DESCRIPTION OF SERVICES.

1.1. SCOPE. The contractor shall provide testing services necessary to support the 325th Force Support Squadron (325 FSS), as defined in this Statement of Work (SOW), except where specified as government-furnished property and services, at Tyndall AFB, Florida.

1.2. LOCATION. 325 FSS/FSDE, 921 Beacon Beach Road; Tyndall AFB, FL; 32403-5701.

2. APPLICABLE DOCUMENTS. Test Examiner will provide services of test examination for the following programs:

2.1. TEST EXAMINATIONS. Air Force Professional Military Education (PME) testing programs, Career Development Course (CDC) testing, Air University (AU) testing programs, Air Education and Training Command (AETC) exams, Defense Language Proficiency Test (DLPT), specialty or certification exams and any test required by the Air Force, if not administered, through the local military test control officer.

3. GENERAL REQUIREMENTS.

3.1. QUALIFICATIONS. The individual performing the work must have a bachelor’s degree from an accredited United States of America institution. Confirmation of degree completion will be provided to the ETSC before award. Contractor must have five (5) years of verifiable test specialist/test examiner experience.

3.1.1. Ability to use and maintain computer system, equipment and software is mandatory.

3.1.2 Knowledge of computer-based testing for military and civilian programs, network infrastructure concepts and troubleshooting techniques for Ethernet local area network (LAN), intranet and internet connectivity.

3.1.3. English Language Requirement. All contract personnel providing services under this contract shall read, write, understand, and speak the English language with sufficient skill to effectively communicate (orally and verbally) with government employees, educators and students.

3.1.4. Able to lift 25 pounds.

3.1.5. Contractor cannot sub-contract testing services.

3.2. WORK HOURS/DAYS.

3.2.1 The contractor will be paid per unit of work. One unit of work, normally 4 hours, will consist of administration and supervision of a minimum of two (2) to a maximum of fifteen (15) examinees. One unit of work per month may be authorized for administrative requirements directly associated with contracted duties (reports, computers software updates, test refresh/updates) with approval of Education & Training Section Chief (ETSC). Individual units will be the call of the ETSC, or his/her representative, on the basis of appointments for testing.

Also, testing sessions may be cancelled by phone with a

24-hour notice if session does not have at least five (5) examinees scheduled.

3.2.2. Contractor may not work, when the office is closed, or outside the standard work hours of 0700 to 1700 (7:00 a.m. to 5:00 p.m.), Monday through Friday. Exact days and times of services may be jointly determined by the Contractor and the Government in advance, and may be modified by mutual agreement, according to changing-program needs.

3.2.3. RECOGNIZED HOLIDAYS. The contractor is not required to provide service on the following days:

3.3. SPECIFIC REQUIREMENTS.

3.3.1. GUIDELINES. Test scheduling, administration, scoring, and computer based exams, to include all Air University (AU) E-Exam computer based exams, D e f e n s e M a n p o w e r D a t a C e n t e r ( DMDC) exams, H a z a r d o u s M a t e r i a l s ( HAZMAT) and AU online exams. Use Career Development/Student Assistance/Registrar (CDSAR) and Air University Student Information System (AUSIS) to verify student enrollment, exam scoring, exam numbers, and passwords. Contractor will access and use the Air Force Automated Education Management System (AFAEMS) for scheduling, tracking, and reporting exams as needed. Contractor must conform to all Air Force, Air University, and DMDC exam policies, guidance, and procedures to insure no test compromises and errors in test administration, scoring, and score posting. Contractor will create and keep current a testing continuity book with step by step procedures and test center guidance of all military test/exams administered within 30 days of contract start date, making updates, as needed. The contractor will provide the ETSC a copy of continuity book within 30 days of contract start date and make updates, as needed.

