, are not considered tamper-resistant. To prevent a tripping hazard, fasteners shall not extend above the surface of the lid. Rigid closed-cell foam insulation of 2-inch thickness shall be mechanically attached to the underside of the lid. All fasteners shall be made of corrosion resistant stainless steel. The insulation shall have an R-value of no less than 10 per 2-inch increment. Lids shall come pre-equipped with a vent and carbon filter.

C. Biotube Pump Vault

Orenco Systems, Inc. Model PVU Series, Universal Biotube Pump Vault Model PVU95-1819-L or approved equal, shall be provided. The filter shall have a minimum effective screen area of no less than 14.5 square feet. The Biotube pump vault shall consist of a 12-inch diameter polyethylene vault with eight (8) 2-inch diameter holes evenly spaced around the perimeter, located appropriately to allow for maximum sludge and scum accumulation before requiring pumping (approximately 70% of minimum liquid level). Housed inside the polyethylene vault shall be the Biotube assembly consisting of 1/8-inch mesh polypropylene tubes. Attached to the vault is a flow inducer to accept one or two high-head effluent pumps. A separate Universal Biotube Pump Vault Model PVU84- 1819-L, or approved equal, shall also be provided for the final tank at the new wastewater treatment facility.

D. Discharge Hose And Valve Assembly

Orenco Systems, Inc. Model HV200BSXPR or approved equal, 2-inch diameter, 150-psi PVC ball valve, 200-psi PVC True union ball check valve, PVC flex hose with minimum working pressure rating of 60 psi, and Schedule 40 PVC pipe. For cold weather climates and deep bury tanks, Orenco Systems, Inc. Model HVCW200-KIT or approved equal shall be incorporated with the Hose and Valve Assembly. It shall include a 2” Schedule 40 PVC elbow with a 1/8” drain back orifice, Schedule 80 nipple, and male adapter. An anti-siphon valve shall be incorporated into each Hose and Valve Assembly and shall be an Orenco Systems, Inc. 150-psi Schedule 40 PVC anti-siphon valve, Model HVAX200 or approved equal.

E. Float Switch Assembly

Orenco Systems, Inc. Model MF4P or approved equal with four mechanical switch floats mounted on a PVC stem attached to the filter cartridge. The floats must be adjustable and must be removable without removing the pump vault. The high/lag- and low-level alarms, timer on/off and override on/off function shall be preset. Each float lead shall be secured with a nylon strain relief bushing at the splice box. The floats shall be UL or CSA listed and shall be rated for a minimum of 5.0A @ 120 VAC. Floats shall be supplied with a minimum lead length of 20-feet.

F. Blowers

Blowers shall be Model 3BA7310-0AT26 as manufactured by AIRTECH or approved equal. Blower shall be 1.3 HP, 230VAC, three-phase, 60 Hz, three-wire motor housed in an FRP enclosure manufactured by Orenco Systems, Inc. or approved equal. Two Blowers shall be provided.

Blowers shall be integrated into the pump control panel as specified below.

G. Airflow Meter and Valves

An air flow meter and stainless shut-off valve shall be provided to monitor air flow from the blower to the aeration equipment in the tank.

H. In-Tank Diffuser

Two one-meter tube-type fine bubble air diffusers shall be provided with the equipment. The diffusers shall be Flexair 88F-18 Magnum diffusers as manufactured by Environmental Dynamics, Inc. or approved equal. All required airline and hardware, including a tube and airline connector/manifold shall be provided with the aeration system.

I. High Head Effluent Pumps (Secondary Tank)

All pumps shall comply with general requirements set forth in Section 3.2 (above). Orenco Systems, Inc., Model PF300732-30 series or approved equal, 3/4-2.0 HP, 230 VAC, three-phase phase, 60 Hz, three-wire motor, with 30 foot long extra heavy duty (SOOW) electrical cord with ground. Pump shall be UL and CSA listed as an effluent pump. The Government will provide a system head curve prior to the submittal phase for final pump selection. The pump discharge piping shall include an adjustable flow restriction device that will increase total pump head as needed to achieve the desired operating condition and flow rate for the pumps.

