CONTRACT INVOICE PAYMENTS

Invoice payments will be paid via the Government Purchase Card Method of Payment.

All payments will be made at the end of each service month, once the service is completed.

No payment will be made for months in which no service call was conducted.

Invoices must be accompanied by a MCAAP POC signed service ticket acknowledging the monthly service call.

TECHNICAL SPECIFICATIONS

Technical Specifications

For Elevator Inspections

General

1. Description

1.1 It is the declared and acknowledged intention and meaning to provide and secure inspection of the elevators listed in Appendix A. The work shall be located at the Army Ammunition Plant, McAlester, OK (MCAAP).

2. References

2.1 The publications listed below form a part of this specification to the extent referenced. The publications are referred to in the text by basic designation only.

American Society of Mechanical Engineers (ASME) ASME A17.1 (2002) Handbook on Safety Code for Elevators and

Escalators ASME A17.2.2 (2002) Checklist for Inspection of Hydraulic Elevators

3. General Requirements

3.1 The work includes providing labor, transportation, equipment, materials, supplies, and supervision as required for monthly, quarterly, semi‐annual, and annual inspections, testing, adjustment, and reporting of conditions of passenger elevators shown in Appendix A. Testing and adjustments shall be in accordance with A17.2.2, and as modified herein. In addition the contractor shall perform minor repairs and adjustments to the equipment as necessary for safe and proper operation.

3.2 Contractor will provide personnel dispatched for emergency service calls.

Emergency service calls will be initiated by the MCAAP POC with a requested response time of 4 hours, or less, from the initial call to the associated Service Call Center.

4. Inspections

4.1 Monthly Inspections

4.1.1 Monthly inspections shall include the following:

4.1.1.1 Check all safety devices to assure proper operation, and replace any defective safety devices.

4.1.1.2 Observe car floor and landing sill locations and door operation at each floor, adjust as required for proper operation.

4.1.1.3 Observe hydraulic power unit for proper operation and oil leaks, repair any oil leaks, check oil level and add oil as required.

4.1.1.4 Observe limit switches, and door operators for proper operation and adjust as required.

4.1.1.5 Inspect jackhead and piping for leaks.

4.1.1.6 Check operation of firefighter recall services.

4.1.1.7 Tighten all loose bolts, nuts, and screws.

4.2 Quarterly Inspections

4.2.1 Quarterly inspections shall include the requirements for the monthly inspection and the following:

4.2.1.1 Clean and lubricate guides, doors, and other points as recommended by the elevator manufacturer.

4.2.1.2 Clean pit floor, car top, and all accessible equipment.

4.2.1.3 Observe car speed in both directions.

4.2.1.4 Control box, clean, grease, and check cams for alignment, wear, and verify cams are tight on shaft.

4.2.1.5 Check conditions of relating cables, hanger rollers, and door gibs.

4.3 Semi‐Annual Inspections

4.3.1 Semi‐Annual Inspections shall include the requirements for both the monthly and quarterly inspections as well as the following:

4.3.1.1 Clean safety edges, examine for loose or worn parts, and check electrical and metal cables for wear and adjustment.

4.4 Annual Inspections

4.4.1 Annual inspections shall take place in June, and shall include the requirements for monthly, quarterly, and semi‐annual inspections as well as the following:

4.4.1.1 Check roller guides for alignment, loose hardware, fatigued rubber bushings, noisy bearings, and worn rollers.

4.4.1.2 Remove car station cover and clean, tighten all wire connections, and check operation of buttons and switches.

4.4.1.3 Check hatch doors, clean accessible equipment, examine and adjust the relating cables, upthrust rollers, and check hanger rollers for wear, tighten any loose hardware. Clean, examine and adjust interlocks, examine contacts and tighten loose connections.

4.4.1.4 Check door gibs for wear and tightness.

4.4.1.5 Depress limit switches to verify proper operation and clean.

4.4.1.6 Remove fixture cover and clean corridor pushbuttons, tighten wires, examine for loose or worn parts.

4.4.1.7 Test relief bypass valve, verify that bypass will open at 125% of working pressure.

4.4.1.8 Examine traveling cables for evidence of wear or abrasion. Relocate cables to prevent rubbing against structural members.

