1. Please provide your company name, address, point of contact (POC), telephone number, fax number, email address, CAGE Code, and DUNS Number.
2. Please provide your company's business size, average annual receipts for the last three fiscal years (please indicate fiscal period), and number of years in the construction industry.
3. Please provide your applicable business size and/or socioeconomic statuses (i.e. Large Business, Small Business, SBA Certified 8(a) Program Participant, SBA Certified HUB Zone Firm, Service Disabled Veteran Owned Small Business, Woman Owned Small Business, Economically Disadvantaged Woman Owned Small Business).
4. Please demonstrate your ability to execute a broad range of maintenance, repair, and construction of real property (i.e. carpentry, excavation, interior renovation, exterior renovation, electrical, plumbing, painting, paving, roofing, demolition, masonry, HVAC, etc.) from an on-the-shelf design, or a design provided by in-house or subcontract designers (including design/build ranging from concept up to 100% level of effort). Identify specialty construction trades performed with company employees, if any, as well as experience in delivering multi-trade design-build projects.
5. Please identify your in-house Architect and Engineering (Up to 100%) design capability or ready access to design capability. Identify if you have in-house design capability, if you subcontract all Architect/Engineering design work, if you have state certified Professional Engineers and/or registered Architects on staff, and which disciplines you have the ability to design in-house (i.e. Mechanical, Electrical, Structural, Fire Protection, etc.).
6. Please provide past/current (present to up to past 5 years) performance background such that you were the Prime Contractor, to include:
a. Contract identification number(s)
b. Title and a brief summary of the objective of the effort
c. Contracting agency or firm (Government or commercial)
d. Point of Contact information such as: Name/Email/Telephone Number
e. Value of the contract(s)
f. Type of contract(s), i.e., fixed price, cost reimbursement, labor hour
g. Design-Build or Bid-Build. Percentage of work done as the Prime Contractor
7. Please identify the largest multi-trade project your company has successfully delivered (to include dollar amount, client, date completed, renovation or new construction).
8. Please identify your bonding capacity (single and aggregate) and name(s) of surety(ies).
9. Please identify any commercial practices that the Government should consider implementing for this acquisition.
10. In order to be interested in proposing on a MACC program roughly like that described on the SAM.gov posting, would the minimum guarantee value impact your decision to propose?
a. If you answered 'Yes' to question 10, what should the minimum dollar guarantee amount be and why?
11. Suppose the Government solicited for a MACC program roughly like that described on the SAM.gov posting-what do you think the ideal number of awardees should be, or what do you think the ideal range of awardees should be (e.g. 3-5) and why?
12. Do you have other business locations that are registered in SAM?
a. If you answered yes to question 12, please provide addresses for all locations.
b. If you answered yes to question 12, which location would most likely serve as the offeror on a MACC program roughly like that described on the SAM.gov posting?
13. For questions 13-19, assume a MACC program roughly like that described on the SAM.gov posting. Would you be able to provide a Program Manager with at least 5 years of experience in the management of government, commercial, or industrial construction and renovation projects, involving all of the following different disciplines: architecture, civil, mechanical, and electrical?
14. Would you be able to provide a Construction Manager with at least 5 years of experience in construction, superintendence for similar size/scope projects, familiar with all aspects of general construction management, practices, methods, and materials?
15. Would you be able to provide a Job Foreman on each project site that possesses at least 3 years’ experience in the field of work of the job being supervised?
16. Would you be able to provide a Quality Control Officer with at least 3 years’ experience in the area of construction quality control?
17. Would you be able to provide an Environmental Officer with at least 3 years’ experience in environmental management for construction work (which could be fulfilled by a consulting firm)?
18. Would you be able to provide a Site Safety and Health Officer, who could also be the previously mentioned Quality Control Officer, with at least 3 years’ experience in safety work on construction projects and 30-hour OSHA construction safety class, or equivalent, within the last 3 years?
19. Would you be able to provide a California licensed Professional Engineer (P.E.), a California Registered Architect (R.A.), and/or a NICET Certified Technician with at least 5 years of experience in design, design review, design coordination, design quality assurance, constructability reviews, for similar size/scope projects? If yes, elaborate on which one(s) of the three professional designations, and how many of each, you could provide.
20. Of the Key Personnel identified in the preceding questions 13-19, do you think any are unnecessary, redundant, would make performance less cost effective, or should otherwise not be required for a MACC program roughly like that described in question l?
a. If you answered ' Yes' to question 21, identify which personnel position(s) you might take exception to and why.
21. Within the past 5 years, have you performed construction work under indefinite-delivery type master contracts, where each contract involved performance at 2 or more geographically separated locations (bases, forts, air stations, etc.), with varying levels of project complexity, and involving simultaneous performance of two or more projects?
a. If you answered yes to question 21, provide (below or as an attachment) your most recent examples of indefinite-delivery type master contracts (up to 5) that involve performance at 2 or more geographically separated locations-include customer(s), work locations, contract number, maximum contract value, and whether each contract includes only bid-build work OR bid-build work and design-build work.
22. Assume a MACC program roughly like that described on the SAM.gov posting. How do you think you would involve: yourself as a prime, predecessor companies, key personnel, key sub-contractors, joint ventures, teaming partners, and/or mentor-protégés, to establish a record of experience?

24. What estimating/pricing software have you/might you use to formulate a competitive proposal for a construction project, if any? Also, to what extent have you/might you rely on subcontractor bids to formulate a competitive bid for a construction project?
25. Is there any other information or suggestions you would like to share and think would be helpful to the Government in developing its acquisition strategy for a MACC program roughly like that described on the SAM.gov posting?
