a) Issuance of Amendment A00004: The purpose of this amendment it to extend the proposals due date and to answer vendor follow-up questions and clarifications from amendment A00002.

1) In amendment 2, the VA said it wanted 3 exam rooms per PACT plus one consult room per PACT.
1. Is a women’s health exam included in the 3 exam rooms per PACT or is it in addition to the three? Yes, it can be.
1. Will the VA allow one of the three exam rooms per PACT to be used as the consult/exam room. Yes
2) In amendment 2, question 9, the government notes it will need three exam rooms for its mental health providers. Are these part of the 3 exam rooms per PACT or are they separate? Separate Do these individuals require separate offices? Yes, as they will be providing patient care.
3) In amendment 2, the VA said it will provide a Social Worker, Dietician, and PharmD on site.
a. Since they will be in the clinic part time, how many offices does the VA require for them? Two offices used as swing space.
b. What are the size requirements for these offices? Standard room size. 125 sq ft.
4) In amendment 2, question 32 the Government said Onstage/Offstage is NOT required. Please confirm it is not required and a traditional buildout is acceptable. Onstage/offstage is required. The type of doors used to accomplish this is at the judgement of the contractor.
5) Per Amendment question 48- VA will provide radiology. Could the VA clarify is they plan to provide radiology on site? Radiology will not be provided on site.
6) Does the tele-health tech have to be a LPN or MA? No
7) The RFP calls for tele-health tech – 1 per 5000 patients enrolled. Given the enrollment for Granbury, does the VA still want the same ratio as this will result in tele health tech less than 2 days a week. The contractor is required to provide staffing to cover the telehealth rooms 5 days a week.
8) The VA’s response to question 1 and 2 in the Amendment A0002 states that the current number of enrolled and billable veterans is 1,617.
a. Please explain the source of the 1,617? This is based on the number of active/billable unique SSN currently utilizing Granbury CBOC.
b. Why is there a discrepancy between the 1,617 and what is shown as the Team Assignment at the Granbury CBOC as shown on the PACT COMPASS report (snapshot below). The billable number is scrubbed monthly.
9) Which enrollment number should prospective bidders use as the starting enrollment? The 1,485 shown in the PACT COMPASS report (shown above), the 1,617 stated in Amendment A0002 as the most recent enrolled and billable or the 1,550 shown as the Base year estimated quantity in the RFP? A discrepancy of up to 132 enrollees could pose a significant financial risk if the contractor was to price based on the higher enrollment number (resulting in a lower PMPM) but realizes the lower enrollment number when the contract has begun. Each month there is a change in billable unique. The clinic enrollment would be captured with the team assignments and pending assignments.
10) The VA’s response to question 7 does not align with what is shown in the PACT COMPASS report shown below. The Team Assignment for Dec-FY16 shows 1,579 and then has a dramatic drop to 373 in Jan-FY16 with a subsequent recovery to 1,601 in Apr-FY16. Please explain the cause this drop and recovery.
As already stated, there was no significant drop in active enrollees during that timeframe.
11) Question 32 in Amendment A0002 has two questions for which the VA provided only one answer. Please clarify. The questions are:
c. Does the current CBOC floorplan meet the requirements of the RFP? No
d. Does the VA require the PACT design of Onstage/Offstage? Yes
12) In section 4.7 SPACE REQUIRMENTS: The RFP clearly states, in the description grid under the “VA Provided” column, that phones are to be provided by the VA. However, The VA responded “No” to question 35: Is the VA supplying the phones and phone system? As this can be a significant additional cost to the proposal, please clarify by answering the following question. Will the Contractor be responsible for purchasing and installing the phones/phone system for the CBOC?

The VA’s response to question 41 states that the government would like 3 patient care rooms per PACT team. Can one of the 3 Patient care rooms be configured as a Female Exam room? Yes
13) we have just identified a latent ambiguity in regard to which VA directive controls panel sizes, and we respectfully request that this ambiguity be resolved for all offerors:
Paragraph 2, p. 17, states “Actual panel sizes can be determined by the VA in accordance with PCMM Handbook 1101.02.” It is our understanding that PCMM Handbook 1101.02 has been superseded by VHA Directive 1406. In fact, VHA Directive 1406 is referenced under Section 1.4, which provides relevant VA policies and procedures, while Section 1.4 does not list PCMM Handbook 1101.02 as a relevant VA policy/procedure. Because of this, we find paragraph 2 of the Solicitation to be ambiguous because it refers to directive that has been superseded by a newer directive. Please clarify if panel sizes should be in accord with PCMM Handbook 1101.02 or with VHA Directive 1406.
