Steam & Condensate (S&C) IDIQ Contract Sources Sought Questionnaire
1. Please provide your company name, address, point of contact (POC), telephone number, fax number, email address, CAGE Code, and DUNS Number.
2. Please provide your company's business size, average annual receipts for the last three fiscal years (please indicate fiscal period), and number of years in the construction industry.
3. Please provide two or three past/current (present to up to past 5 years) S&C performance background such that you were the Prime Contractor, to include:
a. Contract identification number(s)
b. Title and a brief summary of the objective of the effort
c. Contracting agency or firm (Government or commercial)
d. Point of Contact information such as: Name/Email/Telephone Number
e. Value of the contract(s)
f. Type of contract(s), i.e., fixed price, cost reimbursement, labor hour
g. Percentage of work done as the Prime Contractor
4. Please identify the largest S&C project your company has successfully delivered (to include dollar amount, client, date completed).
5. Please identify your bonding capacity (single and aggregate) and name(s) of surety(ies).
For questions 6-11, assume an S&C program roughly like that described in the draft S&C IDIQ SOW posted to beta.SAM.gov. For these questions (6-11), Potential Offerors should assume each Task Order to be between $5,000 and $1.3M with an average of $650,000 per project. Potential Offerors should assume there will be multiple projects on going simultaneously.
6. Would you be able to provide a Project Manager with experience in the management of government, commercial, or industrial S&C projects of similar size and complexity identified.
7. Would you be able to provide a Contract Manager with a minimum of ten (10) years of documented applicable construction experience with a minimum of five (5) years being in a relevant construction management position? For similar size/scope projects, familiar with all aspects of general construction management, practices, methods, and materials?
8. Would you be able to provide a Project Superintendent on each project site that possesses experience in Construction, with some of those years as a Superintendent on projects of similar size and complexity?
9. Would you be able to provide a Contractor Quality Control Manager (QCM) with at least 5 years of documented relevant construction quality control experience?
a. Provide QCM with the qualifications listed in question #9, who has also completed a course in
Construction Quality Management for Contractors?
10. Would you be able to provide a Safety Manager that meets the requirements identified in Engineer Manual
(EM) 385-1-1 Section 1?
11. Please identify any commercial practices that the Government should consider implementing for this acquisition.
