Per FAR 36.204 the estimated price range for this project is between $25,000 and $100,000.
NOTE 1: In accordance with FAR Part 28.102-1(b)(1), the Contractor will be required to provide one of the following payment protections for all construction contracts greater than $35,000.00 but not greater than
$150,000.00.
(i) Payment bond
(ii) Irrevocable letter of credit
A Payment Bond or Irrecovable Letter of Credit is the SELECTED payment protection for the Government.
The penal amount of bond shall be 100% of the original contract price. If the contract price increases, the Government will require an increase in the penal sum of the existing bond. This bond shall be submitted to the Contracting Officer within 10 days after receipt of award.
The payment made under this provision shall not exceed the amount included for Bid Item 0001 of the bid schedule, which payment shall take into consideration any discounts or rebates to the Contractor from the Surety.
FURTHERMORE, THE GOVERNMENT SHALL ONLY REIMBURSE THE CONTRACTOR THE ACTUAL
AMOUNT PAID TO THE SURETY COMPANY FOR PAYMENT BONDS, LESS ANY DISCOUNTS OR
REBATES. AN INVOICE FROM THE SURETY SHALL BE PROVIDED BEFORE PAYMENT IS MADE.
CONTRACTORS ARE ADVISED THAT BID ITEM 0001 IS ONLY FOR THE COST OF BONDS. OTHER
COSTS SHOULD BE ALLOCATED TO THE REMAINING BID ITEM(S).
NOTE 2: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation and is the lowest price responsive and responsible offeror.

Contractors shall provide completed FAR clause 52.204-8 (Annual Representations and Certifications) found in Section K of this solicitation with their quote, if the Contractors have not completed their annual representations and certifications electronically via the SAM website at www.sam.gov. (See Paragraph 2 above)
7. Contractors are not required to submit an Accident Prevention Plan (APP) with their quote. The successful offeror will be required to submit this document upon award. The APP must be accepted by the Government Designated Authority prior to the preconstruction conference. EXAMPLES of the APP may be found at http://www.lrh.usace.army.mil/CT under the General Contracting Information heading.
The contractor that receives this award will have 10 calendar days to provide an Accident Prevention Plan (APP).
If this deadline is not met, the contractor will be given a cure notice stating that they will have 10 calendar days to acceptable APP, otherwise the contract will be terminated for cause.
SCOPE OF WORK
SCOPE OF WORK –SUTTON LAKE
LOWER GERALD R FREEMAN SHOWERHOUSE
1. Provide all, labor, equipment, materials, and site preparation necessary to construct the concrete floors and all interior and exterior masonry walls for a shower house 40 foot by 28 foot in size, not including privacy partitions, in Lower Gerald R Freeman campground at Sutton Lake. The walls shall be constructed 8’ high with 12 courses of block. This shall include stubbing all utilities (water, sewer, electric) to a height of one foot above floor elevation and 5 foot outside of the foundation of the shower house. Located within Gerald R. Freeman Campground is a shower house constructed with the same specifications that will be available for comparison. All work shall be done in accordance with the attached safety requirements and in a thorough, workmanlike, professional manner in accordance with all applicable codes and practices common to the trade.
2. Drawings / Documentation. Once work is complete, as-built drawings, manuals, warranties, and any other applicable documentation shall be provided to the Resource Manager.
Specifications and the attached drawings are provided to show locations and views of the building, placement of door openings, window openings, plumbing, drain locations and conduit locations.
3. Access to Site and Security
3.1. Access to the work site shall be Monday-Friday, 7:15 a.m. to 4:00 p.m., excluding Federal Holidays
Federal Holidays- When one of the designated holidays falls on a Sunday the following Monday is observed as a legal holiday. When a legal holiday falls on a Saturday, the preceding Friday is observed as a holiday.
Work that interferes with other ongoing functions at the project shall be scheduled around normal working hours to be approved by Project Manager. Work shall be scheduled around the above listed Federal holidays.
