10.8 MCDS. The Maintenance Consolidated Database System (MCDS) on the TAMMS-A/SNRR Web Portal (https://tammsa.redstone.army.mil) can be used to generate the following DA Forms: 2410, 2408-5-1, 2408-16 and 2408-16-1. This use of MCDS or other approved electronic method eliminates the requirement for the Contractor to mail or fax DA Form 2410s to AMCOM. Use of MCDS or other approved electronic method does not eliminate the requirement to include the required forms with the component when shipped.

10.9 Forms and Tags/Label Instructions. Detailed preparation and disposition instructions for the following forms and tags/labels are outlined in DA Pamphlet 738-751:

DA Form 2408-16, Aircraft Component Historical Record. This form is a permanent record of historical data for selected aircraft and engine Time Change (TC), Retirement Change (RC), and Condition Change (CC) item(s) that are removed and replaced at specified aircraft operating hours, specified component operating hours, specified component starts. IAW TB 1-1500-341- 01 shows if an item requires its own DA Form 2408-16 or if the item will be listed on the next higher assembly 2408-16. If required, ship with the item(s).

DA Form 2408-16-1, History Recorder, Component, Module Record.

This form is a permanent record of historical data for selected TC, RC, and CC item(s) for turbine engines equipped with a history recorder and Auxiliary Power Units (APU). IAW TB 1- 1500-341-01 shows if an item requires its own DA Form 2408-16-1 or if the item will be listed on the next higher assembly 2408- 16-1. If required ship, with the item(s).
