Covered Telecommunications Equipment or Services-Representation (Dec 2019)

(b) Procedures. The Offeror shall review the list of excluded parties in the System for Award Management (SAM) (https://www.sam.gov) for entities excluded from receiving federal awards for “covered telecommunications equipment or services”.
(c) Representation. The Offeror represents that it □ does, □ does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument.
QUESTIONS AND ANSWERS
Questions and Answers:
1. Is there a mix of GOV Owned Cylinders & Contractor Owned utilized in this requirement? There are several gases in Exhibit A with “C/O” which typically stands for “Customer Owned.” If so, please confirm that all GOV Owned cylinders are labeled as such (decal/stencil). Answer: Yes, there is a mix of vendor / GOV owned cylinders. The cylinders are identified by a plastic ring placed on the neck of the cylinders to show vendor owned. As those are used, they are returned to the vendor with no replacement sent back to us. We do not stencil due to no guarantee from vendor returning same exact cylinders back.
2. What gas type(s) are utilized at the (5) Clinics? Please specify between GOV Owned and Contractor Owned where applicable.
Answer: All cylinders at clinics are GOV owned. Most gases at these clinics involve oxygen and nitrogen.
Some clinics have liquid nitrogen if they are a health clinic. Dental and VETCOM do not.
3. How are vendors to account for varying rental costs of medical gas cylinders? Recommend adding cylinder monthly rental groupings by size, type, and functionality: Answer: Vendors charge as we request.
We could hold a cylinder vendor owned for more than a month but only be charged that first time we request it.
4. Is the 20‐Liter Liquid Nitrogen Dewar VA Owned? Answer: The cylinders are all CRDAMC owned.
5. What do the asterisks represent in Exhibit A? Answer: Disregard asterisks.
6. Has the AMC historically required (15) annual Emergency Deliveries (as stated in Exhibit A)? Answer: No;
at the most….maybe one to two per year; however, with the occurrence of COVID‐19; there was a slight increase.
7. What types of service(s) are to be expected under “Service Charge Labor, Cryogenic” in Exhibit A? Answer:
This is the inspection done by the driver of the cryogenic canister and its parts to ensure no leaks and test for serviceability of the cylinder.
8. If the AMC has deliveries weekly and the (5) Clinics are bi‐weekly, won’t the actual quantity of annual deliveries greatly exceed the quantity of (52) stated in Exhibit A? Answer: Yes, this needs to be raised by at least 26. 52 weeks of deliveries for the hospital prime, and 26 scheduled for clinics. Total delivery will be updated to 78 deliveries.
9. What is the rationale requiring Certificates of Analysis for each medical gas cylinder? This is not an industry practice for cylinders but for Bulk Gases. There are stickers/decals physically on the cylinder denoting Supplier, Product, Air Liquefaction, Lot Number, etc. however there is no separate certificate supplied for every cylinder delivered. Answer: Not all gases require this. Only oxygen. Purity level needs to be confirmed. We are not subject matter experts to identify the purity level is what the vendor states.
This would not be per cylinder as many are same lot number. It would be required per lot number.
10. Does the GOV require (400) Dewar fills or (16) 25‐Liter Dewar Fills (totaling [400] Liters annually)? Answer:
400 fills. Cylinders are filled weekly, sometimes 8, sometimes 16 depending on usage and amounts brought back by the customer.
11. Page 27 of 68 52.212‐1 Addendum paragraph f and Page 28 of 68 paragraph 2 states “Section C of Exhibit A FAR Clause 52.204‐26 must be returned unless the section is completed online in the Reps and Certs to be eligible for award.” Where is the Section C of Exhibit A provided for completion? Answer – Exhibit B will be added , FAR Clause 52.204‐26, will be added to the solicitation.
12. Page 56 of 68 Para 5 states that empty cylinders are picked up on Wednesday (hospital) and Friday (external clinics) and returned the following Wednesday (hospital) and Friday (external clinics). Is this referring to GOV Owned Cylinders? Answer: Yes. Cylinder refill is done offsite. One week turnaround is what is expected when they remove from CRDAMC property.