Ten Federal holidays observed are as follows: New Years Day (January 1st) Martin Luther King Day (3rd Monday in January) President’s Day (3rd Monday in February) Memorial Day (Last Monday in May) Independence Day (July 4th) Labor Day (1st Monday in September) Columbus Day (2nd Monday in October) Veterans Day (November 11th) Thanksgiving (November 24th) Christmas (December 25th)
4. Site Preparation. The Government will remove the old existing building and have a nearly level site prepared for the Contractor to construct the building on. The Contractor shall do any final grading required.
5. Layout. The contractor shall lay out the site so the building is square, walls are plumb and all gravity drain systems drain in accordance with applicable codes. The building shall be oriented so that the front of the building faces toward the lake.
6. Floor.
6.1. The Contractor shall excavate the area as needed to provide a solid foundation. The Contractor shall provide a 5 1/2" thick 4000 psi concrete floor slab brown in color over a base formed by a minimum 2" of compacted #57 limestone.
6.2. The floor shall have a hand float finish, i.e. no sharp spikes, lines, etc, but sufficient texture to prevent slipping. The floor shall be finished to assure water on the floor drains to the floor drains and that each shower stall drains to its appropriate floor drain. The concrete floor shall be poured using tinted concrete and shall be brown in color.
6.3. A 6 mil vapor barrier shall be placed over the stone before the concrete is poured. Concrete reinforcement shall include 6x6 10 gauge wire or heavier.
6.4. The contractor shall install conduits to protrude through the finished floor a minimum of 12 inches above the floor in the interior of the building at the location of the electric panel. The conduits shall run to a location 5 feet outside the foundation near the laundry room door. Three conduits shall be provided and installed and shall be 3 inches in diameter.
7. Walls.
7.1 Laying Blocks- the Contractor shall professionally perform all work necessary to lay the blocks to form the exterior and interior walls for the building.
7.2. All masonry blocks on the exterior walls and above final grade shall be split faced and standard gray in color 8x8x16 in size, except the top course on the 40 foot exterior walls shall be standard 8x8x16 smooth block. Split faced corners shall be used as required. Walls shall be supported by a foundation that are standard with practices of the trade, but at a minimum of 2 foot in width and 1 foot thick 4000 psi concrete with two parallel runs of ½ inch rebar encased in them.
7.3. Interior pipe chase walls shall be standard 8x8x16 masonry blocks smooth on both sides and level with the exterior walls of the building. .
7.4. Shower stall walls shall be standard 6x8x16 blocks smooth on both sides.
7.5. Window rough openings of 2 foot by 40 inch shall be in the locations shown on the attached drawings. A total of 8 window openings are required.
7.6. Door rough openings of 40 inches X 90 inches shall be provided in the locations shown. A total of 4 door openings are required.
7.7. Privacy partitions at the entrance to the men’s and women’s restroom shall be 4 foot 8 inches by 6 foot 8 inches and 5 foot 5 inches high constructed with split faced block standard gray in color 8x8x16 in size and capped with a beveled hand finished concrete cap. Concrete sidewalk shall be poured within this area and shall have a hand float finish, i.e. no sharp spikes, lines, etc, but sufficient texture to prevent slipping. The area shall be finished to assure water drains away from the building and the door. The concrete in this area will not include tint.
8. Plumbing.
8.1. Plumbing shall exit the building at an elevation that allows flow into an existing gravity flow sewer line. All drain and waste plumbing shall be 4 inch schedule 40 pvc. All sewer drops in pipe chase shall extend a minimum of one foot above finished floor and shall be 6” from the pipe chase wall.
8.2. Water line shall be provided and installed by the contractor to extend from the location shown on the drawing in the pipe chase and extending 5 feet outside the foundation of the building. This water line shall be 2 inch PVC.
8.3 The Contractor shall provide and install floor drains, sewer pipe, water lines and conduit at locations as shown on the attached drawing or as described in these specifications.
9. Safety
9.1. This work shall be performed at Lower Gerald R Freeman campground at Sutton Lake, and safety shall be properly analyzed by the Contractor and practiced at all times to prevent injuries.
