POWERHOUSE ROOF
SCOPE OF WORK
1. GENERAL REQUIREMENTS
General The work is located at 7451 Lockway West, Pittsburgh, PA 15206-1104. The site of the work may be reached from state and local roads and streets. All work shall be done in a professional manner and in accordance with industry standards. The general description of the work pertains to the following building.
1.1.1. Operations Building Powerhouse Roof
The general description of the work is to furnish all equipment, labor, layout of work, supervision, and materials necessary to complete the work as shown and as specified.
Remove the existing roofing materials down to the concrete deck, roof drains, insulation, vent covers, and gravel stop from an approximately 1,700 square foot building and dispose of properly. Install a new air/vapor barrier on concrete deck and adhere one layer of 3.5-inch Polyiso insulation. Install a new, fully adhered, 60 mil EPDM roofing system, conforming to ASTM D4637/D4637M, with a new flashing system around the perimeter and all roof top penetrations. The roofing system shall have new roof drain inserts equivalent to Firestone TIS #1513, new tapered insulation minimum thickness of 1 inch and the roof shall have a 1/4 inch drop per foot towards the roof drains, and include the addition of any features to ensure positive drainage towards the roof drains. New flashing shall be installed on pipe vents protruding from roof surface. New vent cover and caps shall be installed on the generator exhaust pipe. The existing lights and electric conduit, junction boxes, etc. shall be relocated only if necessary. Once altered, they must be placed back on the new roof in fully working condition and to appropriate codes. Though the cameras have been removed, the camera systems’ mounting plates need to be permanently removed so the flashing can be placed in these areas – see pictures.
1.1.2. Labor and materials
The contractor shall provide all labor and materials necessary to replace the existing built up roof, roof drains, and associated items. Details of the building construction are shown on the attached reference drawings. The work shall include removal and disposal of all roofing materials over the entire roof down to the existing concrete deck, including all flashing, roof drains, flashing around base of the scuttle structure, and installation of new tapered insulation, flashing, roof drains, and all other work to furnish all equipment, labor, layout of work, supervision, and materials necessary to complete the work as shown and as specified.
The Contractor shall be responsible for transport of materials to the site and to prevent demolished materials from entering the river. All material provided in the performance of this contract shall be unused and meet the requirements of the specifications.
1.1.3. Scuttle structure roof
This project does not include replacement of the scuttle structure roof.
1.1.4. Existing items
The EPDM shall have a proper waterproof seal around all existing items that protrude thru or
7 | P a g e from the roof. All fixtures, such as, but not limited to: stairs, grated walkways, hatches, shall be cleaned up, touched up as necessary, and repaired to the satisfactory of the COR. The flagpole mount shall be flashed over and sealed on the inside of the wall.
1.1.5. Protection
If an electrical circuit is altered, that circuit must meet the current code. The LOMA unit shall be cribbed so the contractor can work underneath, removing roofing material and installing the new roofing. The LOMA, and any other surface bearing items, shall have paver blocks on a layer of reinforced membrane or walkpad applied over the completed roof membrane to protect the new roof, as discussed in 4.8 Isolated Paver Blocks. The two communication lines will stay in place and should not be disturbed.
1.1.6. Existing roof
The existing roof is approximately 1,700 square feet and consists of a concrete roof deck.
The Contractor shall verify dimensions and clearances in the field before beginning work. It does not include the roof of the scuttle roof, but does include flashing around the base of the scuttle structure. Roof elevation is approximately thirty feet above lot. See Reference drawings, “AR2 Roof Reference drawings” sheet numbers 3 for details of the existing sloped concrete substrate. The existing roof system is about 4” thick and consists of the following layers over the concrete substrate:
o Mopped asphalt o 2 ply built up roof o 0.5-inch wood fiber board o 3-inches of insulation
Safety Requirements All work shall be performed in accordance with Federal, State, and Local safety requirements, including those specified by the Occupational Safety and Health Administration (OSHA) and the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1. Where a conflict exists between safety standards/requirements, the Contractor shall comply with the more stringent requirement.
Any employee has the authority to halt any work if unsafe work practices or conditions are identified. The Resource/Project Manager or Lockmaster has the authority to halt all work should it be necessary to protect life or property. Work may not proceed until satisfactory corrective action has been taken. No part of any lost time due to such work stoppage shall be made subject to any claims for extension of time or excess costs or damages sustained by the Contractor, nor will the Contractor be paid for work not performed as a result of the stop order.
The latest version of the EM 385-1-1 is available online at:
https://www.publications.usace.army.mil/Portals/76/Publications/EngineerManuals/EM_385- 1-1.pdf?ver=2015-04-09-142531-467.