4.4.1.9 Clean reflectors, filters, and lens, and check for alignment and focus of light.

4.4.1.10 Contractor shall provide the services of an elevator inspector to inspect elevator, witness testing, and provide elevator Inspection Certificate signed by the inspector for each elevator. Testing shall be in accordance with the requirements of ASME A17.1, and ASME A17.2.2. Inspector can be Contractor employee, or from independent testing company.

4.5 Rated‐Load Tests

4.5.1 Each elevator shall be rated‐load tested at the interval required by ASME A17.1 for passenger elevators. Appendix A indicated the last date the passenger elevators were rated‐load tested.

4.6 Minor repairs and maintenance work shall not exceed one hour on each elevator at each inspection. Work requiring more time or replacement of parts shall be reported as specified hereinafter.

5. Notification of Inspection

5.1 The contractor shall notify the contracting officer (CO), a minimum of 7 days in advance of inspections. The Government will reserve the right to delay inspections as additional 7 days at no additional costs. All work will be done between 0800 hours and 1630 hours: Monday through Thursday, excluding Federal holidays.

5.2 The contractor shall notify the MCAAP POC upon arrival and the POC will sign the service ticket prior to contractor departure.

6. Qualifications of Elevator Inspector and Mechanic

6.1 Qualifications of elevator inspector: The elevator inspector shall be certified in accordance with requirements of ASME A 17.1, and have all required state licenses. Statement of qualifications for each inspector shall be submitted for approval prior to start of inspection.

6.2 Qualification of elevator mechanic: All elevator mechanics assigned by the contractor to the performance of the work under this contract shall be of journeyman status as recognized by the industry with at least four years of experience and training in the inspection, repair, and maintenance of elevators.

All inspections shall be performed by journeymen elevator mechanics as defined above who may be assisted by qualified helpers in conformity with local trade practices. Certification of elevator mechanics performing inspection shall be submitted to the contracting officer in writing for approval. The contracting officer may, in writing, require the contractor to remove from the work any employee the contracting officer deems incompetent, careless, or otherwise objectionable.

7. Security Requirements

7.1 No contractor employee will be admitted to the site work unless contractor furnished satisfactory proof that employee is a citizen of the United States, or if an alien, residence within the United States is legal. A contractor employee that is a United States citizen shall escort alien employees at all times while at

MCAAP.

7.2 ALL contractor service personnel will be required to undergo an NCIS background check prior to entering the facility.

8. Signs

8.1 Provide barricades or warning signs for passenger elevators out of service. Signs shall also indicate the direction of exit.

9. Reports

9.1 Two copies of inspection, service, and testing of successful elevator operation shall be provided for each unit. The report, submitted within 10 days of the inspection, shall include date of service, list of items receiving service, minor repairs, and adjustments. For each elevator requiring more than minor repairs, provide a complete deficiency list of required major repairs and adjustments needed. A detailed cost estimate, including estimated time to complete repairs, shall be included with the deficiency list.

9.2 One original signed elevator Inspection Certificate, for each elevator. The Inspection Certificate shall be submitted within 10 days of inspection. For each elevator that cannot be certified, a complete list of deficiencies, required repairs and adjustments shall be provided. A detailed cost estimate, including estimated time to complete repairs, shall be included with the deficiency list.

9.3 Two copies of the rated‐load test results shall be submitted within 10 days of test. Test results shall contain time and date of test, location of elevator, amount of weight used, and names of all personnel who witnessed test. For each elevator that failed the rated‐load test, provide a complete list of required repairs, and adjustments, with a detailed cost estimate, including estimated time to complete repairs.

10. Contract Length with Options

10.1 Basic contract will be to provide material, labor, transportation, and supervision for inspection, minor repairs, and certification for a one‐year period starting after the contract is signed by the government. At the option of the government the contract may be extended in one‐year increments for up to four additional years.

