Section SF 30 - BLOCK 14 CONTINUATION PAGE

INSTRUCTIONS & BASIS FOR AWARD

INSTRUCTIONS TO OFFERORS - Addendum to FAR Clause 52.212-1, Instructions to Offerors – Commercial Items.

1. The United States Army Contracting Command – New Jersey (ACC-NJ) has a requirement in support of the 99th Readiness Division (RD) to procure Refuse and Recycling Services located at the Orangeburg USARC, 123 Route 303, Orangeburg, NY 10962-2209 in strict accordance with (IAW) the Performance Work Statement (PWS).

2. The Government intends to award a single firm-fixed-price purchase order. The period of performance for this effort shall be a 6-month base period and four, 12-month option periods from the date of contract award.

3. See Federal Acquisition Regulation (FAR) Clause 52.212-1, “Instructions to Offerors - Commercial Items" for additional information.

4. Contractor shall have an active registration in System for Award Management (SAM) to be considered for award. To obtain information on SAM or to register with SAM visit the web site at: https://www.sam.gov.

5. Questions regarding this solicitation shall be emailed to the Contract Specialist, Deborah E. Terrell at deborah.e.terrell.civ@mail.mil and the Contracting Officer, Sharon Wilson-Emmons at sharon.wilson-emmons.civ@mail.mil no later than 31 July 2019, 1:00 PM EST. Questions must specify the section or paragraph of the Request for Quote (RFQ) for which the clarification is desired.

6. All services shall be IAW the PWS.

7. Contractor offers shall be submitted via the Federal Business Opportunity (FBO) website at https://www.fbo.gov/ using the document upload option. Instructions are available, if needed, in the FBO Vendor Guide on the FBO website. Offers shall include:

- Pricing:

a. Unit and extended Pricing for all Contract Line Item Numbers (CLINs) for the base and option periods found in Section B. A total price should also be included. Prices shall not extend past the second decimal point for unit and extended amounts. The proposed price shall include all personnel, supplies, services, management, overhead, and other direct costs, G&A and profit to fulfill the contractual requirements of the PWS and any Technical Exhibits/Attachments.

b. CAGE Code, DUNS Number, and business-size standard under the applicable North American Industry Classification System (NAICS) code of 562111, Solid Waste Collection.

c. Complete blocks 12, 17 and 30 of the SF1449 as well as signing/acknowledging any amendments. In doing so, the offeror accedes to the contract terms and conditions as written in the Request for Quote.

d. Contractors shall provide a valid copy of the Certificate of Insurance IAW FAR Clause 52.228-5, Insurance - Work on a Government Installation. Coverage amounts shall comply with insurance requirements listed in FAR Subpart 28.307-2.

8. All offers shall remain valid for 120 calendar days.

9. Multiple offers will not be accepted.

10. The following paragraphs under FAR clause 52.212-1 are not applicable to this RFQ: (d) Product Samples and (e) Multiple Offers.

11. Attachments: Site map with dumpster locations, Orangeburg, NY

12. Funds are not presently available for this acquisition. No contract award will be made until appropriated funds are made available.

BASIS FOR AWARD

Evaluation – Commercial Items

1. Award shall be made to a single contractor. Offer must include prices for each line item listed so quotes may be properly evaluated. Failure to do so may be cause for rejection of the entire offer. Award shall be made to the responsible contractor whose total aggregate price is low.

2. The Government intends to award a contract without discussions with respective offerors so offerors should submit their best offer initially. The Government, however, reserves the right to conduct discussions if deemed in its best interest.

3. PRICE:

a. Proposals shall include prices for each line item listed and a total price, in order that proposals may be properly evaluated. Offers shall also include the completed SF 1449 and any amendments as well as the Certificate of Insurance.

b. The Government will evaluate offers for award purposes by adding the total price for all Contract Line Item Numbers (CLIN) (base period and option periods) for the total evaluated price. In the event there is a difference between a unit price and the total CLIN amount, the unit price will be held as the intended price multiplied by the number of units. If the offeror shows only the total amount but fails to submit a unit price, the total CLIN amount divided by the quantity will be held as the intended price. Evaluation of options shall not obligate the Government to exercise the option(s).

c. Unbalanced pricing exists when, despite an acceptable total evaluated price, one or more contract line items is significantly overstated or understated as indicated by the application of cost or price analysis techniques. A proposal may be rejected if the Contracting Officer determines that the lack of balance poses an unacceptable risk to the Government.

4. The Past Performance Information Retrieval System and Federal Awardee Performance & Integrity Information Systems as well any other Government-sponsored information available may be utilized to help make a responsibility determination in accordance with FAR Subpart 9.104.

5. Failure to provide the requested information may deem the proposal Unacceptable and may not be eligible for award.
