About this file

This is a VA CWM Utility Shutdown Request form and instructions document used to request and manage utility service interruptions at VA medical center facilities.

The form requires contractors to submit shutdown requests with detailed information including project number, company name, shutdown point of contact with phone number, requested shutdown date(s) with expected start and stop times, location of shutdown, location of work, location of LOTO equipment, competent person(s) performing the work, required PPE, reason for shutdown, and description of work. Contractors must identify all buildings and areas impacted by the shutdown, including downstream and upstream systems affected. The form requires identification of which services are being shut down (steam, HVAC, power, domestic water, heating hot water, chilled water, sanitary system, nurse call, fire alarm, sprinkler system, IT, emergency power, or other) and which services are affected during the work period. Contractors must provide photos of all affected panels and panel schedules with the request.

The document establishes notification timelines: minor shutdowns require seven calendar days' notice while major shutdowns (those affecting medical center operations) require 21 calendar days' notice. All fire alarm and sprinkler shutdowns require coordination with VA Fire Alarm Technician. VA M&O shops require 72-hour notice from VA Contracting Officers Representatives (COR) for all shutdowns. LOTO equipment and mandatory PPE must be on site; failure to have proper equipment results in cancellation. The form includes sections for COR approval/denial and Operations & Maintenance shop acknowledgment, with specified responsibilities for contractors, VA COR, and M&O shops throughout the shutdown coordination and execution process.