At the McAlester Army Ammunition Plant truck shipments are accepted at Warehouse 8, Receiving Office, Monday though Thursday (except holidays) from 6:30 A.M. until 3:00 P.M. only. All shipments and deliveries must be at the main gate (truck gate) on or before 3:00 P.M. No shipments will be accepted after 3:00 P.M. unless prior arrangements have been made.

52.0023-4000 CONFORMANCE WITH ENVIRONMENTAL MANAGEMENT SYSTEM-FOR WORK

PERFORMED AT MCALESTER ARMY AMMUNITION PLANT

The Contractor shall perform work under this contract consistent with all applicable federal, state, local requirements and with the relevant policy and objectives identified in McAlester Army Ammunition Plant’s (MCAAP’s) environmental management system (EMS) located to http:www.mcaap.army.mil. The Contractor shall perform work in a manner that conforms to all applicable Environmental Management Programs and Operational Controls identified by the MCAAP EMS, and provide monitoring and measurement information as necessary for the organization to address environmental performance relative to MCAAP’s EMS objectives and goals. In the event an environmental nonconformance or noncompliance associated with the contracted services is identified, the Contractor shall take corrective and/or preventative actions. In the case of a noncompliance, the Contractor shall respond and take corrective action immediately. In violations caused by Contractor negligence, the Contractor shall reimburse the Government for the amount of that fine and all associated costs. The Contractor shall also provide cleanup of any spills, including oil, that result from the Contractor’s operations and report spills to MCAAP’s Environmental Management Office. In the case of a nonconformance, the Contractor shall respond and take corrective action based on the time schedule established by the Environmental Management Office and approved by the Contracting Officer. In addition, the Contractor shall ensure that their employees are aware of the roles and responsibilities identified by the EMS and how these requirements affect their work performed under this contract.

Prior to commencing work, all on-site Contractors shall provide documentation to the Contracting Officer that any required EPA sponsored environmental training specified for the type of work conducted has been obtained. Upon contract award, and during performance of the contract, the Contracting Officer’s Representative will provide or arrange EMS training for appropriate Contractor staff.

52.0037-4100 SUPERVISION OF EMPLOYEES (FEB 2004)

The Contractor’s employees shall remain under the Contractor’s direct supervision at all times. Although the Government will issue tasks and coordinate directions within the scope of the contract, detailed instructions for Contractor employees and supervision of individual workers shall remain the responsibility of the Contractor.

52.0037-4101 REMOVAL OF CONTRACTOR PERSONNEL

The Government reserves the right to require removal of any Contractor personnel from the job site who endanger persons or property, or whose continued employment is inconsistent with the interest of military security. The Government also reserves the right to require the Contractor to remove from the job site, any employee found to be under the influence of alcohol, drugs or any other incapacitating agent or any employee involved in the theft of Government property. The removal from the job site of such personnel shall not relieve the Contractor of the requirement to provide personnel to perform services.

52.0045-4001 PROHIBITED WITHIN GENERAL CONFINES OF INSTALLATION (FEB 2004)

The following are prohibited within the general confines of the McAlester Army Ammunition Plant at any time:

(a) Firearms/dangerous weapons

(b) Drugs/intoxicants

(c) Binoculars, cameras, camera equipment, cell phones and PDAs with camera capability (unless authorized by the Security Officer)

(d) Matches, lighters or other fire flame or spark producing devices in designated areas

(e) Gas cans/flammable material storage containers (unless approved by MCAAP Fire Department)

(f) Fireworks without the Commander's approval

(g) Smoking in all buildings, when handling flammable and hazardous materials, in railroad cars, trucks and vans, on warehouse docks and platforms, in open storage containing combustibles, within 50 feet of a gasoline refueling storage or drainage area, where posted signs prohibit smoking.

