Abatement Project – Warehouse - #TJCC2014-603 Tulsa Job Corps Center 1133 N. Lewis Avenue Tulsa, OK 74110

Scope of Work

Inspect the Warehouse for asbestos-containing materials per the inspection protocol in 40 CFR 763 and Oklahoma state regulations. Sample and analyze any suspect material not previously sampled and determined to contain asbestos.

Perform destructive testing as needed to uncover and sample asbestos-containing materials in areas to be demolished or of materials to be disturbed by renovation. Specifically, the extent of layers of asbestos floor tile or floor tile mastic below carpeting, non-asbestos tile or other flooring shall be investigated. Pipe insulation in chases and behind walls shall be investigated on heating pipes and domestic water pipes. Building project area with a steel structure behind bulkheads or finished columns shall be investigated for asbestos fireproofing. Space above ceilings shall be investigated until the deck for the floor above is reached. Repairs of areas where samples are collected are the responsibility of the asbestos inspector. Coordinate repair with the Center Operator.

Conduct a field investigation for PCB fluorescent light ballasts and caulk. Review at least 2% of ballasts within the buildings project area, choosing older lighting fixtures where possible. The consultant shall not sample dielectric fluid from ballasts but shall rely on labeling to determine PCB status. Unlabeled ballasts shall be assumed to be PCB-containing. Note leaking PCB ballasts for special removal and disposal per 40 CFR 761.

List mercury containing materials such as fluorescent light bulbs for removal and proper disposal prior to demolition.

Perform a survey for lead-based paint using paint chip samples or XRF detector. The survey shall be performed for HUD Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing Chapter 7: Lead-Based Paint Inspection.

All work shall be completed by trained and licensed Oklahoma asbestos and lead inspectors. Prepare a report which lists all hazardous materials identified with assessment of their condition and recommendations. Include a building floor plan with the location of hazardous material and the location of samples collected. Estimate the cost of removal for all identified hazardous materials.

Provide specifications and drawings for abatement prepared by a licensed asbestos project designer in Oklahoma.

Provide unit prices for removal of the following:
