AMENDMENT 0002 Issued 15 February 2019

1. Amendment 0002 is hereby issued to extend the soliciation deadline from 2:00 PM EST on Friday February 15 2019 to Thursday February 21, 2019 at 2:00 pm EST. All quotes shall be emailed to both the contract specialist, Karyn Bohli, at karyn.a.bohli.civ@mail.mil and the Contracting Officer, Katie Lyons, at katie.j.lyons.civ@mail.mil.

AMENDMENT 0001 Issued 13 February 2019

1. Amendment 0001 is hereby issued to extend the soliciation deadline from 2:00 pm EST on Wednesday 13 February 2019 to 2:00 pm EST on Friday 15 February 2019. All quotes shall be emailed to both the contract specialist, Karyn Bohli, at karyn.a.bohli.civ@mail.mil and the Contracting Officer, Katie Lyons, at katie.j.lyons.civ@mail.mil.

THIS ACQUISITION IS ISSUED UNRESTRICTED

1. Solicitation W15QKN-19-Q-1096 is issued for the procurement of refuse services in support of the 99th Regional Support Command (RSC). Refuse pick up and disposal services shall be performed in accordance with the Performance Work Statement (PWS).

2. Services shall be performed at the Big Bend United States Army Reserve Center (USARC) located at RR #1, BOX 174, BIG BEND, WV, 26136-9724. All services outlined in the PWS shall be performed.

3. The period of performance for this effort shall consist of one (1) twelve (12) month base year and four (4) twelve month option years. Services shall commence under the resulting contract beginning 01 April 2019.

4. The North American Industry Classification System (NAICS) Code for this acquisition is 562111 Garbage Collection Services, Size Standard $38.5 million.

5. The offeror must be registered and show an active status in the System for Award Management (SAM) in order to be eligible for award. For more information or registration instructions, please visit http://www.sam.gov

6. Site Visits are encouraged for this effort. Site visits shall be conducted on FRIDAY FEBRUARY 01 2019 at 10:00 am. Any contractor who wishes to view the performance site shall email both the contract specialist, Karyn Bohli, karyn.a.bohli.civ@mail.mil and the Area Facilities Operations Specialist, Eric Ritchie, eric.b.ritchie.ctr@mail.mil to confirm their attendance for the site visit.

7. All questions regarding this effort shall be directed to the contract specialist, Karyn Bohli, at Karyn.a.bohli.civ@mail.mil. When emailing the contract specialist, contractors shall also CC the contracting officer for this requirement, Katie Lyons, at Katie.j.lyons.civ@mail.mil. All questions are due no later than TUESDAY FEBRUARY 05 at 2:00 PM EST.

SUBMITTAL OF QUOTATIONS:

8. All quotes in response to this effort are due no later than THURSDAY FEBRUARY 21 2019 at 2:00 PM

EST. All quotes and supporting documentation shall be emailed to the contract specialist for this effort, Karyn Bohli, at Karyn.a.bohli.civ@mail.mil. When emailing the contract specialist, contractors shall also CC the contracting officer for this requirement, Katie Lyons, at Katie.j.lyons.civ@mail.mil

9. CONTRACTOR QUOTES SHALL INCLUDE THE FOLLOWING:

PRICING:

Unit and extended pricing for all Contract Line Item Numbers (CLINS) found in the SF1449 of this solicitation

TECHNICAL:

Contractors shall include a capability statement which shall detail the contractor’s capability to perform the work required at section 5: Specific Tasks and each subparagraph under section 5 of the PWS

Contractor shall provide the name, title, address, email and telephone number of the company point of contact regarding business decisions made with respect to the company’s quote and who may contractually obligate the company

Contract shall provide the following information:

EVALUATION CRITERIA

10. The Government will award a single firm fixed price (FFP) contract resulting from this solicitation to the offeror who has been deemed responsible and whose offer is the lowest price meeting the technical specifications of this solicitation and PWS.

The Government will evaluate offerors based on their ability to successfully complete the work at section 5 of the PWS utilizing the lowest price technically acceptable source selection methodology. The Government will determine total evaluated price by adding the total price of the basic requirement (CLINS 0001-0002) and adding the sum of all option CLINS (1001-4002).

The Government intends to award a single contract without discussions with respective offerors however the Government reserves the right to conduct discussions if deemed in the Government’s best interest.

ADDITIONAL INFORMATION

11. The successful offeror shall furnish a certificate of insurance to the Contracting Officer in accordance with CLAUSE 52.228-4000 “Required Insurance” NO LATER THAN 5 DAYS after contract award.

12. Service Contract Act Wage Determination WD 15-4353 (Revision 8), dated 12/26/2018 is hereby incorporated into this solicitation.

13. Disclosure of Unit Price Information

This constitutes notification pursuant to Executive Order 12600, Pre-Disclosure Notification Procedures for Confidential Commercial Information (June 23, 1987) of our intention to release unit prices in response to any request under the Freedom of Information Act, 5 USC 552. Unit price is defined as the contract price per unit or item purchased. We consider any objection to be waived unless the contracting officer is notified of your objection to such posting prior to the submission of initial quotes.