52.0045-4002 SECURITY REQUIREMENTS - BADGING

(a) Any foreign nationals without a green card will not be allowed entrance to the installation. Those individuals with a green card must be escorted within the restricted area at all times

(b) Contractors by virtue of contract award will be given access privileges to McAlester AAP. Contractors/Sub- Contractors are required to provide security at McAlester AAP a letter of introduction for company personnel by providing the following: 1) Name; 2) Physical Address (including city and state); 3) Date of Birth; 4) Place of Birth; and 5) Whether or not they are a U.S. Citizen on company letterhead. Photo I.D. will also be required to obtain a security badge. Information shall be provided to McAlester AAP, Security Division, Attn: Ms. Traci Howard, McAlester, OK 74501 or faxed to 918-420-7379. (Applies to contracts that require access to restricted areas)

(c) Each member of the Contractor's work force shall be individually badged at the Security Office in Building 31.

The badge shall be retained and worn by the Contractor's employees throughout the contract period. (Applies to contracts that require access to restricted areas)

(d) Contractor is responsible for the turn-in of all security badges, including sub-contractors badges, daily and upon completion of the contract or termination of individual employees. (Applies to contracts that require access to restricted areas)

(e) Report lost badges to the Security Office and the Contracting Officer as soon as the loss is discovered. (Applies to contracts that require access to restricted areas)

(f) Be prepared to produce security badges each time entering a security area. Temporary badges for any individual who may have forgotten his/her badge can be obtained from the Security Office. (Applies to contracts that require access in restricted areas).

52.0045-4003 SECURITY REQUIREMENTS - VEHICLES

(1) TRAFFIC ENFORCEMENT: (a) McAlester AAP traffic laws are enforced by the Law Enforcement and Security Division using Oklahoma state codes. All roads/streets have speed limit signs posted; (b) Traffic violations are adjudicated by the U.S. Magistrate located at McAlester AAP. Court appearances may be requested or mandatory; and (c) Radar is used to enforce traffic speed limits.

(2) INSURANCE: All vehicles entering the installation must possess the minimum insurance as required by the State of Oklahoma.

(3) VEHICLES: (a) Vehicles entering McAlester AAP, whether Contractor-owned or privately owned, shall undergo inspections and may be issued a pass if entering a restricted area. Vehicle permits for restricted areas will be issued for up to 45 days maximum. Each vehicle will be required to carry a fire extinguisher at all times; (b) Company or privately-owned vehicles, when parked, must have ignition keys removed. After duty hours, vehicles left on the installation will be parked outside restricted areas unless otherwise stipulated in the contract. Vehicles or equipment remaining inside restricted areas will be properly secured or immobilized; and (c) Contractor is solely responsible for the removal of incapacitated vehicles or equipment from restricted areas. (d) All vehicles shall be parked at least 20 meters from all buildings unless unloading or loading; and (e) The contractor can expect to be inspected at one or all of the two manned gates. The contractor should expect five to ten minute delays at each gate.

In addition, to get to some of the remote areas, the contractor should expect at least a 15-minute delay both in and out to allow security to open necessary gates.

(4) RESTRICTED AREAS: Any contractor personnel granted access into restricted areas will be subject to vehicle search upon entering or leaving these areas possibility resulting in some delay. Any non-US contractor personnel granted access into restricted areas w ill at all times be escorted by US contractor personnel who have been properly badged and granted access. (Applies to contracts that require work in restricted areas)

52.0047-4001 HOURS OF OPERATION – MCAAP (FEB 2004)

Normal work hours are Monday through Thursday 6:30 A.M. until 5 P.M. Contractors will not be permitted to work on Fridays, Saturdays, Sundays, or legal holidays for any reason, unless otherwise approved by the Contracting Officer. The following legal holidays will be observed:

January 1st 3rd Monday of January 3rd Monday of February Last Monday of May July 4th 1st Monday of September 2nd Monday of October November 11th 4th Thursday of November December 25th

When one of the designated holidays falls on a Friday or Saturday, holiday will be observed on Thursday. If the holiday falls on a Sunday, holiday will be observed on the following Monday.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

Technical and past performance, when combined, are N/A.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph

(d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1)in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—
