Description of Supplies/Services: Madigan Army Medical Center Tacoma WA is requesting a Sole-source acquisition of Preventive Maintenance & Repair Services for the PINPOINT Endoscopic Fluorescence Imaging System which will be used to support different types of surgical procedures at the hospital. The system is used to visually assess tissue perfusion and identify anatomical structures. The clinical information is obtained through the real-time visualization of tissue perfusion that this modality gives, has assisted surgeons in making informed surgical decisions and has positively impacted patient outcomes. Without the abilities of this equipment, the risk of anastomotic leaks and complications are higher. This system has been utilized in the medical community in various colectomy procedures, laparoscopic cholecystectomies, and high risk gastric bypass surgical cases.

|_| (b) FAR 13.106-1(b)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item; OR
|_| (c) FAR 8.405-6(a)(1)(i)(A): An urgent and compelling need exists, and following the procedures would result in unacceptable delays; OR
|_| (d) FAR 8.405-6(a)(1)(i)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; OR
|_| (e) FAR 8.405-6(a)(1)(i)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order not previously issued under sole-source or limited-sources procedures.
5. Reason for Authority Cited: The Novadaq Corp PINPOINT® Endoscopic System service is inclusive and proprietary, as they are the OEM and at present the only vendor to provide these services with no outside distribution of parts, repairs, or services by other sources. The equipment is Government-Owned located throughout DOAS and the Surgical Suites. The continued daily use of the equipment in support of patient care is essential and a consistent intangible benefit to the Government in continuing the mission of the US Army Medical Command.
6. Efforts to Obtain Competition: A general competitive effort is made by the surgeon and attending team throughout the requirement review process which considers the applicability of products & services from various sources to determine the product that best meets the needs of the patients and the team. In addition, the intended contractor is the only source capable of meeting the requirements as prescribed.
