About this file
This document is the Q&A from a vendor conference for Solicitation #24-7026/2030-RFQ02 for Dental Laboratory Services issued by the DeKalb County Board of Health (DKPH) in Georgia. DKPH is seeking a qualified vendor to provide high-quality workmanship in the fabrication of fixed and removable dentures, partials, crowns, bridges, and other dental services. The contract will be for one year with four one-year renewal options. Vendors can submit pricing for the full contract term and have the option to request cost increases for any renewal years. DKPH plans to award one primary contract, with the possibility of a backup contractor.
The pricing form, Exhibit F, lists 30 line items for various dental laboratory services such as complete dentures, partials, crowns, bridges, pediatric items, and other miscellaneous items. Vendors are required to provide per unit pricing as well as delivery timeframes. DKPH will pay vendors via ACH, and the insurance requirements for the contract are detailed, including statutory worker's compensation, professional liability, and comprehensive liability coverage. Vendors not located in Georgia can use the provided insurance requirements as a guide for their home state.

Solicitation # 24-7026/2030-RFQ02
DENTAL LABORATORY SERVICES
Solicitation #24-7026/2030-RFQ02
Vendor’s Conference Q&As May 30, 2024
1. Will a vendor be allowed to participate if they are not within a 50-mile radius of T.O. Vinson Health
Center located at 440 Winn Way, Decatur GA 30030?
Yes. We have agreed to expand vendor servicing beyond the 50-mile radius; however, the vendor will still be required to meet the deliverables as required of vendors located within a 50-mile radius.
2. Must the provider physically come and pick up order requests from T.O. Vinson?
The impressions are to be retrieved daily. Pick-up is to be from the T.O. Vinson Health Center located at 440 Winn Way, Decatur, GA 30030.
3. Could a courier be used for the pick such as UPS/FedEx? If providing the shipping boxes and the foam your dentist would enclose the impression and shipping label, then just schedule UPS to come pick it up or if they're already in your facility regularly, they would just pick it up the same day.
Yes.
4. Are the prices needed for the whole contract award period? Does that mean all four years?
Our contracts are written for one year with four renewal year options. The amount for each year contract will remain as stated in the initial contract.
Vendors have the option of using escalators in their bid proposal. The vendor can also opt to request a cost increase for any renewal year award, thereby forgoing escalators. If electing to request a cost increase, the vendor must justify the cost increase request.
5. Referencing the Price List, is it flat-rated or all-in? Or is it where we can include certain items that go along with restorations?
Please provide your cost for what we've identified. For anything not identified on the Price List that you desire DKPH to consider, include that detail with cost as well.
The Price List/Cost Proposal has been updated and is attached below as Exhibit F.
6. Since we can submit more items/services what about implants, is that something that's prescribed?
No. We do not provide implant services.
7. Do you normally request economy teeth or is it expected to be premium teeth?
Something in between, we look for quality teeth since we are a local health agency using state/public funding.
8. How many vendors or contracts are you looking to award?
We are looking for one, but we are open to having a backup if it works within our bottom-line number to allocate funds per contract. (2 contractors)
9. How are invoices to be paid?
DKPH remits payment via ACH. Each contracted vendor is required to provide their banking details for account set-up. If the vendor does not provide ACH information, they are paid via check.
10. In the event, cases are to be picked and delivered to each clinic how many individual locations would this be?
There will only be one pick-up/delivery location which will be at the T.O. Vinson Health Center located at 440 Winn Way, Decatur, GA 30030.
11. Since quantities aren’t provided, can you provide the approximate number of dentists at each location available to service patients? This can give some idea of estimated volume to assist in providing the best cost.
There’s 1 full-time dentist, two hourly dentists, and one dentist vacancy. They are floaters between the clinics.
12. Our company is not located in Georgia, so is it possible to provide the insurance requirements based on our state of incorporation instead?
Because we do not have the insurance requirements for any state other than Georgia, we can only provide you Georgia’s requirement whereby you can in turn use this guide in sharing with your insurance provider the requirements for this contract award.
Along with the contract documents sent to DKPH for execution, the contractor shall furnish
Certificates of Insurance from companies doing business in the State of Georgia or written evidence of self-insurance acceptable to DKPH covering:
