About this file

This is a Past Performance Questionnaire template used to evaluate contractor performance on federal contracts. The form captures comprehensive assessments across seven key performance categories: Quality (conformance to specifications and workmanship standards), Schedule (timeliness against milestones and delivery dates), Cost Control (effectiveness in forecasting and managing costs; marked "Not Rated" for Firm-Fixed Price contracts), Management (integration and coordination of activities, problem identification, corrective actions, and customer satisfaction), Utilization of Small Business (maximum practicable opportunities for small businesses including ANCs, Indian Tribes, SDBs, WOSBs, HUBZone, Veteran-Owned, and Service-Disabled Veteran-Owned Small Businesses per FAR 52.219-8 and 52.219-9), Regulatory Compliance (adherence to applicable regulations and codes), and Overall Performance.

The questionnaire requires respondents to rate contractor performance using a six-point scale: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Rated. All positive and negative ratings must be supported by factual data with specific dates and supporting documentation made available upon request. The form includes nine evaluation questions addressing whether the contractor was prime or subcontractor, performance ratings with rationales for each category, overall performance assessment, and future recommendation. Respondents must provide detailed contact information including name, organization, title, email, phone, date, and signature. The template emphasizes that information must be accurate and complete to preclude follow-up evaluations, allowing consultation with knowledgeable personnel within the organization if needed.

Period of Performance Being Assessed:

Evaluation Indicate the contractor’s past performance for each category area by identifying with an “x” for the most appropriate rating according to the scale shown below. Provide rationale, including specific examples, as appropriate, to support your ratings in the space provided for comments for each category area. All positive and negative ratings must be supported by factual data with the dates positive or negative performance occurred. Also, supporting documentation must be made available upon request. If more space is needed, continue your comments in the “Additional Comments” section at the end of this questionnaire. It is important that the information be as factual, accurate, and complete as possible to preclude the need for follow-up by the evaluators. If there is an area that you do not have detailed knowledge then you may consult with someone in your organization that does. Use Table 1 and Table 2, along with the guidance below to answer the questions that follow.

Consider the criteria for each category area in Table 1 in the assessment.
