The e-mailed Letter of Transmittal shall include:

-Contract Identification -Date of Delivery -Deliverable Identification and Description -Uploaded File Name(s) included or -Content management system path or URL where files can be uploaded files

COST REPORT DATA: The following data is required with each Monthly Cost Report. It shall be provided in the format using DL and Indirect Cost Reporting format in task order attachment 005.

Prime/ Subcontractor/Other Direct Costs: Select from the drop down indicating which category this line indicates.

Location - Onsite or Offsite: Select location from the drop down.

Regular Labor Hours Expended: Hours billed at or under 80 hours per biweekly period.

Overtime Labor Hours Expended: Hours billed above 80 hours per biweekly period.

SSES NexGen LCAT/Skill Level: Select from the drop down the labor category of the employee.

Employee Name or Employee ID # required for each separate employee.

Minimum, Maximum and Weighted Proposed Direct Labor (DL) Rates within each LCAT and skill level: Awardee is required to include the Minimum, Maximum and Weighted Average DL rate(s) for each DL rate proposed for the corresponding Contractor Fiscal Year. For instance, if the current billing cycle is occurring during year 4 of the subject contract, the year 4 DL rates (Minimum, Maximum and Weighted Average) that were originally proposed shall be included in columns S, T, and U, respectively..

The Weighted Average DL rates are to be determined by utilizing the corresponding Labor Hours that relate to the specific labor rates within that LCAT/Skill Level. If there are 10 different rates proposed for say, 10 “Budget Analyst Entry”, the Weighted Average must be computed by utilizing the corresponding labor hours reported for each of the 10 different labor rates and reported in column U.

Maximum Proposed Direct Labor Rate within LCAT: See above description.

Weighted Average (Hours) of Direct Labor Rate within LCAT:

See above description.

D-8

Employee Name or Employee ID# (column R) and DL Rate (column V): Contractor/Awardee is required to provide either the name or employee ID # (or both) in order to identify the employee. Additionally, Contractor/Awardee is required to provide a SEPARATE Payroll submission that includes Employee Name/or Employee ID #, LCAT/Skill Level, # of Hours and the Extended Labor Dollars. The payroll per LCAT/Skill Level and Total needs to reconcile with the Total Direct Labor Dollars billed on the spreadsheet.

Contractor/Awardee has the option of:

1) Make multiple entries for each, say, "Budget Analyst Entry" and utilize column P "Employee Name/Employee ID#" or;

2) Sum all Budget Analyst Entry labor dollars and divide by the number of billable hours to arrive at the average labor rate billed for that LCAT/Skill Level (column V). If the Contractor/Awardee chooses this methodology for the spreadsheet, the detail, for each employee will be required on the separate payroll submission with each LCAT/Skill level to include the hourly billable rate for each employee, the # of billable hours as well as the extended billable DL dollars and billable hours for each employee and subtotal(s) for that LCAT/Skill level (DL dollars and hours).

Either options will require a SEPARATE Payroll submission as described above.

Note: Contractor/Awardee is offered maximum flexibility to properly compute and report each respective indirect and fee dollars and respective rates reported. Either use existing formulas provided or reformulate to meet the Contractor/Awardee’s accounting practices/disclosures. If hard dollars are reported, the adjacent columns will automatically compute the “actual” or effective rates.

Fringe Dollars: Formula, Fringe rate * direct dollars (see instructions above).

Overhead {O/H rate*(DL + Fringe)}: The contractor/Awardee is encouraged to modify this formula/column as necessary to remain in compliance with your financial disclosure and accounting practices.

Other Indirect 1

Other Indirect 2

Other Indirect 3

G&A: Formula (see instructions above)

Fee: (see instructions above)

D-9

FCCOM: Contractor/Awardee is reminded that Facilities Capital Cost Of Money (FCCOM) cannot be billed if FCCOM was not proposed in the Base Contract/IDIQ.

Total ODC, Material, Travel (column AS): Summation of the total ODCs, Material, and Travel costs billed. These ODC choices are to be selected from the drop down menu included in column K, “Prime/Subcontractor/Other Direct Costs.” Make a selection for appropriate category on its own separate row, i.e., Travel, row 19, Material, row 20, etc.

Total $ Billed: Summation of the total costs billed.
