About this file

This is a Statement of Work (SOW) for an Indefinite Delivery/Indefinite Quantity (IDIQ) contract supporting the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 22 (VISN 22) Urology department. The SOW establishes requirements for the supply and maintenance of urology scopes and related equipment to support URO (Urology) services across the VISN 22 network.

The SOW outlines the contractor's responsibilities for providing high-quality urology endoscopic equipment, including rigid and flexible scopes, with specified performance standards and delivery timelines. Key requirements include ensuring equipment meets clinical standards, providing preventive and corrective maintenance support, maintaining inventory levels to support patient care operations, and delivering equipment within specified timeframes to support VISN 22 medical facilities. The contract vehicle is structured as an IDIQ agreement, allowing for variable quantities of scopes and related equipment to be ordered as needed throughout the contract period. The SOW emphasizes compliance with VA medical equipment specifications, quality assurance protocols, and the contractor's obligation to support uninterrupted urology service delivery across multiple VA medical centers within VISN 22. Pricing terms, specific equipment models, and detailed performance metrics would be established in accompanying contract documents and pricing schedules.

7.1.2.2 IAW VA 0710 Handbook, appointees and contract personnel appointed to Low/Moderate/High Risk positions must be subjects of a background investigation conducted by OPM and receive a favorable adjudication from the VA Office of Security and Law Enforcement to ensure compliance with such policy. However, a Security Access Clearance (SAC) background screening will be required for appropriate preliminary checks IAW VA Directive 0710.

7.2 Information Technology Security Considerations

7.2.1 Prior to termination or completion of this Contract, Contractor/subcontractor must not destroy information received from VA, or gathered/created by the Contractor in the course of performing this Contract without prior written approval by the VA. Any data destruction done on behalf of VA by a Contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. Self-certification by the Contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the Contract.

7.2.2 All electronic storage media used on non-VA leased or non-VA owned IT equipment that is used to store, process, or access VA information must be handled in adherence with VA Handbook 6500.1, Electronic Media Sanitization upon: (i) completion or termination of the Contract or (ii) disposal or return of the IT equipment by the Contractor/subcontractor or any person acting on behalf of the Contractor/subcontractor, whichever is earlier. Media (hard drives, optical disks, CDs, back-up tapes, etc.) used by the Contractors/subcontractors that contain VA information must be returned to the VA for sanitization or destruction or the Contractor/subcontractor must self-certify that the media has been disposed of per 6500.1 requirements. This must be completed within 30 days of termination of the Contract.

7.2.3 Bio-Medical devices and other equipment or systems containing media (hard drives, optical disks, etc.) with VA sensitive information must not be returned to the vendor at the end of lease, for trade-in, or other purposes. The options are:

7.2.4 VA’s initial medical device purchase includes a spare drive which must be installed in place of the original drive at time of turn-in; or

7.2.5 VA must reimburse the company for media at a reasonable open market replacement cost at the time of purchase.

7.2.6 Due to the highly specialized and sometimes proprietary hardware and software associated with medical equipment/systems, if it is not possible for the VA to retain the hard drive, then;

7.2.7 The equipment vendor must have an existing BAA if the device being traded in has sensitive information stored on it and hard drive(s) from the system are being returned physically intact; and

7.2.8 Any fixed hard drive on the device must be non-destructively sanitized to the greatest extent possible without negatively impacting system operation. Selective clearing down to the patient data folder level is recommended using VA-approved and validated overwriting technologies/methods/tools. Applicable media sanitization specifications need to be pre-approved and described in the purchase order or Contract.

7.2.9 A statement needs to be signed by the Director (System Owner) that states that the drive could not be removed and that (a) and (b) controls above are in place and completed. The ISO needs to maintain the documentation.

7.3 Insurance Requirements for work performed on Facility Grounds

7.3.1 The Contractor agrees to procure and maintain, while the Contract is in effect, Workers' Compensation and Employee’s Public Liability Insurance in accordance with Federal and State of California and/or Nevada laws.

7.3.2 The Government shall be held harmless against any or all loss, cost, damage, claim expense or liability whatsoever, because of accident or injury to persons or property of others occurring in the performance of this Contract.

7.3.3 The Contractor shall be responsible for all damage to property, which may be done by him, or any employee engaged in the performance of this Contract.

7.3.4 Before commencing work under this Contract, the Contracting Officer shall require the Contractor to furnish certification from his/her insurance company indicating that the coverage specified by FAR 52.228-5 and per FAR Subpart 28.307-2 has been obtained and that it may not be changed or canceled without guaranteed thirty (30) day notice to the Contracting Officer.

7.3.5 Contractor is required to provide copies of proof of Workers' Compensation and Employee Public Liability Insurance within fifteen (15) calendar days after notification of Contract award.
