School Psychologist Related Service Contract

Scope of Work

Psychological Services is an essential component of the Tuba City Boarding School COVID-19 Reopening Plan. Tuba City Boarding School is to offer services to students and their families to help with the adjustment period of returning to school. The students have been isolated at home for the last eighteen months and the return to a face-face social situation requires a support system to be in place.

Statement of Work:

• Contractor shall give direct psychological services to student and families regarding the impact of COVID-19 on the family unit.

• All student’s evaluation and services will be logged.

• Contractor shall consult and work with TCBS staff to provide optimum student services.

• Contractor shall be available for staff training, to give information in the area of expertise.

Assessment Services:

• Contractor shall establish a process to help identify and provide support to students and/or families that are at higher risk for significant trauma or stress from COVID-19.

• Contractor shall guide students and families in developing calming techniques during this unsettling time and develop support groups for families to heal.

• Contractor shall teach/educate students to articulate their feelings and provide a safe place for them to talk.

• Contractor shall provide support to teachers who are working with students experiencing significant trauma.

• Contractor shall provide information to parents who need community resources to support their child.

• Contractor shall collaborate with the counseling staff to provide a comprehensive support plan for referred students.

Psychologist Therapy Service Summary:

Contractor shall sign in at front office for attendance and cross-referencing.

Invoice shall contain the following information:

Contractor shall submit a monthly-summarized invoice.

Invoice shall contain the following information:

a) Purchase Order Number

b) Date of Service

c) Initial of Students

d) Time Spent with students

e) Evaluation information to include student initials.

Contractor shall be knowledgeable in the following areas:

Culturally sensitive strategies as it relates to Navajo and Hopi Cultures.

Treatment for anxiety, depression, and stress-related trauma.

Developing a comprehensive support plan with the family, student, and teacher.

Contractor shall comply with the following requirements:

At least, two years’ experience providing psychological services.

Licensed in the State of AZ as a psychologist.

A completed background check with the Federal Government.

Registered in SAM.Gov

Location of services site: Tuba City Boarding School 306 Main Street, Tuba City, Arizona.

Therapy Session Dates: Thursday & Friday , 8 am -4pm.

The COR will be provided with full schedule, and any deviations must be approved by the COR.