1.2.1. Accident Prevention Plan (APP)
Due to the nature of anticipated work, the Contractor may submit an abbreviated Accident Prevention Plan (APP). The Abbreviated APP shall be developed in accordance with EM 385-1-1 Appendix A, Paragraph K. In addition to the items specified as part of the Abbreviated APP, other programs and procedures pertinent to the Contract’s anticipated activities shall be submitted as appendices to the APP, to include, but not limited to, 8 | P a g e
“Hazardous Energy Control Program and Procedures” (12.A.01) and “Fall Protection and Prevention Plans” (21.D). The APP and applicable appendices shall be submitted to the Contacting Officer within 15 calendar days after receipt of the Notice to Proceed. The APP shall be reviewed and accepted by the Contracting Officer prior to any commencement of work.
The abbreviated APP Form, Form A-01, may be used to ensure each area of the required information has been provided.
1.2.2. Risk Management
Risk management is a business process that includes the identification, assessment, and prioritization of risks, followed by coordinated and economical application of resources to minimize, monitor, and control the probability and/or impact of unfortunate events to an acceptable level. A risk management assessment tool, such as activity hazard analysis (AHA), job safety analysis (JSA), or job hazard analysis (JHA) shall be developed by the contractor performing the work for each activity, task, or defined feature of work (DFOW).
The assessment shall identify the work sequence, specific anticipated hazards, site conditions, equipment, materials, personnel, and the control measures to be implemented to control the hazards. A Risk Assessment Code (RAC) shall be assigned to each job step based on the residual risk remaining after controls are implemented. A RAC shall then be assigned to the overall activity consistent with the highest RAC identified for the job steps. If developed by a subcontractor, the Prime Contractor shall review and approve the assessment before submission to the Contracting Officer. The Risk Assessment shall be signed by the SSHO, Superintendent, QC Manager, and any subcontracting Foreman's performing the work. If Qualified or Competent Persons are required for a particular activity, they shall be identified and included in the assessment, and their competency/qualification identified. Work shall not begin until the assessment has been accepted by the Contracting Officer and discussed with all engaged in the activity.
These risk assessment tools are considered living documents and shall be updated as needed. If the RAC increases from what was originally accepted by the Contracting Officer, the assessment shall be re-submitted for acceptance prior to work proceeding. Changes that do not increase the RAC are not required to be resubmitted for acceptance.
The risk assessment shall be developed in accordance with EM 385-1-1 Section 1, or as specified by the Contracting Officer. Risk assessments shall be submitted for review at least 15 working days prior to the start of the activity, task, or defined feature of work is set to begin. The Government reserves the right to require the Contractor to revise and resubmit any documents if they fail to effectively identify the work sequence, specific anticipated hazards, site conditions, equipment, materials, personnel, and the control measures to be implemented.
Each employee performing work as part of an activity, task, or defined feature of work shall review the risk assessment for the activity and sign the signature log designated for the assessment prior to beginning work. A signature log shall be maintained for each risk assessment.
Completed risk assessments need not be submitted at the time of the APP submission, but the Contractor shall identify each activity, task, or DFOW that will require a risk assessment to be completed at the time of submission.
1.2.3. Implementation of SOH Program
9 | P a g e
Based on the limited-scope of this Contract, in lieu of appointing a Site Safety and Health Officer (SSHO) at the project site, the Contractor may appoint a Designated Representative (DR) at the site to function as a collateral duty safety person, provided that an individual meeting the requirements of an SSHO as stated in EM 385-1-1 Section 1, Paragraph A.17 supervises the functioning of the DR. The DR shall perform safety programs and tasks as prescribed by the SSHO and report safety findings to the SSHO. SSHO or DR shall be on site at all times work is being done.
Note: Any activities that have a RAC of high or extremely high retracts the allowance to only have a DR, and shall have an SSHO on site.
1.2.4. Mishap Reporting
A mishap is any unplanned, undesired event that occurs during the course of work being performed, to include, accidents, incidents, and near misses. All mishaps shall be reported to the Contracting Officer within 24-hours after notification from the affected employee(s).

If material is encountered that was not indicated, but may be hazardous to human health upon disturbance, that portion of the work shall stop and the Contracting Officer immediately notified. The Government will determine within 14 days of notification if the material is or is not considered hazardous. If the material is not hazardous or poses no danger, the Government will direct the Contractor to proceed without change. If the material is determined to be hazardous or dangerous and handling of the material is necessary to accomplish the work, the Government will issue a modification pursuant to FAR 52.243-4, Changes and FAR 52.236-2, Differing Site Conditions.
1.2.6. Damage Reports
All instances where Government materials supplies, property, and/or equipment are damaged shall be reported immediately to the Contracting Officer and Lockmaster.
1.2.7. Minimum Personal Protective Equipment
Personal Protective Equipment (PPE) is governed in all areas by the nature of the work that personnel are performing and should be described in the applicable Activity Hazard Analysis (AHA). Mandatory PPE, at minimum, shall include: hard hat, long pants, safety toe shoes/boots, and short sleeve shirts.