The Contractor shall prepare an Accident Prevention Plan in accordance with EM 385-1-1 Appendix A. The Contractor shall employee a Site Safety and Health Officer (SSHO) as specified in EM 385-1-1 Section 01.A.17.
ACCESS TO WORK LOCATION
Access to the work locations shall be coordinated with the Resource Manager. Gate keys will be issued to the Contractor for areas that are locked. The recreation areas will be closed to public access during this work. For non‐gated areas, the Contractor shall be responsible for temporarily barricading the work site from public use. The Contractor shall ensure that access is provided to Corps of Engineers personnel travelling through work areas and shall coordinate access issues with the onsite government representative well in advance. .
Key Control:
1. In accordance with AR 190-51 Appendix D (Issuing and Distribution of Government Keys to Non-Corps Personnel), a written key request must be made to the project Resource Manager for the person(s) who will physically need a key. A copy of the individuals’ driver license(s) will be required and placed in the project’s key custodian’s file with the completed key request. The project key custodian shall go over the key request and the responsibility of possession and maintaining the security of the key(s) during the time the key is being issued such as:
A. Ensuring the security of the key by keeping it on your person at all times.
B. Never allowing pass back (passing off the key to someone so that they can use the key for access) to occur.
C. Never allowing unauthorized personnel to follow in behind you when you use your key.
D. Reporting any lost or stolen keys to the key custodian at the project as soon as possible.
E. Any violations regarding misuse, loss or otherwise are subject to investigation and termination of your privileges to possess a key to any government property.
2. All keys will be returned when their use is no longer needed. If a person retires or leaves the contractor for any reason, prior to job completion, their key will be surrendered immediately.
The contractor will then select a new representative to have a government key issued to and forward them to the project office for the issuance of a key by submitting a new key request.
CONTRACTOR AND GOVERNMENT EQUIPMENT AND SUPPLIES
The Contractor shall furnish all personnel, equipment, transportation, disposal, supplies and materials to perform the work.
Contractor Accident Prevention Plan (APP).
A. General. If the Contractor fails or refuses to promptly comply with safety and security requirements as specified herein, any project employee may stop all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to any such work stoppage shall be made subject to any claims for extension of time or for excess costs or damages sustained by the Contractor. Also, the Contractor will not be paid for work not performed as a result of the stop order.
The Contractor shall comply with all current provisions of the Occupational Safety and Health Act (OSHA), as well as all State and Local laws and requirements in addition to the most recent edition and revisions of the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1 and as otherwise stated herein.
All employees involved in providing services shall read and review the applicable Activity Hazard Analysis (AHA) for that job. All employees performing work under this contract shall wear personal protective equipment as specified in EM 385-1-1 including as a minimum steel-toed safety boots, hard hat, safety glasses, hearing protection, sleeved shirts, long pants, and gloves and other equipment common to trade as specified in EM 385-1-1.
B. Contractor Accident Prevention Plan (APP). The Contractor shall prepare a site-specific Accident Prevention Plan following the outline provided in EM 385-1-1, Appendix A as a minimum. The contractor shall address each element and sub-element in the outline in the order they appear in Appendix A. If other areas of the EM 385-1-1 are pertinent to the contract the contractor must assure these areas are addressed as well. Any item from the outline that does not apply must be listed, in its assigned place and a brief explanation of the reason it is not required provided. This plan must be accepted by the Contracting Officer prior to commencement of any work. Contractors are encouraged to contact the USACE Safety & Occupational Health Office for questions or information regarding the development of contract Accident Prevention Plans. (1) The contractor shall have on site an employee designated as a Site Safety and Health
Officer (SSHO) who has completed the OSHA 30-hour Construction Safety training or its equivalent as specified in EM 385-1-1, Appendix A, paragraph 4 and has at least five years experience. No work will be permitted unless the SSHO is present at the work site. Work crews of two or more must have two people holding current certification in First Aid and CPR from the American Red Cross, American Heart Association. Training must include a hands-on element and the contractor must maintain appropriate First Aid kit on site. Single individuals working alone must have current certification in First Aid only.
