SUMMARY OF CHANGES
SECTION SF 30 BLOCK 14 CONTINUATION PAGE
The following items are applicable to this modification:
AMENDMENT 1
Q1) Reference Instructions to Offerors, Volume II - Technical, Factor 1: Can the Government please confirm whether the requirement for maintaining a Varian TrueBeam Linear Accelerator is actually a requirement for this solicitation or if it is a copy and paste error? The Varian TrueBeam Linear Accelerator was not mentioned in Section 5 of the PWS.
Answer: Instructions to Offerors, Volume II - Technical, Factor 1is changed FROM “Address in writing, your firm’s technical experience in providing maintenance and repair service for a Varian TrueBeam Linear Accelerator to other customers with the customer’s point of contact.” TO “Address in writing, your firm’s technical experience in providing Cancer Registry Service to other customers with the customer’s point of contact.”
Q2) Reference Instructions to Offerors, Volume II - Technical, are there specific items/headings within the technical that the Government wants to see?
Answer: The Government did not dictate the use of specific headings. Quoters shall consider using headings as outlined in the Instructions to Offerors Addendum, for example VOLUME II – TECHNICAL. Specific items are outlined in Instructions to Offerors, Volume II - Technical and shall include providing sufficient narrative and supporting data to demonstrate the Quoter has the requisite capability to perform the services identified in the Performance Work Statement; addressing in writing, your firm’s technical experience in providing Cancer Registry Service to other customers with the customer’s point of contact; and include documentation to demonstrate that personnel are trained, certified, and have experience to perform the services.
Q3) Reference Instructions to Offerors, Volume II - Technical - are there page limits to the Technical Volume? Are there font type and font size minimum requirements?
Answer: The Government did not indicate a font type and font size for the quote. Quoters shall consider using Times New Roman font size 10.
Q4) Reference Instructions to Offerors, Volume III - Past Performance - Are there limits to how many past performance citations can be submitted?
Normally, ITOs call for three.
Answer: The Government did not mandate quoters to submit past performance citations nor did the Government establish a limit on the number of past performance citations the quoter is allowed to submit. Should the Quoter choose to submit past performance citations, Quoters shall consider limiting the number to three and shall consider submitting only recent and relevant past performance citations for the same or similar service. Recent is defined for this action to be past performance citations that are within the last three (3) calendar years.
