Statement of Work
#610 LIMESTONE, #57 LIMESTONE and 200# RIP RAP AGGREGATE Cat Island NWR
#610 Limestone Delivery to Cat Island NWR:
POC: Mike Seal – 318-719-1416 (cell)
Purchase and deliver 600 tons of #610 gray limestone to improve locations on Cat Island NWR c/o Southeast Louisiana Refuges Complex, Lacombe, LA. All materials are to be stockpiled (see attached map) on the Cat Island Refuge as indicated on the attached map.
For more detailed information on locations, types, and amounts, please contact POC listed above.
Location delivery point: is approximately 5 miles from the Cat Island Refuge entrance gate (see attached map).
Cat Island National Wildlife Refuge is headquartered at 61389 Hwy 434, Lacombe, LA, 70445 (Phone number 985 882-2000); however, delivery locations are identified and shown on the attached maps.
Delivery will NOT be made to the headquarters address as identified in Lacombe, LA.
Location (drop point):
1. Cat Island: From St. Francisville, LA: From Commerce St. take Ferdinand St. one mile and take a right on Mahoney Rd. Go three miles, cross the low-water crossing bridge and take a left on Creek Rd. Take a slight left to stay on Creek Rd. Creek Rd continues to the Cat Island NWR Entrance. Be aware that this route may not be accessible when river levels are high.
Once delivery begins, a minimum of 200 tons will be delivered daily, during regular working hours (7:30 AM to 4:00 PM Monday through Friday). Delivery will continue on consecutive days until the order is complete. Delivery will not be made on weekends or when conditions are so wet that delivery will damage existing roads. The delivery to each site or group of sites will be made in entirety before moving to new sites to make allowance for refuge personnel to travel to the site for delivery assistance. Any scale weight tickets will be given to the refuge representative for all deliveries and the actual cost will be by tons delivered.
ROADWAY SURFACING MATERIAL – 610 Limestone

2. Attach Material Description (i.e., gravel, aggregate, etc.). #610 Limestone.
3. Total tons/Yard Required: 600 tons of #610 limestone
4. Check one or more: Material will be Stock Piled X ( spot dumped)
________Tailgate spread thickness: __________
5. Maximum/Minimum Amount to be delivered daily: 300 tons maximum to 200 tons minimum for the #610 limestone
6. Attach a delivery or /or local site map. A map is attached.
7. Are there State certified scales in the area? ____ Yes X No NOTE: If yes, weight tickets are required for each delivery. If no, delivery must be by cubic yards.
8. Required commencement date for delivery: 9/18/2020
9. Last date for delivery: 10/30/2020
10. Local conditions (bridge restrictions, weather conditions, water levels, road flooding, etc.) that may affect delivery: Mississippi/Louisiana bridge under construction and down to one lane of traffic only.

Purchase and deliver 600 tons of #57 gray limestone to improve locations on Cat Island NWR c/o Southeast Louisiana Refuges Complex, Lacombe, LA. All materials are to be stockpiled (see attached map) on the Cat Island Refuge as indicated on the attached map.
