SOLICITATION AMENDMENT 0002

The purpose of Amendment 0002 to Solicitation W15QKN-22-Q-5074 is as follows:

1. Extend the Solicitation closing date from 08-July-2022 2PM EST to 11-July-2022 2PM EST.

2. Provide responses to industry questions.

3. Revise Attachment 0001 - Performance Work Statement (PWS). Attachment 0001 – Performance Work Statement, Version 7, dated 7 July 2022, replaces all previous versions.

All other terms and conditions remain unchanged.

The following have been modified:

INSTRUCTIONS TO OFFERORS

THIS REQUIREMENT IS SUBJECT TO THE AVAILABILITY OF FUNDS

1. Solicitation W15QKN-22-Q-5074, Grounds Maintenance Services – Brookeville, PA (PA011), Clearfield, PA (PA017), Dubois, PA (PA020), Punxsutawney, PA (PA080) and Saint Mary’s, PA (PA087) is solicited in accordance with Federal Acquisition Regulation (FAR) Part 12, Acquisition for Commercial Items and FAR Part 13 Simplified Acquisition Procedures.

2. The purpose of this solicitation is to procure Grounds Maintenance services in support of the 99th Readiness Division. Grounds Maintenance services, inclusive of providing healthy grass, trees, shrubs, and plants and present a clean, neat, and professional appearance, that represents the facilities in their best appearance to enhance public relation and attractiveness, are required at the following facilities: Brookville Memorial United States Army Reserve Center (USARC) 20 Spruce Street Brookville, PA 1582 (PA011), PCF Melvin L. Brown USARC, 6400 Clearfield Woodland Hwy Clearfield, PA 16830 (PA017), Blommwn-Russel USARC, 400 Hillcrest Ave Dubois, PA 15801 (PA020), Major Charles D. Stoops USARC, 215 Center Street Punxsutawney, PA 15767 (PA080) and Elks County Memorial USARC, 500 North Saint Mary’s Road Saint Mary’s, PA 15857 (PA087).

3. The Government intends to award a single, firm-fixed-price (FFP) purchase order. The period of performance for this effort shall be a 12-month base period and four, 12-month option periods from the date of contract award.

4. The Offeror must be registered in the System for Award Management (SAM) in order to be eligible to receive a Department of Defense contract award. If you need to register in SAM, go to https://sam.gov/

5. See Federal Acquisition Regulation (FAR) Clause 52.212-1, "Instructions to Offerors -- Commercial Items" and FAR 52.212-1, Addendum for additional information.

6. See FAR Clause 52.212-2, Evaluation -- Commercial Items" and FAR 52.212-2, Addendum for additional information and evaluation criteria.

7. A site visit is HIGHLY ENCOURAGED. In no event shall failure to inspect the site constitute grounds for a claim after contract award. All notifications to attend shall be submitted via e-mail at least 24 hours prior to the scheduled Site Visit date/time to the following: Timothy McCleary, Contract Specialist at timothy.j.mccleary.civ@army.mil, Katie Lyons, Contracting Officer, at Katie.j.lyons.civ@army.mil, Randy Baker, Regional Facility Operation Specialist at randy.a.baker.civ@army.mil and Timothy Peters, Area Facility Operations Manager at timothy.s.peters3.ctr@army.mil. If the notification to attend the Site Visit is not provided 24 hours prior to the Site Visit date/time the request will be denied. Site Visits are available at the following dates/times:

a. Primary Site Visit Date: 24 June 2022 Time: 0930 A.M. EST

Alternate Site Visit Date: 30 June 2022 Time: 0930 A.M. EST

8. All questions regarding this solicitation shall be submitted via email no later than 2:00PM EST 01 July 2022 to Timothy McCleary, Contract Specialist at timothy.j.mccleary.civ@army.mil and Katie J. Lyons, Contracting Officer at katie.j.lyons.civ@army.mil. Questions shall only be submitted via email to the Contracting Officer and Contract Specialist during the solicitation phase of this requirement. Do not send questions/inquires to the RFOS or AFOS during the solicitation phase. The questions will be answered all at once and be posted on SAM website (www.sam.gov/ ) once the question period has closed.

9. COMPLETING A QUOTE FOR SUBMITAL

a. Quotes are due no later than the date and time listed in Standard Form (SF) 1449, Block 8 and shall be submitted via email to: Timothy McCleary, Contract Specialist at timothy.j.mccleary.civ@army.mil and Katie Lyons, Contracting Officer at katie.j.lyons.civ@army.mil Updates to this solicitation will be posted on SAM website (wwwsam.gov/).

b. CAGE Code, DUNS Number, Federal TIN, and any small business preference or socio economic concern (such as 8(a) certified, HUB Zone Certified, WOSB, etc.) under the applicable NAICS code, see block SF1449 Block 10.

c. The Offerors shall submit a price for each FFP Contract Line Item Number (CLIN) for the base period and option periods on the SF1449 Continuation Sheets of the solicitation. Offerors are required to complete blocks 12, 17, and 30. Offerors are required to submit a signed SF1449 (Blocks 30a through 30c). In addition, the contractor shall sign and acknowledge any potential resulting amendments to this solicitation. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation.

d. Provide the name, title, address, email address and telephone number of the company/division point of contact regarding business decisions made with respect to the proposal and who can contractually obligate the company. The offeror shall identify those individuals authorized to negotiate with the Government.

e. Pricing for each CLIN must be submitted in the proper format. The proper format consists of QUANTITY * UNIT PRICE = AMOUNT/NET AMOUNT. In the event there is a discrepancy in the calculation, the UNIT PRICE will be held to the intended price multiplied by the QUANTITY. If the offeror shows only the AMOUNT/NET AMOUNT, but fails to enter a UNIT PRICE, the AMOUNT/NET AMOUNT divided by the QUANTITY will be held to be the intended price.

f. The Contractor shall submit with its offer, the Prime Contractor’s valid Certificate of Insurance in accordance with the requirements and applicable insurance amounts set forth within the solicitation at FAR 52.228-5, Insurance – Work on a Government Installation,” and “Insurance Requirements” listed below.

INSURANCE REQUIREMENTS:

Workers 'Compensation and Employer's Liability -At least 100,000.00 coverage

General Liability

- Bodily injury liability insurance coverage shall be written on the comprehensive form of policy of at least $500,000.00 per occurrence

Automobile Liability

-Shall provide automobile liability insurance written on the comprehensive form of policy

- At least $200,000.00 per person for bodily injury

- $500,000.00 per occurrence for bodily injury

- $20, 00.00 per occurrence for property damage

10. All services shall be performed in accordance with the Performance Work Statement (PWS).

11. All offers shall remain valid for 120 calendar days.
