Pipe insulation elbows (8 inch) 12 ea.

The quantities provided above are estimates only and should be field verified by the abatement contractor.

INTRODUCTION

This Asbestos Survey was performed by Mr. David Morales on April 16-17, 2008. This Asbestos

Survey was conducted in accordance with Asbestos National Emissions Standards for Hazardous

Air Pollutants (NESHAP): 40 CFR Part 61, Subpart M. Sampling protocols were performed in accordance with the Texas Asbestos Health Protection Rules (TAHPR). Building 112 was to undergo renovations and was visually inspected and it was determined that the suspect asbestos-containing materials could be divided into twenty-one (21) homogeneous areas, including the following: cove base mastic (3 types), 12” x 12” floor tile/mastic (3 types), textured drywall/joint

El Paso Office: 7307 Remcon Circle, Suite 103 El Paso, Texas 79912 Fax 915-581-2049 Phone 915-833-3740 compound (2 types), textured plaster (3 types), 2’ x 4’ ceiling panels, pipe insulation (2 types), pipe elbows (2 types), window caulking, door caulking, stairway tread texture, vinyl stair tread mastic, and vinyl landing tread mastic.

DESCRIPTION

Building 112 was being used as office space on Fort Bliss, Texas. The building occupied approximately 6,831 SF. The building was a two-story structure constructed with CMU walls on a concrete foundation. The building also had a basement that was being used for storage. The interior was partitioned with drywall walls. Most of the flooring was covered with various colors of floor tile and carpeting. A red cementitious material (ACM) was found under all flooring materials on the first and second floors. At the time this Asbestos Survey was performed, the date of construction was not known.

SAMPLING METHODOLOGY

A visual inspection of the building was conducted to develop a sampling plan. This included determining sample collection points, homogeneous materials and areas. Friability was assessed by manually manipulating the suspect asbestos materials utilizing hand pressure to determine if the materials, when dry, can be crumbled, pulverized, or reduced to powder. Twenty-one (21) homogeneous areas were established and at least three (3) samples were collected from each area. Areas that have suspect asbestos-containing surfacing, thermal systems insulation, or miscellaneous materials are to be considered homogeneous. These samples were each uniquely and individually identified and logged into a chain of custody document and prepared for shipment to a third-party independent laboratory for analysis. Building materials that are not considered suspect include the following; concrete, metal, glass, and wood. Destructive sampling was not authorized or performed to locate hidden or inaccessible materials.

SAMPLE ANALYSIS

Sixty-three (63) samples were submitted for analysis. Bulk PLM analysis was conducted utilizing the EPA Method 600/R-93/116. This method separates the sample material into various layers based on their morphology, and then the presence of the different asbestos fibers in each

El Paso Office: 7307 Remcon Circle, Suite 103 El Paso, Texas 79912 Fax 915-581-2049 Phone 915-833-3740 of these layers is detected. The analysis was performed by Apex Precision Analytical Services, Inc. located in Friendswood, TX. The term asbestos-containing material (ACM) is defined as a material that contains greater than 1% of any asbestiform varieties of Chrysotile, Amosite, Crocidolite, Tremolite, Anthophyllite, and Actinolite, or combinations of these materials.

Asbestos materials were determined by the July 1993 Environmental Protection Agency as recommended in document EPA/600/R-93/116 titled, "Method for the Determination of

Asbestos in Bulk Building Materials." Copies of the asbestos laboratory results and related documentation have been attached to this Report.

CONCLUSION

Proper notification should be made to the appropriate authorities prior to any asbestos abatement activity, renovation or operations and maintenance (O&M) activity affecting asbestos-containing building materials (ACBM), or any demolition in facilities or public buildings. The Texas DSHS

(§295.34 (a) (2)) states that if disturbance of friable and non-friable asbestos-containing building materials (ACBM) is anticipated, these materials shall be removed prior to performing any demolition or renovation activities by licensed and trained individuals according to methodology specified by §295.58-§295.60. A proper notification shall be submitted to the DSHS no less than

10 working days prior to commencement of abatement activity.

SUMMARY

ENCON International, Inc, (ENCON) presents the findings of the Lead-Based Paint (LBP) Inspection performed at Building 112, Ft. Bliss, Texas 79916. The purpose of this LBP inspection was to determine if LBP was present in the areas scheduled for renovation, and if present, which building components contain LBP. This was determined by performing a surface-by-surface investigation of all painted components. This information is necessary in order for contractors to comply with OSHA regulation 29 CFR 1926.62 Safety and Health Regulations for Construction - Lead.

Lead-based Paint means paint or other surface coatings that contain lead equal to or in excess of 1.0 milligrams per square centimeter (mg/cm2) or more than 0.5%, or 5000 parts per million by weight as established by EPA and HUD regulations. However, for the purposes of this study, ENCON will utilize the “Action Level” of the XRF at 0.3 milligrams per square centimeter (mg/cm2), the NITON XRF detectable limit. Results indicate that seventy-two (72) of the one-hundred-thirty-four (134) XRF readings tested at or above the limit of 0.3 mg/cm2. The LBP Inspection was conducted by Mr.

Tim Jones, a certified Lead Risk Assessor on April 16 and 17, 2008. Results are indicated on a separate drawing and XRF data sheet.

D ESCRIPTION OF AREAS INSPECTED

Building 112 was being used as office space on Fort Bliss, Texas. The building occupied approximately 6,831 SF. The building was a two-story structure constructed with brick walls on a concrete foundation. The building also has a basement that was being used for storage. The interior was partitioned with drywall walls. At the time this Lead Inspection was performed, the date of construction was not known, but it appears that the building has been repainted over the years. The physical condition of the building components and their paints was fair at the time of the inspection. An inventory of the affected painted surfaces was recorded as XRF testing proceeded. See the “LBP Testing Data Sheet.”

CALIBRATION OF THE XRF INSTRUMENT

Before proceeding with the investigation of painted surfaces, the XRF was calibrated in accordance to the manufacturer’s quality control procedures.