(2) All work that potentially exposes the worker to falls of six feet or more will require the contractor to submit a Site Specific Fall Protection & Prevention Plan in accordance with EM 385-1-1, Section 21.C.
(3) Cranes and hoisting equipment shall be operated only by qualified personnel
(operators licensing, physical and drug testing in accordance with EM 385-1-1 Section 16.B.01 must be submitted). Note: CDL Medical Clearance is not acceptable. Please see attached medical clearance requirements.
(4) Effective November 8, 2010 all “Signalpersons” and “Riggers” shall be qualified.
This is a mandatory requirement in accordance with 29 CFR 1926.1400 Regulations. Riggers must be a qualified person for the performance of specified hoisting activities such as during assembly/disassembly, in accordance with OSHA 1926.1404(r)(1) and 1926.1425(c)(3).
C. Activity Hazard Analysis (AHA). The Contractor shall prepare and use an AHA outlining specific job steps and the hazards associated with those steps as one of the tools to insure safe performance of work. It is the Contractor's responsibility to prepare the AHA. The AHA shall be accepted by the GDA and/or on-site government representative prior to commencement of work. All on-site employees shall review the approved AHA prior to performing work and during weekly and “Tool Box” safety meetings as appropriate.
1. AHA shall define the activities being performed and identify the work sequences, the specific anticipated hazards, site conditions, equipment, materials, and the control measures to be implemented to eliminate or reduce each hazard to an acceptable level of risk. AHA should indicate compliance with EM 385-1-1 personal protective equipment requirements (safety-toed work boots, safety glasses, gloves, hard hats, etc.), work crews of two or more must have two people currently qualified in first aid and CPR and the appropriate first aid kit on site. Single individuals working alone must have current certification in First Aid only.
D. Accident Reporting: The Contractor shall report all accidents; accidents shall be investigated to determine causal factors and corrective actions shall be implemented. Accidents resulting in personal injury or property damage of $5,000.00 or more shall be investigated and reported to the GDA using the USACE Accident Investigation Report Form ENG 3394 within 24 hours of the occurrence. All serious accidents (those resulting in death, disability, three or more persons hospitalized, property damage of $200,000.00 or greater, any Arc Flash Incident or if three or more individuals become ill or have a medical condition suspected to be related to a site condition, any crane or hoisting incident) shall be reported immediately. The Contractor shall maintain an OSHA Log of Work Related Injuries and Illnesses, OSHA Form 300 and comply with all accident reporting and recordkeeping requirements specified in 29 CFR 1904.
E. On-site Public Safety: Work on this contract is immediately adjacent to public recreation facilities. Safety precautions to warn and limit public access shall be addressed in both the APP and AHA.
F. Damage Reports: All instances where Government materials supplies, property, and/or equipment are damaged shall be reported immediately to the Resource Manager.
G. Minimum Personal Protective Equipment: Long pants, safety toe shoes/boots, short-sleeved shirt, reflective vests, hard hat and safety eyewear.
H. Pre-Construction Conference: The Contractor shall attend a pre-construction conference, prior to the initiation of work under this contract. The Contractor’s Safety Plan must be accepted prior to this meeting.
CONTRACTOR SUBMITTALS
The successful offeror shall provide as a minimum the following documentation:
A. Accident Prevention Plan – EM 385-1-1 Appendix A and APP (Accident Prevention Plan). These forms are located at the following website:

Wage Rates. In accordance with contract clause FAR 52.222-6, Davis-Bacon wage rates are applicable to this procurement and can be located at http://www.dol.gov. Applicable wage rates for this procurement are: General Decision number WV160004 01/08/2016 WV4.
SECURITY REQUIREMENTS
All Contractors and subcontractor employee(s) working on this contract who require access in and/or around any Huntington District facilities (i.e. flood control dams, locks and dams, etc.)
shall receive authorization to be on our facilities based upon a favorable background investigation in accordance with “Homeland Security Presidential Directive 12: Policy for a Common Identification Standard for Federal Employees and Contractors” (HSPD 12). The Huntington District Security Management Office shall submit contractor(s) information in order to complete the background investigation and make a recommendation to the Contracting Office of an employee’s suitability for work on Government property. The Procuring Contracting Officer (PCO) has the right to deny any contractor or subcontractor employee from working on the contract based upon the results provided by the Huntington District Security Management Office of the background investigation. The Government, Contractor, and subcontractors shall not permit any employee(s) access to drawings, blueprints, records, photos, sites, or projects until the Huntington District Security Management Office has completed a review of such items and determined they can be released. Contractor’s failure to adhere can result in penalties, fines, or sanctions up to and including contract termination and/or forfeiture of future contract consideration.
Contractors shall submit a complete investigation packet for each contractor and subcontractor employee to the Huntington District Security Management office within 10 working days after award of any contract, or prior to the individual being permitted unescorted access in or around all District facilities. The Contractor must also provide the following: (1) Name of the Contract Specialist with whom he/she is working, (2) Project Name, (3) Contract Number and
(4) Task Order Number when applicable.
The documents listed below are required:
SF 85, Questionnaire for Non‐Sensitive Positions (see http://www.opm.gov/forms/ to print form). FD 258, Contractor Fingerprint Card (one card for digital prints, two for ink prints) (NOTE: a trained/qualified individual must take prints, ORI block on the FD 258 must be USOPM000Z) (Contractor Fingerprint Cards (FD‐258) may be obtained by Contracting Division by contacting the Huntington District Security Management office).
Contractor shall report any changes in personnel or at anytime conditions change at least 5 working days prior to individual(s) need to have access to the site. The Contractor shall provide the same information as listed above for all new employees.
Following submission of paperwork, the Security Management Office will conduct an initial criminal check based upon the information provided. If there are no issues, Security Management Office will make recommendations to the PCO or their Designated Representative. The Security Management Office cannot give approval due to unfavorable report(s). The Security Management Office shall coordinate with Office of Counsel and will jointly make recommendations to the PCO and/or the Designated Representative. The PCO will make the determination whether to allow the Contractor personnel to work on the site based on Security and Office of Counsel recommendations, and the PCO will advise Contractor of the decision.
FOREIGN NATIONALS
The Contractor shall provide the package listed above, as well as additional materials noted below for all Foreign Nationals. The Contractor is to expect additional time in the approval process for all Foreign Nationals due to the level of review and elevation of the review to Headquarters Staff to accomplish. All Foreign Nationals must comply with the following paragraph:
In accordance with Engineering Regulation 380‐1‐18, “Technology Transfer, Disclosure of Information and Contacts with Foreign Representatives” dated 1 August 1996, Section 4, all foreign nationals who work on Corps of Engineers’ contracts shall be approved by the Headquarters US Army Corps of Engineers (HQ USACE) Foreign Disclosure Officer before beginning work on this contract; this regulation includes subcontractor employees. The Contractor shall submit to the Security Management Office, the names of all foreign nationals proposed for performance under this contract, along with documentation to verify legal admittance into the United States at least 10 business days (excluding Federal holidays) prior to the requested work start date. Such documentation shall include at least one of the following a U.S. passport, Certificate of U.S. Citizenship (INS Form N‐560 or N‐561), Certificate of Naturalization (INS Form N‐550 or N‐570), Alien Registration Card with photograph (INS Form I‐ 151 or I‐551), Employment Authorization Card (INS Form I‐688A), etc.
All foreign government Requests for Visit Authorizations to the Department of Army organizations must be submitted at least ten business days (excluding Federal holidays) prior to the requested start date of the visit due to attempting to support ,accommodate and approve the high number of short‐fused Requests for Visit Authorizations (RVAs) received into the Foreign Disclosure Office on a daily basis.
In order to assure forms are properly completed, and to minimize time delays, Contractor is encouraged to work closely with the Security Management Office. The Government will return for correction incomplete forms, illegible forms, etc. which will delay the contractor / subcontractor from beginning work.