3.3.2. The contractor must inform the E&TSC of anticipated absence from a scheduled unit at least 24 hours prior to scheduled arrival. Longer absences may be scheduled with the coordination of the E&TSC. Such absences shall not exceed two weeks at a time or four weeks in one fiscal year.

3.3.3. The contractor shall be responsible for providing a fully-trained-substitute-service provider of equal qualifications whenever needed. Any such substitute must be pre-approved by the E&TSC or designee.

3.3.4. In performance of contract requirements, contractor is authorized access to

PME/AU and college examinations and to the security container in which they are held.

3.3.5. IAW the AU TCF Guide, the contractor must remain physically present during the entire test session unless relieved by an appropriate authority.

3.3.6. The contractor is responsible for securing and administering the following categories of tests to eligible military and civilian personnel and their dependents as authorized by AU, Air Combat Command (ACC) and local regulations (NOTE: COR, with CO approval, may request in writing additional tests be added as needed.)

3.3.6.1. All AF AU end of course (EOC) tests including but not necessarily limited to those for CDC, Certification and Testing (CerTests)/E-Tests, 7 level exams, Enlisted Professional Military Education (EPME), Air War College (AWC), Air Command and Staff College (ACSC) and Squadron Officer School (SOS).

3.3.6.2.. Tests requested to be administered by eligible personnel, such as correspondence course and qualification/certification exams.

3.3.6.3. Other tests used in local advisement or guidance programs, to include career assessment and interest inventories, as requested by the ETSC.

NOTE: Tests will be administered in accordance with instructions provided by testing institutions, testing handbooks, test examiner guides and AU instructions to ensure no possibility of test compromise. Any test compromise will immediately be reported to the ETSC.

3.4. RELATED ADMINISTRATIVE TASKS. The following administrative tasks are required in the performance of this contract in conjunction with and in addition to the actual test administration.

3.4.1. Check room prior to testing to ensure proper spacing between desks and that adequate lighting, heat, and air exist. Verify no notes or information has been left in the testing room.

Ensure testing office and testing room are kept in a condition conducive to good test performance, and that an adequate circulation of fresh air is available.

3.4.2. Complete safe and test office security forms each time the safe is opened and at the close of each test session. Ensure all exam materials (tests, CD-ROMs, test disks) are secured after each test session in the appropriate safe drawer. Verify computer test material has been removed from the computer processing unit (CPU) and secured.

3.4.3. Lock the safes and testing office doors and windows before leaving the testing office.

3.4.4. Log in all exams in the AFAEMS data base, ensuring that entries are made no later than the first duty day after tests are received. Track all tests returned, destroyed or forwarded.

3.4.5. Identify all newly arrived AU tests on the envelope, showing the name of the student, test identification number, date received, and expiration date. Enter all AU tests into the data file in the established format. Notify Unit Training Manager (UTM) of receipt of paper based mandatory CDC examinations within 3 working days of receipt. Verify in CDSAR, prior to testing, number of exam to be administered and ensure course has not expired.

3.4.6. Notify students when tests have arrived, first by e-mail and later by phone if needed.

3.4.7. Maintain an official log on all examinees testing, showing the individual’s name, the test title and the start and stop times for all test intervals. The log will be filed in office file cabinet after completion of testing for the day. Ensure each student signs this log upon receiving test and that all entries are complete and legible.

3.4.8. Require all examinees to present military identification and be in military uniform before allowing them to test.

3.4.9. Check test materials for marks/missing pages before issuing them to students.

3.4.10. Instruct all examinees at the beginning of each session as follows:

3.4.10.1. To read carefully and follow instructions exactly as they are printed in the test booklet/on the computer screen.

3.4.10.2. To adhere to any prescribed time limits. (The contractor will ensure that all timing limits are adhered to and the examinee is aware of the time requirements for each test.)

3.4.10.3. Review each answer sheet as it is submitted by the examinee to ensure proper completion and that information is correctly annotated. Ensure no testing information is taken from the testing room by collecting and shredding all scrap paper.

