About this file

This is a Vendor Affidavit document required by Hillsborough County Public Schools (HCPS) in Florida for all nongovernmental entities entering into contracts with the school district. The affidavit is mandated by Florida Statutes Section 787.06(13) and addresses human trafficking and labor coercion compliance. An authorized representative of the contracting company must sign the affidavit under penalty of perjury, certifying that the contractor does not engage in coercive labor practices. Prohibited activities include using or threatening physical force, restricting personal freedom without lawful authority, establishing exploitative debt arrangements tied to labor, confiscating immigration or government identification documents, causing financial harm through threats, deceiving individuals for exploitation, or providing controlled substances for exploitation purposes. The affidavit applies to all contract executions, renewals, and extensions with HCPS.

This affidavit is a mandatory compliance requirement for the Salon Stations and Sinks procurement at Plant City High School, valued at approximately $50,000. Vendors responding to the Invitation to Quote must submit this signed document as part of their bid package along with other required certifications including debarment status, foreign ownership compliance under Florida Statutes Section 287.138, and federal telecommunications equipment prohibitions. The affidavit functions as a prerequisite qualification document and must be completed by an officer or authorized representative with direct knowledge of the contractor's operations and labor practices. Failure to provide a properly executed affidavit will likely result in bid rejection, as it is a mandatory contractual requirement established by state statute.

• entice or lure any person by fraud or deceit; or

• provide a controlled substance as outlined in Schedule I or Schedule II of § 893.03, Florida Statutes, to any person for exploitation of that person.