Holiday Moratorium ‐ The Department of Defense traditionally imposes a moratorium on Requests for Visit Authorization (RVA's) during the December and January timeframes each calendar year. Where it is possible, the Contractor should avoid scheduling visits unless they are mission essential during this timeframe. The DOD Foreign Visits System will be operational;
however, Army foreign disclosure will only process RVA's designated as mission essential by the Contracting Officer. The only two exceptions to the 10 working day suspense requirement are
(1) visits to the Army Secretariat and Army Staff, and (2) visists deemed mission essential or time sensitive by the U. S. Army. A waiver to the Office of the Deputy Chief of Staff, G‐2 must be submitted to authorize the request.
Security requirements are subject to change in accordance with HSPD‐12, Office of Personnel Management, regulation or policy changes, etc. The Government shall add any changes to security requirements by modification to the contract.

Antiterrorism Requirements
Access and General Protection/Security Policy and Procedures. All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation/facility access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office.
Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any installation or facility change, the Government may require changes in contractor security matters or processes.
For contractors who do not require CAC, but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05 / AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative, as
NCIC and TSDB are available), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.
Suspicious Activity Reporting Training (e.g. iWATCH, CorpsWatch, or See Something, Say Something). The contractor and all associated sub-contractors shall receive a brief/training (provided by the RA) on the local suspicious activity reporting program. This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the project manager, security representative or law enforcement entity. This training shall be completed within 30 calendar days of contract award and within 30 calendar days of new employees commencing performance with the results reported to the COR NLT 5 calendar days after the completion of the training.
Pre-screen candidates using E-Verify Program. The Contractor must pre-screen Candidates using the E-verify Program (http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements. The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the COR no later than 3 business days after the initial contract award."
*When contracts are with individuals, the individuals will be required to complete a Form I-9, Employment Eligibility Verification, with the designated Government representative. This Form will be provided to the Contracting Officer and shall become part of the official contract file.
APPENDIX A
MINIMUM BASIC OUTLINE FOR ACCIDENT PREVENTION PLANS
An Accident Prevention Plan (APP) is a safety and health policy and program document. The following areas are typically addressed in an APP, but an APP shall be job-specific and shall also address any unusual or unique aspects of the project or activity for which it is written.
The APP shall interface with the employer’s overall safety and health program, and a copy shall be available on the work site. Any portions of the overall safety and health program that are referenced in the APP shall be included as appropriate. ANSI/ASSE A10.38 should be referenced for Programmatic Issues.
1. SIGNATURE SHEET. Title, signature, and phone number of the following:
a. Plan preparer (Qualified Person, Competent Person, such as corporate safety staff person, QC);
b. Plan must be approved, by company/corporate officers authorized to obligate the company;
c. Plan concurrence (e.g., Chief of Operations, Corporate Chief of Safety, Corporate Industrial Hygienist, project manager or superintendent, project safety professional, project QC). Provide concurrence of other applicable corporate and project personnel (Contractor).
2. BACKGROUND INFORMATION. List the following:
a. Contractor;
b. Contract number;
c. Project name;
d. Brief project description, description of work to be performed, and location; phases of work anticipated (these will require an AHA).
3. STATEMENT OF SAFETY AND HEALTH POLICY. Provide a copy of current corporate/company Safety and Health Policy Statement, detailing commitment to providing a safe and healthful workplace for all employees.
The Contractor’s written safety program goals, objectives, and accident experience goals for this contract should be provided.
4. RESPONSIBILITIES AND LINES OF AUTHORITIES. Provide the following:
a. A statement of the employer’s ultimate responsibility for the implementation of his SOH program;
b. Identification and accountability of personnel responsible for safety at both corporate and project level.
Contracts specifically requiring safety or industrial hygiene personnel shall include a copy of their resumes.
