This should be used as general example of the issues that need to be resolved.
This should not be considered a complete description of all fall protection requirements.

INTRODUCTION
A visual inspection of the equipment was performed. The inspection included a review of the exposed, visible, and accessible components of the system for signs of distress.
The inspection was also based on a review of the Occupational Safety & Health Act and Regulations for window cleaning pursuant to local regulations.
This inspection report was prepared by Pro-Bel Group Ltd. and reflects the inspector’s judgment based upon information available at the time the report was prepared.
DRAWINGS
A copy of the engineer approved DRAWING WAS NOT posted at the entrance to the roof. This requirement is pursuant to OSHA regulations and generally accepted OSHA enforcement practices and manufacturer's recommendations.
Pro-Bel does not have the drawing records on file for this building and may not be able to reproduce a copy for your records. Please review your files and ensure a copy of the drawing is posted at the entrance to the roof and a copy submitted to Pro-Bel.
Window cleaners must read and understand shop drawings including restrictive notes as shown on the drawings
LIST OF EQUIPMENT
The following equipment was accessible for review at the time of the inspection, unless noted otherwise. Please refer to the system equipment locations and details.

SYSTEM DESIGN
This equipment was supplied for a system that is non-specific as the placement and intent of the equipment is not clear. Further discussion is required to review the intent of installed system.
USAGE AND RESTRICTIVE NOTES
Pro-Bel recommends that all workers read and understand the drawings in conjuction with the inspection report prior to accessing the roof or using the equipment. Workers are also responsible to produce a work plan and shall be able to demonstrate that they are able to work safely in all area.
Workers must rig in such a manner in order to protect their primary and secondary lines from being cut off or chaffing at all times.
Window cleaners must read and understand shop drawings including restrictive notes as shown on the drawings.
Important Note: Only one (1) safety line is to be attached to any one anchor at any given time.
When any parapet wall is less than 39" in height, workers must tie-off prior to approaching any anchor located within 6'6" (2 meters) from the unprotected roof edge.
Per 1910.27 (b) (1) (ii) Employers may not permit the use of anchorages unless they have written information from the Building Owner that each anchorage meets requirements of paragraph (b)(1)(i) of section 1910.27. The employer must keep this information for the duration of the job.
FINDINGS
All anchoring points appear to be snug and secure.
No deterioration of the equipment was noted.
All of the available threads were inspected and it was visually noted that all retaining bolts have a minimum of two (2) threads exposed which have been properly deformed past the nut.
The equipment on this building has a valid certification and must be re-certified on or before: 2026-Jan-19.
In addition to this roof. Several others were also inspected. The numbers below will correspond with roof numbers on map provided by hospital.

Pictures showing roof #1. There are mechanical and security cameras within 6' of edge and no parapet. It is recommended guardrail or approach anchors be installed in these areas.
Windows can be reached by using poles from ground or another roof.
Roof #2 has one camera SE corner. This camera is within 6' of edge and no parapet. Recommend approach anchor be installed.
This bldg at highest is 4 stories and can be cleaned by pole.

Roof #3 has several areas of concern. No parapet wall. Camera at SE corner needs approach anchor. Ladder close to edge, within 8'. Suggest railing in this area. The railing will also be used to safely clean windows in lieu of roof anchor at edge. Ladders over 24ft will need to replace cages or wells with fall arrest systems by 2036. (2 ladders). A fixed ladder with fall protection should be installed to access lower roof. Cable system should be installed on lower roof to access mechanical safely.
Roof # 4 no safety systems needed.

Roof # 5 Air handler/vent at edge within 6'. Recommend approach anchor. This will also be used to clean windows in that area. Ladder over 24ft on this roof also. Has cage, will need fall arrest by 2036.

Roof #6. Needs cable system to access lighting. Low parapet. A cable system will also be needed to access "bridge" safely as the parapet is low.
Roof #7. Cable system needed to approach North and east side of mechanical and also to access camera safely.
Should be noted client has all structural drawings and has assured at least 6 inch deck in these areas. Must confirm on drawings.
SUMMARY AND RECOMMENDATIONS
Pro-Bel strongly recommends that a copy of the Engineered Drawing be posted at the entrance of the roof and a copy of this drawing provided to Pro-Bel. Pro-Bel will store this drawing in your Customer Portal at no cost for future reference.

The equipment inspected can BE CONSIDERED CONDITIONALLY ACCEPTABLE FOR USE once all above mentioned concerns are addressed.
Keep this and subsequent annual reports on site along with a record of any modifications or repairs to the system.
Any unauthorized modification or changes, or damage to the equipment is the responsbility of the property manager or exterior maintenance contractor. Pro-Bel accepts no responsibility for damages if any, suffered as a result of unauthorized alterations however cause to the equipment.
