PLEASE NOTE: IN ADDITION TO PRICING PROVIDED ON THE BID/PRICE SCHEDULE, YOUR RESPONSES ON THIS QUESTIONNAIRE (AND YOUR RESPONSES FOR PAST PERFORMANCE/PRIOR EXPERIENCE) ARE CRITICAL TO THE SELECTION OFFICIAL WHEN EVALUATING A BID/QUOTE/PROPOSAL FOR AWARD. Question #1

Please describe in detail how you plan to complete this project while adhering to the statement of work and period of performance and keeping costs in line with the price schedule pricing provided in your bid/quote/proposal – this includes the use of subcontractors or if the contractor plans to complete the work in house. (As applicable - this must include information pertaining to the use of subcontractors and who those subcontractors are that are being utilized to complete this work as outlined in the SOW – this would include the company name, address, contact information and confirmation that the subcontractor is currently secured for this project.) - If this info is not apparent, the quote package may be deemed non-compliant.

Please explain in detail your company’s experience with this type of work as outlined in the Statement of Work. (This is separate from your list of prior experience – as applicable, this can be the experience of the subcontractor). ALSO – verify that all proper certifications/licenses that are required to be able to perform this work on site are annotated in this experience question so we can verify that the selected offeror meets that criteria.