Qualifications shall include the OSHA 30-hour course or equivalent course areas as listed here:
(1) OSH Act/General Duty Clause;
(2) 29 CFR 1904, Recordkeeping;
(3) Subpart C: General Safety and Health Provisions, Competent Person;
(4) Subpart D: Occupational Health and Environmental Controls, Citations and Safety Programs;
(5) Subpart E: PPE, types and requirements for use;
(6) Subpart F: understanding fire protection in the workplace;
(7) Subpart K: Electrical;
(8 Subpart M: Fall Protection;
(9) Rigging, welding and cutting, scaffolding, excavations, concrete and masonry, demolition; health hazards in construction, materials handling, storage and disposal, hand and power tools, motor vehicles, mechanized equipment, marine operations, steel erection, stairways and ladders, confined spaces or any others that are applicable to the work being performed.
c. The names of Competent and/or Qualified Person(s) and proof of competency/qualification to meet specific OSHA Competent/Qualified Person(s) requirements must be attached. The District SOHO will review the qualifications for acceptance;
d. Requirements that no work shall be performed unless a designated competent person is present on the job site;
e. Requirements for pre-task safety and health analysis;
f. Lines of authority;
g. Policies and procedures regarding noncompliance with safety requirements (to include disciplinary actions for violation of safety requirements) should be identified;
h. Provide written company procedures for holding managers and supervisors accountable for safety.
5. SUBCONTRACTORS AND SUPPLIERS. If applicable, provide procedures for coordinating SOH activities with other employers on the job site:
a. Identification of subcontractors and suppliers (if known);
b. Safety responsibilities of subcontractors and suppliers.
6. TRAINING.
a. Requirements for new hire SOH orientation training at the time of initial hire of each new employee.
b. Requirements for mandatory training and certifications that are applicable to this project (e.g., explosive actuated tools, confined space entry, crane operator, diver, vehicle operator, HAZWOPER training and certification, PPE) and any requirements for periodic retraining/recertification.
c. Procedures for periodic safety and health training for supervisors and employees.
d. Requirements for emergency response training. > See paragraph 9.b. below for a list of requirements that may require emergency response training.
7. SAFETY AND HEALTH INSPECTIONS.
a. Specific assignment of responsibilities for a minimum daily job site safety and health inspection during periods of work activity: Who will conduct (e.g., SSHO, PM, safety professional, QC, supervisors, employees – depends on level of technical proficiency needed to perform said inspections), proof of inspector’s training/qualifications, when inspections will be conducted, procedures for documentation, deficiency tracking system, and follow-up procedures;
b. Any external inspections/certifications that may be required (e.g., USCG).
8. ACCIDENT REPORTING. The Contractor shall identify person(s) responsible to provide the following:
a. Exposure data (man-hours worked);
b. Accident investigations, reports, and logs: Report all accidents as soon as possible but not more than 24 hours afterwards to the Contracting Officer/Representative (CO/COR). The contractor shall thoroughly investigate the accident and submit the findings of the investigation along with appropriate corrective actions to the CO/COR in the prescribed format as soon as possible but no later than five (5) working days following the accident. Implement corrective actions as soon as reasonably possible;
c. The following require immediate accident notification:
(1) A fatal injury;
(2) A permanent total disability;
(3) A permanent partial disability;
(4) The hospitalization of three or more people resulting from a single occurrence;
(5) Property damage of $200,000 or more.
9. PLANS (PROGRAMS, PROCEDURES) REQUIRED BY THE SAFETY MANUAL. Based on a risk assessment of contracted activities and on mandatory OSHA compliance programs, the Contractor shall address all applicable occupational risks and compliance plans. Using the EM 385-1-1 as a guide, plans may include but not be limited to:
a. Layout plans (04.A.01);
b. Emergency response plans:
(1) Procedures and tests (01.E.01);
(2) Spill plans (01.E.01, 06.A.02);

The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of "identifiers" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).
Union Rate Identifiers
A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than "SU" or "UAVG" denotes that the union classification and rate were prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.
Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.
Survey Rate Identifiers
Classifications listed under the "SU" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.
Survey wage rates are not updated and remain in effect until a new survey is conducted.
