Statement of Work 2020 – Kayenta Health Center: DMC Logistics Author: Alfred Lugo, Lead CLS

BACKGROUND

The Kayenta Health Center Laboratory’s function is to perform laboratory testing as requested by providers in a timely manner. Results are reported to provide information used in diagnosing and treating patients. Not all testing can be performed on site due to cost and staffing limitations. Patient specimens for special testing is sent to a reference lab with patient specimen being received at the reference laboratory within 12 hours. Microbiology and other special testing specimens are required to be received within 24 hours after specimen collection due to the specialized nature of testing. The reference laboratory provides transportation of specimens Monday through Friday, but does not provide this service on weekends and holidays.

SCOPE

Specialized testing not performed at the KSU laboratory includes: microbiology, special immunoassay testing, mycology, virology, PAP testing, tissue processing and other testing as required by the KSU providers. The reference laboratory provides transportation of laboratory specimens only on regular business days of Monday through Friday. KSU laboratory requires transportation of specimens to the reference laboratory on weekends and holidays for specialized testing.

OBJECTIVES

DMC Logistics will provide the transportation of the laboratory specimens for testing to the reference laboratory on weekends and holidays so that the specialized testing can be completed in a timely manner in order for the results to be used along with the KSU lab test results to determine diagnosis and appropriate treatment of the patient.

TASK

The courier, DMC Logistics, shall provide transportation of laboratory specimens on a weekend and holiday basis.

The vendor shall provide the equipment necessary to transport all specimens sent to the reference lab including containers with dry ice. Other items needed to safely collect and transport specimens to the testing site shall also be provided by the vendor.

DELIVERY

The courier shall provide transportation of laboratory specimens in a timely manner. All laboratory specimens will be transported appropriately at the right temperature and condition to the reference laboratory.

GOVERNMENT-FURNISHED PROPERTY/EQUIPMENT

The KSU lab will provide space for the vendor, appropriate areas required such as a refrigerator and freezer, necessary to maintain specimen integrity as well as space to process specimens.

SECURITY REQUIREMENTS

No special security is required at this time. All couriers entering the lab shall identify themselves to the KSU lab staff.

PLACE OF PERFORMANCE

Specimens will be collected by KSU staff and processed for pickup by the KSU lab staff. The courier shall deliver the specimens to the processing lab in Phoenix, AZ.

PERIOD OF PERFORMANCE

The period of performance shall be January 1, 2020 – December 31, 2020.
