The affidavit specifically addresses compliance with Florida Statutes Section 787.06(13) regarding human trafficking prevention and the use of coercion for labor and services. The document requires an authorized representative of the contractor to certify under penalty of perjury that the company does not engage in prohibited practices including physical force, unlawful restraint or confinement, exploitative debt practices, document confiscation, financial harm threats, fraud, or provision of controlled substances for exploitation purposes. Contractors must sign the affidavit with their name, title, and date before submitting their bid for the forklift procurement (ITB 25562-ITB-EST), with bids due December 8, 2025, at 4:30 PM via www.myvendorLink.com.

This affidavit is a mandatory compliance document for the forklift procurement contract estimated at approximately $108,000, with potential automatic renewal for up to four additional one-year periods contingent on appropriations and contractor performance. The document serves as a prerequisite for bid submission and ensures contractors meet ethical and legal standards established by Florida law. No exceptions or alternatives to this certification are provided; all contractors must execute and submit the affidavit as part of their bid package to be considered for award.