All pumps shall comply with general requirements set forth in Section 3.2 (above). K. Electrical Splice Box

Orenco Systems, Inc. Model SBEX1-4 series external splice box or approved equal, shall be provided. The unit shall be UL approved for wet locations and equipped with up to four (4) electrical cord grips and two 3/4-inch outlet fittings. UL listed waterproof butt splice connectors for each cables provided with 50% extra shall also be included.

3.6 Controls and Control Panel

A. Controls and alarms shall be listed per UL 508. Panels shall be repairable in the field without the use of soldering irons or substantial disassembly.

B. A dedicated ethernet cable or phone line shall be installed and is required to allow real-time connectivity with the telemetry control panel and alarm communication. Phone dialers shall not be considered as an equivalent. The panel shall be a floor mount.

C. Panel shall be Orenco Systems, Inc. TCOM™ control panel or approved equal, meeting the following:

• Data Collection and Utilization: Logs data for system conditions and events such as daily flows, pump run time, pump cycles, and alarm conditions. Logs shall store data for at least a year.

• Downloadable Logs: Download logs into a *.dif or ASCII format for simple conversion to common spreadsheet or word processor programs.

• Multi-Level Password Security: Only qualified personnel can remotely access the system.

• Program Logic Rules: Simple “If … then” declarations.

• Rules can be written based on several operands, including the following:

• Input/output status

• Point status

• Date: mm/dd/yy format

• Time of day: 24 hour clock

• Timers

• Historical data (allows for control optimization or detection of trends)

• Schedule functions to control digital “Points” based on date or day of week/time.

• Automatic daylight savings time adjustment.

• Automatic call-out to pagers during alarm conditions when panel detects trends that could lead to system failure.

D. In addition, the unit shall have the capability of real-time direct connection to the panel via laptop serial port, to allow the operator real-time access to detailed logged data and the ability to change point values.

E. Standard Panel Components

1. Stainless steel panel and deadfront.

2. Motor-Start Contactor: 17-FLA, 1-2 HP, 60-Hz; 2.5-million cycles at FLA (10-million at 50% of FLA for 230-VAC.

3. HOA 3- Way Toggle Switch: Single-pole switch, Hands (manual) Off, Auto ON. 20-amps, 1-HP.

4. Controls Circuit Breaker: 10-amps, OFF/ON switch. Single-pole 120-VAC. DIN rail mounting with thermal magnetic tripping characteristics.

5. Pump Circuit Breaker: 20-amps, OFF/ON switch. Single-pole for 120-VAC or double-pole for 230-VAC. DIN rail mounting with thermal magnetic tripping characteristics.

6. Audio Alarm: 80-dB at 24", warble-tone sound.

7. 120-VAC Ground Fault Circuit Interrupter (GFCI) Receptacle.

8. Current Sensor: 120-VAC with adjustable high & low alarm set points.

9. Visual Alarm: 7/8" diameter red lens, “push-to-silence.” NEMA 4, 1-watt bulb, 115-VAC.

10. Panel Enclosure: NEMA 4X rated, Stainless Steel or Fiberglass; hinges and latch are stainless steel. Conduit couplings provided.

11. Remote Telemetry Unit: ATRTU-Net; self-powered 24-VDC at 10-mA max, 8 digital inputs, 8 analog inputs expandable to 16 with expansion board. On-board modem (9600-baud), Ethernet port (10 base T, RJ45jack) and Modbus port (R5422/485 terminals).

12. Touch Screen Display: interface module with 5.7 color touch screen.

13. Discharge Pumps; ¾-2.0 HP, FLA, and logic (PT, etc.)

14. Effluent Alarm: 95-db at 24", warble-tone sound.

15. Flashing Light: Lexan lens, flanged base, red, UL-recognized.

16. Heater: Anti-condensation heater. Self-adjusting, radiates additional wattage as temperature drops.

17. Surge Arrestor: AG2401 120/230V, three 18” leads, rated for a maximum of 32,000amps, UL/CSA listed.

18. Disconnect switch mounted on deadfront.

19. Power for blowers. Blower shall have capability to be turned on/off and run during specified time frames via the touch screen display. Blowers shall alternate between starts.

20. VFD for each blower manufactured by Schneider Electric or approved equal.

F. Quick Disconnect Features. The control panel will be located within Summersville’s flood control pool, and subsequently, will need to incorporate provisions for rapid disconnection and removal when flooding is anticipated. This provision affects mounting and wiring connections.

