GUAM NATIONAL GUARD
SCOPE OF WORK
FOR
INSPECTION AND LOAD RATE TESTING
The Scope of Work for equipment requiring Inspection and Load Rate Testing consists of the following:
1. General
a. Scope: The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary as required to perform the inspection and load rate testing of each equipment identified on Table 1.
b. Background: The Guam National Guard’s (GUNG) Combined Support
Maintenance Shop (CSMS) has approximately fifty-four pieces of equipment which require annual inspection and load rate testing in order to support the GUNG’s daily operation and mission.
c. Period of Performance (PoP): The Period of Performance shall be one (1)
Base Year not exceeding 12 months and two (2) 12-month option years
d. Contractor shall coordinate with the Combined Support Maintenance Shop for access and location of equipment to be inspected and tested.
e. Upon completion of all inspection and testing work, the contractor shall develop and submit a summary inspection and load testing report identifying components inspected, testing procedure and collected data analysis shall be included in the report.
2. Inspection
a. The contractor shall perform inspections as a required to identify all components that require repair or replacement on each equipment on
Table 1 to ensure long term, reliable operation. Table 1 lists equipment to be inspected and evaluated by the contractor.
b. If during the inspection the contractor finds critical issues with any component of the equipment that may preclude load testing. The contactor shall inform the Combined Support Maintenance Shop prior to
Performing and load testing.
3. Testing
a. The contractor shall perform load testing on all equipment attachments as defined herein. On-site testing shall be permitted to be performed at
CSMS Bldg 800 and Bldg 32, Fort Juan Muna.
b. If during load testing of the equipment, the contractor observes operational issues with the equipment that may result in damage to the unit, the contractor shall immediately abort the testing and notify CSMS point of contact.
c. The GUARNG does not have certified weights for load testing. The contractor shall choose appropriate loads for the test based on the manufacturer’s load ratings for the condition of use. A 25% minimum partial load, the rated load, and a proof load as close as possible but not exceeding 125% of rated load shall be used. Check manufacturer’s limitations and or specific instructions on testing.
d. Rigging should be arranged for the test loads which conforms to good rigging practice for a safe lift.
e. Safety cones or markings should be set up around the operating perimeter if there is any possibility that personnel or vehicles will intrude into operating area.
f. Operators, riggers, and a signalman should establish and review signals that would be used and how the test will be conducted.
g. Inspect lifting attachments in its entirety according to the checklist normally used for each type of equipment. Particular attention should be given to the structural elements of the lifting systems which will be subjected to stress during the load testing procedure. Any evidence of degradation among the structural elements should be evaluated and a determination made as the suitability of the lifting systems condition for load testing and the need for repair or replacement for components.
4. Post-Test Inspection
a. After the tests are completed, conduct a thorough examination to ensure that lift systems has satisfactorily withstood tests. Park all equipment at the proper location for service access and relax all rigging to allow for inspection of wire cables and components.
b. Check all components of the lifting system for structural damage including cracks, weld separation, permanent deformation, paint flaking, and loose fasteners.
c. Check lift system electrical components for deterioration and proper condition, verify the integrity of wiring, connections and enclosures, checking for neatness, security and conformance.
d. Make note of any damage, deficiencies, or corrections that may be required.
e. Hoist the load blocks clear of personnel or obstructions and verify that the wire cable has wound properly on hoist drums.
f. Stencil all equipment tested that pass load testing with load test expiration date so it is visible on equipment tested.
f. Complete the report and conduct an exit briefing, if appropriate.
