About this file

The Surplus Material Evaluation Form is a standardized document designed to provide comprehensive information about surplus material being proposed for a federal contract opportunity. The form requires detailed documentation about the surplus item, including its identification (NSN, part number, manufacturer), condition status, warranty information, and provenance. Offerors must specify the material's condition in one of four categories: (1) new and unused, (2) new and reconditioned, (3) new and modified, or (4) used and overhauled, with accompanying details about reconditioning, modifications, or overhaul sources. The form is specifically related to a Department of the Air Force Materiel Command Air Force Sustainment Center solicitation (FA812124R0041) for a seal, air, aircraft component, and requires attachments including a copy of the original packaging label and a Request for Quote form.
