About this file
This Statement of Work (SOW) details laboratory supply requirements for blood culture collection devices at Gallup Indian Medical Center in New Mexico. The facility estimates collecting 3,500 blood cultures from September 2025 to August 2026, with orders scheduled approximately 4 times per calendar year. The vendor must supply three specific product variants: 21 gauge safety slide needles, 23 gauge safety slide needles, and peripheral IV connectors with 6' extensions, each to be packaged in 200-count cases. Key requirements include maintaining a blood culture contamination rate below 3%, providing sterile packaging with one device per package, offering no-cost training, ensuring compatibility with existing medical equipment like Becton Dickinson Bactec bottles and Vacuette/Vacutainer needles, and adhering to U.S. Department of Transportation shipping guidelines for medical materials.

Statement of Work - Blood Cx Collection Diversion Device
Laboratory supplies are required to collect blood samples for diagnostic testing in the laboratory of Gallup Indian Medical Center, 516 East Nizhoni Blvd., Gallup, NM 87301. Blood sequestration devices are used to prevent blood culture contamination during the blood collection process.
Current standards dictate a blood culture contamination rate of less than 3%. These materials will be required on an as needed basis with orders being scheduled approximately 4 times in the calendar year. Blood culture contamination is monitored in the facility’s laboratory on a monthly basis. We estimate that this facility will collect 3,500 blood cultures during a 12 month period from September 2025 to August 2026.
The vendor must be able to reliably supply quantities of the aforementioned product sufficient to meet the estimated demand. Variant options for the device should include:
1. 21 gauge slide-needle variant
2. 23 gauge slide-needle variant
3. Peripheral IV connector w/ 6’ ext. for use concurrent with IV initiation
Payments to any subcontractor of the vendor are the sole responsibility of the vendor.
Vendor must package their supplies in a sterile manner. Each package should include only one blood culture collection device.
Any training provided by the vendor is to be offered on a no charge basis. The vendor will be strictly responsible for any training, travel or, per diem costs incurred. These efforts to be coordinated with Gallup Indian Medical Center’s nurse educator.
Representatives of the vendor are to register all visits with the Gallup Indian Medical Center Security department on entry to the facility. Vendor trainers and representatives are not to oversee actual patient collections without proper HIPPA and Privacy Act training, nor without written approval of the GIMC privacy officer.
Versions of the supplies required are listed below estimated as follows:
