Statement of Work

Project: Symmetra PX 40 KW and Symmetra LX 16kVA Uninterruptable Power Supply (UPS) Modular Power Revitalization Service (MPRS)

GENERAL INFORMATION

1.0 Scope of Work: The Contractor shall perform a MPRS on the Symmetra PX 40 KW UPS located in Bldg. 45001, RM 100-09 and perform a MPRS on the Symmetra LX 16kVA UPS located in Bldg.

45001, RM E-1 per each technical requirements/tasks listed in section 3.0.

2.0 Background: The Symmetra PX 40 KW UPS and Symmetra LX 16kVA UPS provide uninterruptible, backup battery power to the critical communication closets within Raymond W.

Bliss Army Health Center. These communication closets contain the data processing equipment that are vital to the Raymond W. Bliss Army Health Center organization performing administrative and electronic health record functions. Components of the Symmetra PX 40 KW UPS and Symmetra LX 16kVA UPS degrade over time and require refreshing/replacement outside of the existing preventative maintenance check contract. A modular power revitalization service (MPRS) is sought to provide a comprehensive on-site UPS refresh service, extending the useful life of the Symmetra PX 40 KW UPS and Symmetra LX 16kVA UPS.

CONTRACTOR REQUIRMENTS

3.0 Technical Requirements/Tasks. The Contractor shall:

- Confirm the components ordered, along with the ship to address, shipment date and any special site conditions.

- Coordinate the time and date of the Service Professional arrival at the customer site.

- Arrive on site as scheduled, coordinating the service delivery and any special requirements, with the appointed contact.

- Inspect the system condition.

- Move the new components from the shipping pallet location to the UPS system location.

- Safely remove the components, and stage them in a nearby location for later relocation to customer’s shipping dock. Safely install the new components, without interrupting critical operations.

- Update the system with the new component information and verify the UPS system is functioning to specification.

- Relocate all used components from the UPS System location to the customer’s shipping dock. Palletize the used components for shipment, and load them onto the shipping truck.

Ship the used components to the approved recycling location.

- Ship waste components to approved recycling and disposal locations.

- All services performed on-site shall be coordinated with the customer and executed within M-F 5:00pm – 8:00am, Saturday.

- In conjunction with the aforementioned technical requirements/tasks, render the following services for the Symmetra PX 40 KW UPS:

o MPRS of 4 X SYPM10KF and include an One Year Advantage Ultra Service Contract o Modular Battery Replacement Service of 4 X SYBT4 o 1 Year 8HR 7X24 Response Upgrade to Factory Warranty or Existing Service Contract for up to 40 kVA

- In conjunction with the aforementioned technical requirements/tasks, render the following services for the Symmetra LX 16kVA UPS:

o MPRS of 4 X SYPM4KP, 4 X SYBT5 o 1 Year 8HR 7X24 Response Upgrade to Factory Warranty or Existing Service Contract for up to 40 kVA