1. Mounting: Provisions shall be provided for quick and easy removal of the control panel from its mounting stand. Sufficient hardware shall be provided such that when installed, the panel is securely mounted and weatherproof (high winds, etc.) All quick mounting hardware shall be stainless steel and compatible with the control panel enclosure material to prevent galvanic corrosion. The supplier shall propose a mounting-system for approval during the submittal process.

2. Wiring Connections: All wiring to the control panel for power, emergency power, control devices, and equipment loads shall be made with a receptacle/plug type setup. All connections providing power (normally energized), shall be the receptacle (female) type. All connections receiving power (load side), shall be of the plug (male) type. Receptacle/plug connections shall be configured on the panel for weather resistance. Oversizing of the control panel enclosure to accommodate receptacles may be required.

3.7 Serviceability

The advanced treatment system components shall be completely serviceable, with easy access to the pump(s), effluent screen, aeration system, and floats. The pump shall be designed for removal without removing the effluent screen and floats.

3.8 Spare Parts

The manufacturer shall provide spare one spare pump of each type of pump used, spare control panel parts (two motor contactors and breakers rated for the pumps provided), and three (3) floats with the equipment.

4.0 Submittals

The contractor shall furnish six (6) Hardcopy sets and one digital copy (on CD/DVD) of all required submittals. The submittals shall clearly specify the materials of construction, equipment compatibility, and drawings for each unique item being supplied. The Government shall approve the submittals before any items are manufactured or delivered. The contractor shall provide the following submittals:

A. Manufacturer Experience B. Product Literature C. Technical Data Sheets D. Shop Drawings for each component E. Shipping, Unloading, and Handling Requirements F. Installation Manual or Instructions G. Operation and Maintenance Manuals

5.0 Delivery

Delivery shall be included in the cost of the materials provided. The pretreatment system components shall be delivered to the Pretreatment Facility Site in the Battle Run Campground at:

Contractor shall make contact with Summersville Lake, 3 days prior to delivery.

A. Business Hours: 0730-1600 Eastern Time Monday through Friday. Work may continue outside these times with permission from the onsite government representative. All work shall be performed during daylight hours. Delay of work may occur during excessive rainfall or wet periods due to access to the site.

B. Federal Holidays: When one of the designated holidays falls on a Sunday the following Monday is observed as a legal holiday. When a legal holiday falls on a Saturday, the preceding Friday is observed as a holiday. Work that interferes with other ongoing functions at the project shall be scheduled around normal business hours to be approved by Project Manager. Work shall be scheduled around the above listed Federal holidays unless approved in advance.

Ten Federal holidays observed are as follows:

New Years Day (January 1st) Martin Luther King Day (3rd Monday in January) President’s Day (3rd Monday in February) Memorial Day (Last Monday in May) Independence Day Labor Day (1st Monday in September) Columbus Day (2nd Monday in October) Veterans Day (November 11th) Thanksgiving Day (4th Thursday in November) Christmas Day (December 25th)

C. Access to Work Location: Access to the work location shall be through the main road of the office.

D. Contractor and Government Equipment and Supplies: The contractor shall furnish all personnel, equipment, transportation, disposal, supplies and materials to perform the work. No government equipment or supplies shall be used for this project.

6.0 Warranty

The advanced treatment system manufacturer shall provide a three (3) year warranty for the entire treatment system, including, but not limited to the pump, pump vault, hose and valve assembly, control panel, and splice box. Warranty term shall ensue after the government’s acceptance and system startup procedures are complete. The manufacturer shall submit detailed exclusions from the warranty or additional cost items required to maintain the equipment in warrantable condition.

The warranty shall be documented in product literature. The manufacturer shall, upon request, furnish a two-year warranty bond. The warranty bond will guarantee the performance of the equipment in the event that it fails within the bond period.

7.0 Manufacturer Site Visits

The manufacturer shall provide a properly trained and experienced representative for a minimum of two (2) visits consisting of one (1) eight-hour day each. The visits shall be for:

A. Quality Control, Installation Inspection, and Field Testing Training B. System Commissioning and Operation and Maintenance Training

Installation Inspection and Field Testing Training shall consist of the manufacturer’s representative instructing the government personnel in the proper installation and field testing of each equipment component per the manufacturer’s recommendations and requirements.