3.4.10.4. Check each reusable answer booklet as it is returned for marks and erase completely.

Check all booklets prior to and after testing for missing pages and enter the time they are returned in the test log.

3.4.11. Discard obsolete materials within 7 (seven) days of receipt of new publications and ensure dated materials/exams are returned by expiration deadlines. Test examiner should destroy used or nonessential AU CE’s following timeline guidance by shredding after conducting a test inventory with the ETSC. Verify AU status as required through CDSAR. Annotate on the test inventory log the course exam, control number, reason for destruction, date of destruction, and signature of person performing the destruction in accordance to TCO policy. No exams will be destroyed without the ETSC being present.

3.4.12. Inform ETSC immediately of any irregularities, which may represent the potential for a test compromise or complaint from a testing candidate. Report any computer related malfunctions to the ETSC immediately.

3.4.13. CerTest/E-Test /Administration. Follow administration directions provided for computer based testing. Must be able to utilize answer sheet manager (FTP) to forward test results, be able to score and print exam results and ensure examinee is provided the correct exam.

Keep abreast of changes or downloads required by checking web site weekly and verifying updates have been completed by maintaining a log indicating date of update.

3.4.14. Maintain familiarity with the AU catalog/web site ensuring currency of all test instructions and policies. Keep Base Training Manager (BTM) staff informed of testing changes.

3.4.15. Maintain statistical data related to test administration as required by ETSC and assist with reports and PowerPoint slides as requested. Such data will include (but not necessarily be limited

to) monthly statistics on types of tests administered and to whom (by rank and service) and any additional test related data required for semi-annual and annual reports.

3.4.16. Maintain all document and correspondence files in a neat and orderly manner. Separate files by calendar year. Contractor will be responsible for making file labels and setting up the file for each new calendar year before testing begins for the following year.

3.4.17. Refer all questions on schedule changes or testing policy to the ETSC and all questions on interpretation or application of test results to the counselors.

4. MATERIALS AND EQUIPMENT. The government will provide the computer equipment required to operate the computer networks explained in the contract. Office supplies and materials will also be provided by the government.

4.1. TRAINING. DoD/AF training may be required to fulfill contract obligations. AF will pay contractor’s training fees and attendance time if, as result of the training, it enhances the performance provided by the contractor and it benefits the Government. This training must be coordinated through the Contracting Officer prior to the start of the training.

5. SECURITY REQUIREMENTS. PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

5.1. Key Control. (IF APPLICABLE) The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government.

5.1.1. The contractor shall immediately report to the ETSC any occurrences of lost or duplicated keys.

5.1.2. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, the government shall replace all locks and keys for that system and the total cost deducted from the monthly payment due the contractor.

5.1.3. The contractor shall prohibit the use of keys, issued by the government, by any persons other than the contractor and the opening of locked areas by contractor to permit entrance of persons other than contractor engaged in performance of contract work requirements in those areas.

5.2. Lock Combinations. (IF APPLICABLE) The contractor shall control access to all government provided lock combinations to preclude unauthorized entry. The contractor is not authorized to record lock combinations without written approval by the government Program Manager. Records with written combinations to authorized secure storage containers, secure storage rooms, or certified vaults, shall be marked and safeguarded at the highest classification level as the classified material maintained inside the approved containers. Must provide combination to ETSC.

5.3. BASE COMPUTER NETWORK ACCESS. Contractor personnel are required to have access to the government LAN. The government and the contractor personnel shall follow the requirements in accordance with AFFARS 5352.242-9001- COMMON ACCESS CARDS (CACs) FOR CONTRACTOR PERSONNEL (NOV 2012).

