4. Authority Cited: This action will be awarded in accordance with 10 U.S.C. 2304(c) (1), as implemented by FAR 6.302-1: Only one responsible source and no other supplies or services will satisfy agency requirements.
5. Reason For Authority Cited:
a) Background: The VHA REdI program has identified a need for licenses that enable access to AHA BLS Part 1, BLS Part 2/3, ACLS Part 1, ACLS 2/3, LEARN: Rhythm Adult ECG, and Acute Stroke Online to enable their medical professionals to meet the mandatory training requirements to maintain AHA certifications to provide critical lifesaving procedures. The VHA also requires the licenses to provide access to AHA educational content online via the VHA’s Talent Management System (TMS) using existing Laerdal Voice Assisted Manikin (VAM) systems. The VHA REdI PM sought assistance, via an Interagency Agreement (IAA) from the Program Executive Office for Simulation, Training, and Instrumentation (PEO STRI) in acquiring the licenses needed to meet the VHA’s requirements. PdM MedSim has a recurring VHA requirement that will continue to exist as a future need to optimize patient safety and treatments. A plan is being developed for a competitive, long-term solution for future acquisitions of the licenses.
b) Justification: The VHA is bound to utilize AHA licenses per VHA Directive 1177 signed by the Under Secretary for Health, dated 06 APR 2017. The Directive 1177 establishes VHA Policy to optimize patient safety within the Department of Veterans Affairs (VA) system and specifically states that “Only the American Heart Association (AHA) and Military Training Network (MTN) are acceptable for BLS and ACLS certification required by this Directive.” for VA medical professionals that actively participate in medical care in any clinical setting including community/home care settings. The only contractor that provides AHA licenses for the courses needed to meet the VHA’s requirements for this procurement is HealthStream, Inc. HealthStream, Inc. offers licenses that provide access to AHA educational content online and via the VHA’s TMS using existing Laerdal VAM. HealthStream, Inc. offers medical professionals the ability to practice and test their skills on the VAM while receiving real time voice feedback on performance through their Learning Management System (LMS). The LMS evaluates and certifies the students CPR skills. The LMS interfaces with the VHA TMS to generate an online e-certification which is tracked by VHA.
c) Impact: VHA medical professionals’ certification expire every three years. Uncertified personnel may not perform procedures trained through AHA BLS Part 1, BLS Part 2/3, ACLS Part 1, ACLS 2/3, LEARN: Rhythm Adult ECG, and Acute Stroke Online on their Veterans’ patients. The lack of certifications will affect approximately 180,000 medical professionals which are practicing in over 1000 VHA medical facilities throughout the world. The impact will potentially affect the medical professionals’ ability to treat millions of veterans and lives will be at risk.
d) Alternatives: At this time, there are no alternative solutions to the VHA AHA certifications requirements as stated in Directive 1177.
6. Efforts to Obtain Competition: A synopsis for this procurement was posted to
FedBizOpps on 12 APR 2017, which closed on 27 APR 2017. Pacific Medical Training (PMT) responded to the synopsis by submitting their capabilities statement. PMT’s capabilities were identified as BLS Part 1, ACLS Part 1, and ECG Licenses. PMT’s licenses are not AHA certified licenses and do not meet the VHA’s requirements.
Additionally, PMT does not have the capability to provide all the requirements (BLS Part 1, BLS Part 2/3, ACLS Part 1, ACLS 2/3, LEARN: Rhythm Adult ECG, and Acute Stroke Online licenses). Based on the information provided in paragraph 5 above, competition for this effort is not feasible.
7. Actions to Increase Competition: This AHA HealthStream license award will provide the Government up to 5 years to satisfy the VHA licenses requirements. Once awarded, the Government will continuously survey the market to ascertain if any additional sources of supply have been approved for the sale and distribution of AHA approved licenses.
8. Market Research: Market research revealed the only current contractor that provides AHA approved licenses for the courses needed to meet the VHA licenses requirements for this procurement is HealthStream, Inc.
Even though several companies are able to provide some AHA licenses under this action, the VHA requires BLS Part 1, BLS Part 2/3, ACLS Part 1, ACLS 2/3, LEARN:
Rhythm Adult ECG, and Acute Stroke Online licenses to be able to integrate with the VHA TMS. HealthStream, Inc. is the only company able to meet the full requirements.
9. Interested Sources: PMT was the only company that responded to the synopsis.
However, PMT’s capabilities did not meet all the requirements for this solicitation.
Preliminary technical information submitted by PMT in response to the synopsis has been reviewed by the requiring activity, who have advised that PMT’s capabilities do not meet the requirement in the synopsis, i.e. BLS Part 1, BLS Part 2/3, ACLS Part 1, ACLS 2/3, LEARN: Rhythm Adult ECG, and Acute Stroke Online licenses.