System Commissioning and Operation and Maintenance Training shall consist of the manufacturer’s representative inspecting the installation of the equipment before start-up, assisting with start-up of the new equipment in accordance with the manufacturer’s procedures, and providing a training session on the recommended procedures for operation and maintenance of the new equipment.

8.0 MINIMUM SAFETY REQUIREMENTS

The Minimum Personal Protective equipment required for anyone on the construction site shall be long pants, safety toe shoes/boots, shirts with minimum 4 inch sleeves, reflective safety vest, hard hat, safety eyewear, and hearing protection as needed. Additional Personal Protective Equipment may be needed and should be determined using an Activity Hazard Analysis. All Personal Protective Equipment shall be in compliance with the U.S. Army Corps of Engineers Safety & Health Requirements Manual EM 385-1-1 as currently revised.

9.0 SECURITY REQUIREMENTS

All Contractors and subcontractor employee(s) working on this contract who require access in and/or around any Huntington District facilities shall receive authorization to be on site at these facilities based upon a favorable background investigation in accordance with “Homeland Security Presidential Directive 12: Policy for a Common Identification Standard for Federal Employees and Contractors” (HSPD 12).

The Huntington District Security Management Office shall submit contractor(s) personal information in order to complete the basic background investigation in order to make a recommendation to the Contracting Office of an employee’s suitability for work on Government property. The Procuring Contracting Officer (PCO) has the right to deny any contractor or subcontractor employee from working on the contract based upon the results provided by the Huntington District Security Management Office of the background investigation. The Government, Contractor, and subcontractors shall not permit any employee(s) access to drawings, blueprints, records, photos, sites, or projects until the Huntington District Security Management Office has completed an Operations Security (OPSEC) review of such items and determined they can be released.

Contractors shall submit a complete investigation packet for each contractor and subcontractor employee to the Huntington District Security Management office within 10 calendar days after award of any contract, or prior to the individual being permitted unescorted access in or on District facilities. The Contractor must also provide the following:

(1) Name of the Contract Specialist with whom he/she is working,

(2) Project Name,

(3) Contract Number and

(4) Task Order Number when applicable.

The documents listed below are required:

SF 85, Questionnaire for Non-Sensitive Positions (see http://www.opm.gov/forms/ to print form).

FD 258, Contractor Fingerprint Card (one card for digital prints, two for ink prints) (NOTE: a trained/qualified individual must take prints, ORI block on the FD 258 must be USOPM000Z) (Contractor Fingerprint Cards (FD-258) may be obtained by Contracting Division by contacting the Huntington District Security Management office).

Contractor shall report any changes in personnel or at any time conditions change at least 5 calendar days prior to individual(s) need to have access to the site. The Contractor shall provide the same information as listed above for all new employees.

Following submission of paperwork, the Security Management Office will conduct an initial criminal check based upon the information provided. If there are no issues, Security Management Office will make recommendations to the PCO or their Designated Representative. The Security Management Office cannot give approval due to unfavorable report(s). The Security Management Office shall coordinate with Office of Counsel and will jointly make recommendations to the PCO and/or the Designated Representative should a background investigation come back with derogatory information. The PCO will make the determination whether to allow the Contractor personnel to work on the site based on Security and Office of Counsel recommendations, and the PCO will advise Contractor of the decision.

FOREIGN NATIONALS

The Contractor shall provide the package listed above, as well as additional materials noted below for all Foreign Nationals. The Contractor is to expect additional time in the approval process for all Foreign Nationals due to the level of review and elevation of the review to Headquarters Staff to accomplish. All Foreign Nationals must comply with the following paragraph:

In accordance with Engineering Regulation 380-1-18, “Technology Transfer, Disclosure of Information and Contacts with Foreign Representatives” dated 1 August 1996, Section 4, all foreign nationals who work on Corps of Engineers’ contracts shall be approved by the Headquarters US Army Corps of Engineers (HQ USACE) Foreign Disclosure Officer before beginning work on this contract; this regulation includes subcontractor employees. The Contractor shall submit to the Security Management Office, the names of all foreign nationals proposed for performance under this contract, along with documentation to verify legal admittance into the United States. Such documentation shall include at least one of the following a U.S. passport, Certificate of U.S. Citizenship (INS Form N-560 or N-561), Certificate of Naturalization (INS Form N-550 or N-570), Alien Registration Card with photograph (INS Form I-151 or I- 551), Employment Authorization Card (INS Form I-688A), etc.