5.3.1. The contractor shall achieve the same level of trustworthiness for its personnel as required for Government personnel with similar access to automated information systems (AISs) and networks containing DoD Sensitive Information (SI). The contractor shall meet the personnel security requirements for Automated Data Processing (ADP) access in DoD 5200.2-R, Personnel Security, for employees or subcontractor’s employee that require access to Government (IT) systems or DoD SI, or are involved in developing, delivering, or supporting IT systems and services, or safeguarding DoD sensitive information within Government or contractor systems.

The contractor shall classify ADP/IT or related positions, submit appropriate paperwork for background investigations or proof of a favorable adjudication, ensure individual receive requisite training and document compliance. Personnel background investigation and training must be initiated before access to DoD AIS/networks or DoD SI is allowed. Following contract award, details for completing and forwarding forms for requesting an investigation will be coordinated with the Government IT Security Officer designated for the networks and systems being accessed.

5.4. Tyndall AFB FL Access and Computer/Network Access. The contractor must meet the Tyndall AFB FL access requirements defined by the Security Forces Squadron prior to the start date by visiting the base visitor control center. Questions regarding access criteria should be directed to the visitor control center. Once access is approved and gained, the unit security manager will conduct a National Agency Check with Written Inquiry (NACI) at a minimum (other background information may be required), before access to government workstations that have Air Force email systems and networks containing DoD Sensitive Information (SI) to perform contracted work. Contractor personnel unable to meet the base access requirements or receive an unfavorable (National Agency Check) NAC will not be granted access to the base or systems and will be terminated. All background checks for network access and entry to Tyndall AFB FL are at no cost to contractor.

5.5. Security Training. The contractor will receive initial and recurring security education training via computer based training. Training must be conducted IAW DOD 5200.1-R, DOD Information Security Program Regulation, and Air Force Guidance Memorandum to AFI 31- 401, "Information Security Program Management". Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, The Air Force Installation Security Program.

5.5.1. Retrieving Identification Media. The Contractor shall retrieve all identification media, including vehicle pass from employee who depart for any reason before the contract expires; e.g.

terminated for cause, retirement, etc.

5.5.2. Weapons, Firearms, and Ammunition. Contractor employees are PROHIBITED from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately owned vehicle while on Tyndall AFB, FL.

5.5.3. For Official Use Only (FOUO). The contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

5.5.4. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information.

5.6. TERMINATION. The contract may be terminated in accordance with the termination clauses contained within the contract.

5.7. CONTINUATION OF ESSENTIAL DOD CONTRACTOR SERVICES DURING

CRISIS IAW DoDI 3020.37. Contractor Personnel Authorized to Accompany the U.S. Armed Forces, it is the responsibility of the Functional Director/Functional Commander to make the determination of the Test Examiner services to be essential/non-essential. This service is not determined to be essential for performance during crisis. In the event of crisis, the contractor will be notified by the Contracting Officer of the need to discontinue services due to base closure or other causes. If notification to discontinue services is not received, the contractor will be expected to continue performance in accordance with this SOW.

5.8. RECORDS MANAGEMENT.

5.8.1. All records, files, and documents, regardless of media (e.g., paper, electronic, etc.), as described in this SOW, which are the responsibility of the contractor are the property of the Government and shall remain so upon termination or completion of the contract. Complete records management training, as necessary.

5.8.1.1. The contractor shall keep these item(s) current and maintain and dispose of them in accordance with the requirements established in AFI 33-322, Records Management Program, AFMAN 33-363, paragraph 6.4, AFMAN 33-364, paragraph 2.10 and the Air Force Records Disposition Schedule which may be accessed from the following web address:

5.8.1.2. Records shall be turned over to the Government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act.

5.9. SUBMISSION OF INVOICES. The contractor shall submit and process invoice for payment use the Wide Area Workflow (WAWF) in accordance with DFARS 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports. Contractors shall access and register with the WAWF system by visiting https://wawf.eb.mil/.

The Contracting Officer Representative (COR) of the Force Development Center will verify that all services have been performed in a satisfactory manner and accept services performed by generating, signing, and processing by the system within three working days for payment.
