ATTACHMENT J-4

SMALL BUSINESS PARTICIPATION PLAN (SBPP)

All Offerors, both other than small businesses and small businesses, shall submit a Small Business Participation Plan in accordance with the Federal Acquisition Regulations at 52.219-8 & 52.219-9. Offerors should propose the level of participation of small businesses (as a small business prime and/or small business subcontractors) in the performance of the acquisition relative to the objectives/goals set forth in the evaluation of this area.

(a) Check the applicable size and categories for the PRIME offeror only -- Check all applicable boxes:

(b) Submit the total combined percentage of work to be performed by both large and small businesses (include the percentage of work to be performed both by Prime and Subcontractors):

(c) Please indicate the total percentage of participation to be performed by each type of subcategory small business. The percentage of work performed by both prime and subcontractor small businesses that qualify in multiple small business categories may be counted in each category:

Example: Victory Prop Mgt (WOSB and SDVOSB) performing 2%; and Williams Group (SDB, HubZ and WOSB) performing 3%. Results equate to: SDB 3%; HubZone 3%; WOSB 5%; SDVOSB 2%; VOSB 2%;). SDVOSBs are also VOSBs automatically; however VOSBs are not automatically SDVOSBs.

(d) List principal supplies/services to be performed by Small Businesses. Offerors shall attach evidence of commitment to the plan. Evidence includes but is not limited to signed copies of subcontract agreements, teaming agreements with subcontractors, Mentor-Protégé agreement, etc.

Changes to the below table must be pre-approved by the Contracting Officer throughout contract performance.

Example: If a small business qualifies also as a WOSB and a SDVOSB, and you can add them to each category below in which they qualify.

(e) Other than small firms, describe the extent to which small business concerns were utilized during performance on previous contracts within the past three to five years. Other than small firms shall submit copies of eSRS reports to validate past performance. Explanation of efforts where goals were not met should also be provided. Responses will not count against page limitation.

Note for Other Than Small Business Offerors:

Separate from the Small Business Participation Plan, other than Small Business offerors must also submit a Small Business Subcontracting Plan meeting the requirements of FAR 52.219-9. To be eligible for award, the Small Business Subcontracting Plan must be acceptable and shall reflect and be consistent (in terms of dollar value) with the commitments offered in the Small Business Participation Plan.