In order to assure forms are properly completed, and to minimize time delays, Contractor is encouraged to work closely with the Security Management Office. All Foreign National Visit Requests MUST be submitted to the Security Management Office 30 days in advance of the visit per Army Regulation 380-10, Foreign Disclosure and Contacts with Foreign Representatives. The Government will return http://www.opm.gov/forms/ for correction incomplete forms, illegible forms, etc. which will delay the contractor / subcontractor from beginning work.

Security requirements are subject to change in accordance with HSPD-12, Office of Personnel Management, regulation or policy changes, etc. The Government shall add any changes to security requirements by modification to the contract.

AT OPSEC Requirements

Access and General Protection/Security Policy and Procedures. All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation/facility access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any installation or facility change, the Government may require changes in contractor security matters or processes.

Pre-screen candidates using E-Verify Program. The Contractor must pre-screen Candidates using the E-verify Program (http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements. The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the COR no later than 3 business days after the initial contract award." *When contracts are with individuals, the individuals will be required to complete a Form I-9, Employment Eligibility Verification, with the designated Government representative. 52.204-16 COMMERCIAL AND GOVERNMENT ENTITY CODE REPORTING (JUL 2016)

(a) Definition. As used in this provision--

Commercial and Government Entity (CAGE) code means—

(1) An identifier assigned to entities located in the United States or its outlying areas by the Defense Logistics Agency (DLA) Commercial and Government Entity (CAGE) Branch to identify a commercial or government entity;

or

(2) An identifier assigned by a member of the North Atlantic Treaty Organization (NATO) or by the NATO Support and Procurement Agency (NSPA) to entities located outside the United States and its outlying areas that the DLA Commercial and Government Entity (CAGE) Branch records and maintains in the CAGE master file. This type of code is known as a NATO CAGE (NCAGE) code.

(b) The Offeror shall enter its CAGE code in its offer with its name and address or otherwise include it prominently in its proposal. The CAGE code entered must be for that name and address. Enter ``CAGE'' before the number. The CAGE code is required prior to award.

(c) CAGE codes may be obtained via--

(1) Registration in the System for Award Management (SAM) at www.sam.gov. If the Offeror is located in the United States or its outlying areas and does not already have a CAGE code assigned, the DLA Commercial and Government Entity (CAGE) Branch will assign a CAGE code as a part of the SAM registration process. SAM http://www.sam.gov/ registrants located outside the United States and its outlying areas shall obtain a NCAGE code prior to registration in SAM (see paragraph (c)(3) of this provision).

(2) The DLA Commercial and Government Entity (CAGE) Branch. If registration in SAM is not required for the subject procurement, and the offeror does not otherwise register in SAM, an offeror located in the United States or its outlying areas may request that a CAGE code be assigned by submitting a request at https://cage.dla.mil.

(3) The appropriate country codification bureau. Entities located outside the United States and its outlying areas may obtain an NCAGE code by contacting the Codification Bureau in the foreign entity's country if that country is a member of NATO or a sponsored nation. NCAGE codes may be obtained from the NSPA at https://eportal.nspa.nato.int/AC135Public/scage/CageList.aspx if the foreign entity's country is not a member of NATO or a sponsored nation. 52.211-6 BRAND NAME OR EQUAL (AUG 1999)

(a) If an item in this solicitation is identified as “brand name or equal,” the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that “equal” products must meet are specified in the solicitation.

(b) To be considered for award, offers of “equal” products, including “equal” products of the brand name manufacturer, must--

(1) Meet the salient physical, functional, or performance characteristic specified in this solicitation;

(2) Clearly identify the item by--

(i) Brand name, if any; and

(ii) Make or model number;

(3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and

(4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications.

(c) The Contracting Officer will evaluate “equal” products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer.
