About this file
This is a Goods and Services Proposal and Fees template attachment for an Electronic Medical Records System RFP issued by The Rector and Visitors of the University of Virginia, Department of Student Health and Wellness, located in Virginia. The RFP solicits a complete Electronic Medical Record (EMR) platform with associated implementation and ongoing support services. Respondents must provide comprehensive proposals organized by defined service categories including the core EMR platform and modules, implementation and project management, system configuration, data conversion and migration, interfaces and integrations, testing and quality assurance, training and change management, go-live and post-go-live support, hosting and maintenance, optional modules, and third-party products or services. For each offering, firms must clearly delineate whether items are included in the base proposal, required for implementation or operations, optional, or provided by third parties, along with associated assumptions, dependencies, limitations, and exclusions.
Pricing submissions must provide complete transparency and distinguish between one-time implementation costs, recurring subscription and licensing fees, usage-based or transactional fees, optional services, and third-party pass-through expenses. Implementation pricing must be organized by proposed milestones with identified deliverables and fee allocations for each phase, including total cost summaries for the initial implementation and each contract year. Firms must clearly identify all assumptions, dependencies, exclusions, minimum commitments, and circumstances that could trigger additional fees. Following award, the University and selected firm will execute a detailed implementation Statement of Work or project plan that establishes final scope, deliverables, milestones, responsibilities, acceptance criteria, change-control procedures, payment schedules, and University approval requirements, with payments contingent upon University review and acceptance of applicable deliverables.

· Implementation and project management services;
· System configuration and workflow design;
· Data conversion and migration;
· Interfaces and integrations;
· Testing and quality assurance;

For each item, the Firm shall identify whether it is included in the base proposal, required for implementation or ongoing operation, optional, or provided by a third party. The Firm shall also clearly identify any assumptions, dependencies, limitations, and exclusions.
The Goods and Services listed in this section shall correspond directly to the items included in the Firm’s Fee Structure/Pricing Methodology.
B. Fee Structure / Pricing Methodology:
The Firm shall provide a complete and transparent fee and rate structure for all Goods and Services included in its proposal. Pricing shall clearly distinguish between one-time implementation costs, recurring costs, usage-based costs, optional services, and any third-party or pass-through expenses.
At a minimum, the Firm shall provide pricing for:
1. One-time implementation services, including project management, discovery, system configuration, data conversion and migration, integrations, testing, training, go-live support, and post-go-live stabilization;
2. Recurring subscription, licensing, hosting, maintenance, and support fees;
3. Transactional, volume-based, per-user, per-provider, per-module, or other variable fees;
4. Optional modules, products, services, and enhancements;
5. Professional services rates for work performed outside the initial implementation scope;
6. Third-party products, interfaces, services, travel, and other pass-through costs; and
7. Any anticipated annual price adjustments or escalation factors.
Implementation pricing shall be organized by the Firm’s proposed implementation milestones. The Firm shall identify the deliverables associated with each milestone and the portion of the implementation fee attributable to that milestone.
The Firm shall also provide a total cost summary for the initial implementation period and each year of the proposed Agreement term. All assumptions, dependencies, exclusions, minimum commitments, and circumstances that could result in additional fees shall be clearly identified.
Following award, the University and the Selected Firm will develop and execute an implementation Statement of Work or mutually agreed implementation project plan. The implementation document will define the final scope, deliverables, milestones, responsibilities, dependencies, acceptance criteria, change-control process, payment schedule, and University approval requirements. Payments associated with implementation milestones will be subject to University review and acceptance of the applicable deliverables.
