About this file

This document is a U.S. Preference Form related to procurement regulations for products manufactured in the United States, likely for a state or local government contract opportunity. The form outlines specific criteria for vendors claiming a five percent preference for U.S.-manufactured products, including requirements that the items must be produced entirely within the United States, with manufacturing, assembly, processing, packaging, and testing occurring domestically. The preference applies when the U.S.-made products are equal in quality to alternative products and do not exceed the cost of foreign-manufactured items by more than five percent.

The form provides a detailed framework for vendors to claim this preference, requiring them to specify which line items qualify, provide the location of manufacture, and certify the U.S. origin of the products. Key conditions include the vendor agreeing to sell at the lowest bid price, the products meeting quality standards, and the manufacturing process adding value within the United States. The document is structured to allow vendors to declare their eligibility for the U.S. manufacturing preference by checking yes or no and providing additional details about the manufactured products' origin and specific line items.
