Scope of Work

1.1 Work associated with these items is described as follows. Incidental work items not listed that are necessary for completing the project shall be included. The general project scope is to provide services, materials, labor, and equipment to rehabilitate the 2ndd floor bathroom.

Unless noted otherwise, work is generally confined to the D9 Representational Facility, 12992

Edgewater Drive, Lakewood, Ohio 44107.

1.1.2 Remove the existing toilet, lavatory, light fixtures, and mirror.

1.1.3 Remove the existing floor tile and prep the subfloor for new ceramic tile.

1.1.4 Provide a new low flow toilet and seat, new 36 inch vanity cabinet with three drawers and integral sink, faucet and related plumbing.

1.1.5 Provide and install new ceramic tile floor and base.

1.1.6 Provide new mirror/medicine cabinet, light fixture(s), and replace/relocate one (1) GFCI outlet.

1.1.7 Remove existing bathtub and acrylic surround. Install new acrylic 15 inch tub and surround, white in color. Integral grab bars are to be installed in bathtub surround similar to the existing bathtub surround. A tub insert is not acceptable.

1.1.7 Install new ½” moisture resistant drywall as required due to demolition.

1.1.9 Prepare and paint the walls of the bathroom.

1.1.10 Clean and prepare the new bathroom for occupancy.

1.1.11 The following items are to be provided and installed. They will be selected by the

Contracting Officer’s Representative:

a. Low flow toilet with toilet seat

b. Vanity (36 inch), solid surface/cultured marble vanity top with integral sink.

c. New acrylic 15 inch tub, white in color

d. Faucet, Shower Valve, Showerhead.

e. Exhaust fan (maximum 1.5 sones, minimum 50 CFM)

f. One (1) GFCI outlet and cover

g. Ceramic floor tile and base

h. Mirror/medicine cabinet

i. Light fixture (s)

j. Towel bars

k. Robe hook

l. Toilet paper holder

m. Free standing shelf with doors below to store paper products

1.2 Repaint all surfaces with a premium solid white color. All surfaces to be painted shall be properly prepared, (i.e. cleaned, sanded, feathered, puttied, primed, and etc.) as necessary. Estimated surface to be painted is 54 sf. Surface preparations will include the repair of damage to finished surfaces, (i.e. splits, cracks, nail holes, chips, indentations).

All surfaces shall be free of dirt, grease, loose debris, and any other foreign matter that would adversely affect the appearance.

1.3 Where caulking is required, such as the window, doors, and trim work, should be accomplished along with all other preparation work. Caulk will be furnished by the contractor and will be matching in color and made exterior use.

1.4 Contractor shall notify the POC listed upon completion of all preparation work before applying paint. After the POC has inspected and accepted the preparation work, paint may be applied.

1.5 A primer coat will be required on surfaces with different finishes before a finished coat can be applied.

1.6 Contractor shall paint all previously painted surfaces. Paint shall be applied carefully with clean rollers and brushes to produce a complete and even coverage with a smooth finish, free of runs and other defects.

1.7 After painting is completed, the POC needs to be notified to inspect and approve work before any items are installed.

1.8 The contractor shall dispose of all waste materials properly and IAW EPA regulations at the contractors expense.

1.9 Project is subject to a site visit if needed, which will be determined at a later date.

References

3.1 All work is to be completed shall comply with Federal, State and Local Laws, Codes and

Regulations.

3.2 Manufactures’ requirements and recommendations shall be followed for all products used.

Liabilities

4.1 The contractor will be financially responsible for any and all damages incurred while working on Government property.

Coordination

5.1 All communications will go through the POC listed. No work can be modified or changed without their permission.

5.2 Work to be completed should have minimal impact on the unit’s operation. In the event any of the utilities need to be secured, these items should be well planned out.

5.3 Normal work hours for the unit is 8:00 AM to 4:00 PM, Monday-Friday, excluding all

Federal Holidays. Any work outside these times need to be worked out ahead of time.

5.4 All required License, Certificates, Accreditation Card, etc. will be submitted with the quote or bid.

Project Completion

6.1 All work completed is up to the POC’s approval. The site is to be at a minimum as clean of a condition it was found in. All trash, debris, materials removed, etc. should be properly disposed by the contractor.

6.2 This contracted will be awarded and should be completed no later than April 30, 2021.
