*************************************IMPORTANT************************************** CONTRACTORS MUST BE REGISTERED IN SYSTEM FOR AWARD MANAGEMENT (SAM) AT WWW.SAM.GOV. IN SAM MUST MEET THE SMALL BUSINESS SIZE STANDARD REQUIREMENT FOR NAICS CODE 238990 (reference FAR provision 52.219-1 in the Representations & Certifications section of your SAM registration).
I. Each offeror SHALL complete and submit the following forms, clauses, and statements with his/her offer listed below.
Failure to furnish all required information, any rejections of and/or addition to the terms and conditions of the solicitation may cause your offer to be considered as non-responsive and ineligible for award.

A. SECTION 1:
Standard Form-1442-Solicitation, Offer, and Award. Pages 1 & 2
Complete Blocks 14 thru 20c (remember to include the signer’s title in block 20a). If you company has a “doing business as” in block 14. Ensure the address typed in block 14 matches the address in your SAM registration.
Block 21 thru 31 are to be completed only by the government.
Include DUNS Number, Tax Identification Number, CAGE Code, and email address where indicated in block 10.

Complete the unit price (JB as job) and Amount blocks (these amounts will be the same).
NOTE: the amounts listed in this section should be the same as the amount listed in block 17 of the Standard Form – 1442.
C. SECTION 6:
Provisions, page 22 Provisions 52.215-20 Alternative-IV:
Submit a cost breakdown of your quotation pursuant to the instructions in this provision. This listing should be of individual costs (e.g. labor hours and cost per hour, material units and cost per unit, etc.) not merely lump sums.
D. Amendments, if any, must be acknowledged and submitted with the offer by one of the methods. Described in term eleven of the Standard Form-30 (amendment of solicitation/modification of contract).

| II. | PRE-QUOTATION CONFERENCE/SITE VISIT: |
*********************************IMPORTANT***************************************** A pre-quotation conference and site visit are scheduled for 07/26/2023 at 10:00 A.M. Central Standard Time at MCC CHICAGO 71 WEST VAN BUREN ST, CHICAGO IL, 60605.
This is the only pre-quotation conference/site visit scheduled. The pre-quotation conference/site visit is not mandatory (see provision 52.236-27 Alternate of the solicitation). However, all offerors are urged to attend.
NCIC background checks will be required for participants attending this conference. The authorization for release of information (Form BP-A0660 NCIC Check, attached) shall be completed for each attendee and submitted to MCC Chicago no later than 07/24/2022 by 10:00 AM CST. Email form to Michael Steiner, Supervisory Contract Specialist, msteiner@bop.gov. For questions on filling out the form, contact Michael Steiner at 312-322-0567 ext. 1522.
The forms must include a cover letter indicating they are for the MCC CHICAGO UNIT 13 SHOWER RENOVATION project in addition to the following information:
Company Name and list/number of individuals from the company who will be attending.
Phone number/point of contact for the company for notification of any NCIC checks that are denied.
NOTE: The FBOP is prohibited from divulging any information regarding any reasons for NCIC denials. Only the fact that a person has been denied access will be communication to the company point of contact.
The point of contact for the pre-quotation conference/site visit is Michael Steiner, Supervisory Contract Specialist, 312-322-0567 ext. 1522, email msteiner@bop.gov Any questions relating to the solicitation after the closing of the pre-quotation conference/site visit shall be sent to Michael Steiner, Contracting Officer, via email at msteiner@bop.gov no later than 08/07/2023 at 4:00 P.M. CST. In order to provide sufficient response time prior to the offer due date, the Government is establishing the above cut-off date questions about the solicitation. All offerors shall offer in accordance with the current solicitation to include all amendments (if any) as issued by the Contracting Officer Only.
