SUMMARY OF CHANGES
SECTION SF 1449 - CONTINUATION SHEET
The following have been modified:
ADDENDUM 52.212-2
Award will be made using the Lowest Price Technically Acceptable (LPTA) source selection process. Award will be made to the responsible offeror on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. Award maybe made without discussions with offerors (except communications conducted for the purpose of minor clarification). Therefore, each initial offer should contain the offeror's best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if it is later determined by the contracting officer to be necessary.
(a) The Government will award a Purchase Order resulting from this solicitation, to the responsible Request For Quote (RFQ) conforming to the solicitation, that is lowest price technically acceptable (LPTA). Award will be on an all or non basis.
Explain how your company will meet the salient characteristics of the items as specified in the solicitation.
prospective vendors shall demonstrate that the items offered comply with the technical requirements described in the salient characteristics by submission of a written capability statement with their offer.
The following evaluation factors shall be used to evaluate offers: Price and Technical Acceptability.
1. Technical: Quote shall be submitted for the entirty (not partial) of the requested radiopharmaceuticals reflected in this solicitation.
a) The offeror’s technical proposal and all submitted literature must be complete. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation for substantiating that the items offered meet the requirements of the solicitation. The proposal shall not simply rephrase or restate the Government’s requirements, but rather shall provide convincing rationale to address that the products offered clearly meet the Government’s requirements. Offerors shall assume that the Government has no prior knowledge, and will base its evaluation solely on the information presented in the offeror’s proposal.
(b) Offeror must be able to provide the aggregate of all required radio pharmaceuticals. Meeting the specifications of the item purchase description, the contractor shall need to demonstrate that the radiopharmaceuticals they are offering meets the radiopharmaceuticals in the purchase description.

Darnall Army Medical Center, Fort Hood, Texas. Offers not able to meet this delivery time shall be considered not acceptable for the delivery evaluation. Offerors may offer a delivery time lower than the acceptable delivery. If an award is made to an offeror and the offeror fails to meet the delivery time, this may result in a termination for default.
(e) Received dose activity must be within 5% of the calibrated dose activity. Pharmacy must be in compliance with USP 797 guidlines and regulations.
The following ratings will be used in evaluation offerors proposal:
Acceptable –The offeror shall provide sufficient details for the Government to determine that the items offered satisfactorily meet the salient characteristics of the solicitation. The proposal shall clearly demonstrate that the offeror fully understands and is capable of performing the requirement for the total proposed price indicated in its proposal.
Unacceptable – An unacceptable rating will be assessed on any offeror that presents a proposal that demonstrates any of the following:
(1) Failure to meet the radiopharmacautical item description set forth herein.
